Statement of Work OR Lights Surgical Platform at VA Audie Murphy San Antonio, TX The VA Audie Murphy facility in San Antonio, TX, is embarking on a critical project to renovate Operating Room 2 (OR 2). The primary aim of this renovation is to install new OR equipment, specifically Booms and Lights, thereby modernizing and enhancing the operating room's capabilities. This statement of work delineates the comprehensive scope, management, interim infection control, labor, structural, and electrical tasks necessary for the successful completion of this project.
Scope of Work:
The VA Audie Murphy facility’s Operating Room 2 renovation encompasses pre-installation services for Booms and Lights. The renovation work is expected to occur during normal working hours and must be completed in a single phase. Any alterations to the stated scope will require approval and may incur additional costs.
Existing and New Equipment:
The project involves the removal of the current medical equipment, which includes a Classic Equipment Boom, a Classic Anesthesia Boom, two Visum Dual Suspension LT/FP units, and an SPI3 in Stryker Doc Station. The new equipment installation will feature an Anesthesia Boom, two Dual Suspension LT/FP units, and a COR IP in Room integrated into the existing Stryker Doc Station.
Project Management:
To ensure smooth progress, the project will involve meticulous coordination with all relevant departments, personnel, and contractor staff. An on-site supervisor will oversee the entire project, ensuring that all activities adhere to the predetermined schedule. This includes coordination of all deliveries, inspections, and necessary certifications, as well as obtaining all required electrical documentation and permits pertinent to the electrical work involved.
Interim Infection Control:
A comprehensive infection control plan will be implemented to minimize any health risks during the renovation. This involves installing Class IV infection control measures, sealing off diffusers with tape or plastic, and venting air scrubbers into return air vents. Additionally, a plastic ante room will be constructed within the OR, with sticky floor mats to ensure clean entry and exit. The hospital will be responsible for removing mobile equipment and covering immovable units with plastic to shield them from dust.
General Labor:
The project labor will encompass the provision and management of a dumpster on-site for debris disposal, the removal and disposal of specified outdated equipment, and the installation of rack fans and square speakers. All debris will be disposed of in the designated contractor provided dumpster at the hospital.
Structural Work:
The renovation assumes that existing structural mounts are adequate for the new equipment and can be used as-is. A deflection test will be performed once the new equipment is installed to verify structural integrity. Any necessary rework of inadequate structures will be subject to additional fees. The project also includes the installation of contractor-provided mounting plates.
Electrical Work:
Electrical tasks will involve relocating existing circuits, installing junction boxes for lights and monitors, and laying down conduits as specified. Final electrical connections will be made to the new OR Lights, Monitors, and Booms. Additionally, existing data in the anesthesia boom will be re-terminated.
Plumbing and Med Gas:
The project includes capping the nitrogen line at the old equipment boom location, rerouting existing med gas piping, demoing existing med gas risers, and installing new med gas risers provided by the contractor. All medical gas systems will be third-party tested and certified to ensure compliance and safety.
Mechanical and HVAC Work:
No significant mechanical or HVAC work is anticipated beyond the infection control measures. The hospital will assist with airflow damping if necessary.
Finish Work:
Finishing work will include patching and painting areas affected by the renovation, trimming out ceiling speaker locations, and preparing the room for a terminal clean. The hospital will provide the matching paint color for all touch-up work, ensuring a professional and cohesive appearance in OR 2.
Schedule and Exclusions:
The renovation project will follow a detailed schedule submitted prior to commencement, accommodating normal working hours without premium wage rates and excluding holiday weekends. Any changes to this schedule may incur additional fees. Notably, the project excludes the furnishing and installation of equipment fire suppression, post-tension scanning, asbestos or mold abatement, and other work not explicitly outlined above.
Pricing:
Prices are effective for 90 days and do not include federal, state, or local taxes. Contractor reserves the right to cancel a purchase order if installation is not completed within 30 months of the issuance. Partial shipments and invoices for products are acknowledged and agreed upon by the customer upon issuing a purchase order.
The outlined statement of work holistically addresses the necessary tasks and responsibilities essential for the renovation of Operating Room 2 at the VA Audie Murphy facility. This comprehensive approach ensures that the facility will meet its goal of modernizing the operating room to provide superior healthcare services effectively.

(A) The date fixed under this contract.
(B) The date of the first written demand for payment, including any demand for payment resulting from a default termination.

In the event of such termination, the Contractor shall immediately stop all work hereunder and shall immediately cause any and all of its suppliers and subcontractors to cease work. Subject to the terms of this contract, the Contractor shall be paid a percentage of the contract price reflecting the percentage of the work performed prior to the notice of termination, plus reasonable charges the Contractor can demonstrate to the satisfaction of the Government using its standard record keeping system, have resulted from the termination. The Contractor shall not be required to comply with the cost accounting standards or contract cost principles for this purpose. This paragraph does not give the Government any right to audit the Contractor's records. The Contractor shall not be paid for any work performed or costs incurred which reasonably could have been avoided.
(m) Termination for cause. The Government may terminate this contract, or any part hereof, for cause in the event of any default by the Contractor, or if the Contractor fails to comply with any contract terms and conditions, or fails to provide the Government, upon request, with adequate assurances of future performance. In the event of termination for cause, the Government shall not be liable to the Contractor for any amount for supplies or services not accepted, and the Contractor shall be liable to the Government for any and all rights and remedies provided by law. If it is determined that the Government improperly terminated this contract for default, such termination shall be deemed a termination for convenience.
(n) Title. Unless specified elsewhere in this contract, title to items furnished under this contract shall pass to the Government upon acceptance, regardless of when or where the Government takes physical possession.
(o) Warranty. The Contractor warrants and implies that the items delivered hereunder are merchantable and fit for use for the particular purpose described in this contract.
(p) Limitation of liability. Except as otherwise provided by an express warranty, the Contractor will not be liable to the Government for consequential damages resulting from any defect or deficiencies in accepted items.
