1. The purpose of this Amendment is to provide Answers to Questions submitted by several vendors.

1. Q. I am writing to request information regarding the frequency of site changes for the PET/CT scanner in North Texas, and how often the trailer would need to be moved. Also if there are any isotope/drug delivery contracts already in place at each location or if we need to provide our own radiopharmaceuticals as well as rubidium generators for the cardiac Rb82 exams.

A. The plan is for the coach to be in FW at least 2 days per week. We will order radiopharmaceuticals off our contracts, however, Vendor will be responsible for providing the Rb82 generator.

2. Q. Scope of work indicates minimum of two (2) days per week. Which sites would be the basis of that minimum? A. FW Outpatient Center.

3. What is the current mobile digital PET/CT service schedule by site? A. Current contractor provides services on Tuesdays at the FWOPC.

Is there an award number able to be provided? A. No.

4. Q. What is the minimum weekly service frequency per facility? And what is the notice period to move from 2 days up to 5? A. 2 days and 60 days’ notice period

5. Q. The Scope of Work makes reference to a preference for vendors that are “able to support Cardiac PET using Rb82”. Will the VA be providing its own Rubidium Cart? If not, is the VA aware of any Rb82 Generator vendor providing per dose pricing for Rubidium? A. Yes.

6. Q. Confirm VA is aware that Cardiac Perfusion exams (with Rb82) require contractor personnel involvement (i.e., nurse, physician) onsite radioactive waste disposal, and cardiac specific interpretation software (Cedars, Emory, etc.). A. Yes.

7. Q. Based on the location of each site, the FDG pricing may be different to account for delivery costs. Please identify the number of doses expected from each site listed in the Solicitation. A. VA will order all radiopharmaceuticals, with the exception of Rb82, off of our contracts.

8. Q. The solicitation references both Q technology and LYSO crystal detectors. Q technology is BGO crystal-based technology. Please confirm vendors can propose PET/CT scanner to best match either the current digital PET/CT in use inside the VA North Texas Health Care System or the current mobile digital PET/CT in use at the Fort Worth VA Outpatient Clinic. A. Yes, LYSO (Q-technology does not refer to BGO).

9. Q. Do all the listed facilities currently have mobile pads and power? A. Yes.

10. Q. Clarification on days to start from award (60 or 90, both are shown) A. This information was submitted based on time the SOW was prepared, at this moment we need it be available ASAP.

11. Q. The estimated number of moves per year? A. Requesting mobile PET/CT Oncology and Cardiac Services for VA North TX for a minimum of 2-business days per week. It is anticipated the coach will be on site for each clinic day needed. Past Contractors have moved to service other healthcare systems on days they are not at the VA.

12. Q. Any special requirements for setup at each location? A. Network connections will be setup and coordinated between Contractor and VA North TX to allow for data transfer.

13. Q. Can the quote provide a monthly rate plus mileage for moving the unit to different locations? A. No.

14. Q. Or pricing to include monthly fee and separate flat fee for each time the unit will need to be moved? A. No

15. Q. Confirm if the unit's maintenance schedule and servicing are to be included by the contractor? A. Yes, the contractor is responsible for the maintenance of the unit by an OEM FSE, including all Preventative Maintenance (PM), and all Parts and Labor and any related physics testing.

16. Q. How far in advance can schedules be changed or adjusted? A. Approximately 30-days to allow for alerting any future scheduled patients.

17. Q. What is the protocol for urgent or stat scans? A. VA North Texas will assume responsibility for scheduling any Urgent studies at any of our sites, including the mobile coach. There are no inpatient STAT PET/CT studies anticipated.

18. Q. Please confirm whether staffing is to be included with the mobile MRI unit or billed separately? A. VA North TX is requesting Contractor to provide staffing for a PET/CT unit, including PET certified technologists. Please note MRI unit is not requested here.

19. Q. Are utilities (internet, HVAC) included or separate? A. Must include HVAC system that meets manufacturer’s requirements. Contractor will connect to VA network/internet when onsite, details provided in PWS.

20. Q. What space or site requirements are needed to park and operate the unit? A. Details provided in PWS.

21. Q. Does the facility assist with permitting, site prep, or pad construction? A. Yes.

22. Q. How is the unit secured after hours? A. This will be a Contractor’s responsibility.

23. Q. Please provide pictures of the setup area, connections, or may a site visit be granted? A. Primary location of Service will be the Fort Worth Outpatient Center and yes, site visit may be granted.

24. Q. What size/type of truck will fit to deliver the unit, and what height clearance is required? A. Must provide industry-standard mobile unit; typical minimum size is 8’ 6’’ wide x 48’ long to accommodate full access inside trailer and around PET/CT scanner; please refer to PWS for additional details.

25. Q. Will the facility assist the contractor with EMR integration or PACS connectivity? Please confirm which computer operating system/version and connections will be used. A. Yes site will facilitate PACS connectivity; Please note that all data must be deleted prior to leaving a VA-site.

26. Q. What are the addresses of the Additional VA sites?

29. Q. Does the lead time allow for an alternative proposal for a different piece of equipment while we outfit the requested equipment? A. Yes, a bridge solution is acceptable.

30. Q. Can the COR provide a site visit or photos of existing site conditions? A. Yes, and site visits can be coordinated as well.

31. Q. Is there a current incumbent and what equipment are they currently utilizing? A. Yes, Digital UMI 550.

32. Q. Who is in charge of handling the Radioactive Material licenses? A. VA North TX .

33. Q. When moving the equipment to multiple locations, approximately how many moves will the machine be moved each month/year? And have all the sites already been prepared with the appropriate site preparations to accept and power a mobile? A. Initial service will be Fort Worth and additional sites are pending.

34. Q. It is noted that the primary location is the outpatient clinic – are the additional sites where technology already exists or are we moving the machine there? A. Additional sites are not setup yet.

35. Q. Will the contract allow for an EPA for pharmaceuticals with the current climate? A. The Government will not be using pharmaceuticals, will be using radiopharmaceuticals. The VA plans to order all radiopharmaceuticals off internal contracts (with the exception of Rb82), as the Government has much more favorable pricing than the private sector.
