This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 36C25723Q1389 is issued as a request for quotation (RFQ). The STAT reference laboratory testing being requested are for the Amarillo Veterans Healthcare System laboratory, located at:

The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition.

The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The evaluation criteria are technical acceptability and price. Under technical includes the ability to provide all of the requested items in accordance with the Statement of Work and meet the minimum criteria listed under the clause in the attached solicitation. THIS SOLICITATION IS FOR BRAND NAME ONLY.

To be considered, offerors are to complete Section B.3 in the attached solicitation document, provide a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, provide a letter of authorization (if not the manufacturer) with its offer.

The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. There are no addenda to this clause.
