All work must comply with OSHA, NFPA, and Joint Commission standards. The contractor must schedule work in writing with the Contracting Officer's Representative a minimum of five business days in advance. The price schedule requests a unit price for one job (JB) to replace the door and frame, with specifications referenced in the SOW Paragraphs 1.a.i and 1.a.ii. The amendment includes 24 questions and answers clarifying scope, hardware specifications, and painting requirements, and provides photographs of the door to be replaced.

STATEMENT OF WORK (SOW)
FIRE DOOR REPLACEMENT AND INSTALLATION
SCOPE OF WORK
1) The general contractor and its sub-contractors shall furnish all labor, materials, supervision and all other provisions necessary to replace one (1) fire/external door located at the Corpus
Christi Outpatient Clinic in Corpus Christi, TX. Delivery and work will occur during normal business hours, Monday - Friday from 8:00am – 4:00pm. Delivery/installation and all communication will be coordinated with the VA’s Point of Contact (VA POC).
a) The contractor shall provide the following:
i) Removal, disposal, install replacement of one (1) external swinging steel door and its frame due to rust damage on frame, frame missing filler plate(s), and exposed mineral core to the door. Reference failed door ID# 11505680 in
Appendix A.
ii) Replacement of Door ID# 11505680
1. (1) Door replacement(s) shall be steel, 90 min fire rating. 2-hour fire-rated door shall meet NFPA 80 Standards and equipped with an exit device or panic hardware.
2. (1) Door frame for external exit door listed above which is damaged.
3. Required parts to perform corrective actions for all deficiencies listed in the deficiency report associated with this door.
b) Work shall include the following for the door:
i) One-year warranty for parts and labor on all parts associated with installation.
ii) Removal and disposal of existing door and site cleanup.
iii) Contractor shall inspect, test and verify proper operation of new notification devices.
Verify all doors, frames, and exit devices meet the requirements of all Joint
Commission, NFPA, state, and/or local codes.
iv) Provide report of all replacements completed. Indicate on the client life safety prints the location and address of each fire door replacement.
v) Once compliance is determined, affix labels to the fire rated doors, frame, and / or fire exit stating that the designated fire doors are compliant.

3) All work must be done according to industry standards. Work in the facility must meet the minimum criteria for OSHA, National Fire Protection Agency (NFPA), and The Joint
Commission requirements.
4) Contractor shall ensure that the equipment functions in conformance with the latest published edition of the applicable standards, such as: NFPA, UL, OSHA, The Joint Commission, VA, etc. and the “Manufacturer's Performance Standards / Specifications” as used when the equipment was originally procured and that any upgrades/updates shall meet or exceed the stated standards/specifications.
5) The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick-off meeting or has advised the contractor that a kick-off meeting is waived.
6) The contractor shall schedule with the COR in writing a minimum of five (5) business days in advance of the work.
7) Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
8) Information System Officer, Information Protection: The contractor will not have access to
VA Desktop computers. They will not have access to online resources belonging to the government.
9) Privacy Officer: The Contractor will not have access to protected Patient Health Information
(PHI) nor will they have capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage device will remain in VA control and will not be removed from VA custody. All research data available for Contractor analysis is de-identified.
10) Records Manager: There will not be federal records created, maintained, used or dispositioned with this contract; regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.].
APPENDIX A
A.3 QUESTIONS AND ANSWERS
1. The provided project scope appears to have some confusing and conflicting information.
The scope mentions schlege entranc function hardware. The pictures contain single metal doors, and the double doors are wood with Von Duprin surface vertical rod hardware. I am hoping for a door and hardware schedule or scope that speak to specifically what you would like included in the quote.
A. We do not have a door hardware schedule to provide. The door replaced will need to be equipped with a new door hardware matching what exists in the pictures (i.e. exit device, deadbolt, physical security features, etc.).
2. Can you verify the wall thickness?
A. 12” – 12-1/2”
3. Are the existing doors under warranty from the previous install?
A. Please see revised SOW for project scope. The door within the scope is not covered under warranty.
4. The provided project scope appears to contain some conflicting information. The scope references Schlage entrance function hardware; however, the provided images show single metal doors, while the double doors appear to be wood with Von Duprin surface vertical rod hardware.
- Could you please provide a detailed door and hardware schedule, or clarify the specific scope requirements, so we can ensure our proposal accurately reflects your expectations?
A. We do not have a door hardware list or schedule available. Please refer to the provided report and photographs to assess the required components.
The VA expects that both the door and frame will be replaced to address the identified deficiencies, while preserving their existing functionality.
5. Please confirm, as it seems to be a contradiction in two attachments: door measurements and scope of work:
● Is it two sets of Doors: One set is interior double door and the second set is exterior single door?
A. The scope of work is only to replace the exterior door. Interior double doors are not in the revised scope of work submitted.
6. Please provide specs of door hardware for the exterior single door, such as panic bar, rod, lever, closure?
A. At this time, we do not have specific hardware schedules or detailed specifications available for the exterior single door hardware. The selected hardware should match the existing configuration as shown in the pictures included in the report and below. Please refer to those images for reference regarding the type and configuration of the panic bar, rod, lever, and closure.
7. Is the exterior single door for the latch side (from interior) to have a panic bar exit device?
A. The door needs to match existing hardware and security features.
8. Is the exterior single door for the outside exit device trim (from exterior) to have a night latch pull with cylinder?
A. See response #6.
9. Is the exterior single door to have a glass lite kite? If yes, what size?
A. No, the door needs to match existing configuration.
10. For the exterior single door, does the threshold need to be replaced?
A. No, only deficiencies identified in the report need to be replaced.
11. Is the new exterior single door to be hollow metal door, with a metal door frame?
12. Will the new exterior single door be painted? If yes, what color?
A. The exterior door will need to be factory painted with a brown weather-resistant coating.
13. Will the new exterior single door frame be painted? If yes, what color?
A. The exterior door frame will need to be factory painted with a brown weather-resistant coating.
