B.3 STATEMENT OF WORK

Ceiling lifts for Garland VAMC CLC and Upgrade in Dallas VAMC SCI

A. General Information

1. Title of Project: Ceiling lift Purchase and Installation in Garland VAMC CLC and Upgrade in Dallas

VAMC SCI

2. Scope of Work: The contractor shall provide all labor, supervision and all other resources required to deliver the requirements stated herein, except as may otherwise be specified in this statement of work. 550-pound advanced ceiling lifts as specified (with fast motion, continuous-charge room-covering rail systems, and tool-free emergency lowering, plus scales or positioning locks where specified) are to be delivered, installed, and tested, in places listed in the location list. This project also includes upgrading one standard lift to a trainer module with positioning locks in the Dallas Spinal Cord Injury (SCI) physical therapy room – the lift that is upgraded there will be brought to Garland to use as one of the lift motors in Garland. All work must be coordinated with COR and Supply Chain Management Services in order to coordinate paperwork, and all new lift motors must be taken to PPM/Supply Chain Management in the basement for inventory record/sticker purposes. New continuous-charge rail systems will be required. Locations are in Garland VAMC, 1NW area, 2300 Marie Curie Drive, Garland TX 75042.

Installation will need to be timed in cooperation with Garland Engineering. Details are provided in sections C and D below, Specific Mandatory Tasks and Associated Deliverables. If proposed cost exceeds budget, VA will prioritize and specify final lift locations.

3. Background: Ceiling lifts have been installed in many places throughout the VA North Texas Health Care System. VHA Directive 1611 requires that staff use SPHM technology to keep from lifting over 35 pounds of patient weight, and in areas requiring frequent lifting and mobilization of patients, including CLC patient bedrooms, bathrooms, and therapy gyms, ceiling lifts on room-covering traverses are preferred because they require less space and time than floor lifts, provide more opportunities for repositioning and mobilization than floor-based lifts or straight rail systems, and require less force to move around / cause fewer injuries than floor-based lift equipment. The VA SPHM Criteria that are published on the VA Construction and Facilities Management Technical Information Library (VA CFM TIL) recommend connecting the lift in a bedroom to the lift in a bathroom, but in retrofitted spaces like Garland CLC where the ceiling height of the bathroom is much lower than the bedroom ceiling height, separate lifts are acceptable.

These systems integrate into an existing health care system in which Guldmann ceiling lifts are used to lift, transfer, and reposition inpatients and outpatients. Patients may arrive at any of these locations on a sling or be sent on to another location on a sling applied at these locations.

Staff at this location may also serve other locations, and our goal is to match the lifts in the same room/unit to avoid misuse of unfamiliar equipment. These units may also receive patients wearing assorted brands of slings from home or may need to use slings from Barton/Humancare, Guldmann, Handicare, EZ Way, and/or Alpha Modalities to lift patients and send them to other departments.

Dallas SCI Physical Therapy has an existing GH3 lift which needs to be upgraded to a Trainer Module with double positioning locks. The Guldmann Trainer Module allows offloading of a consistent amount of weight while in motion to assist patients in getting stronger, and the double positioning locks allow the lift to be held still at any one point for exercises and then released for full motion within the space.

4. Performance Period/Schedule: The contractor shall start the work required under this SOW by about June 2022 with the goal of allowing the CLC to open in July 2022, and coordinate installation with Garland VA Engineering for room availability unless otherwise directed by the Contracting Officer (CO). There may be minor construction or patient care in the area that will require a coordinated schedule. The schedule may need to be altered by emergencies including virus-related restrictions to access. Installation dates and designs must be coordinated with Contracting Officer Representative (COR), and dates for installation in locations being remodeled may be adjusted by the Government as required in order to coordinate with room availability.

5. Type of Contract: Firm-Fixed-Price

B. Contract Award Meeting

1. The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick-off meeting, or has advised the contractor that a kick off meeting is waived.

C. General Requirements

1. All installation must comply with requirements of Patient Safety Alert AL14-07, including the Checklist for Installation and Relocation. The installation checklist must be filled out for each installed lift, in addition to any checklist required by the manufacturer, and supplied to the COR.

AL14-07PSAChecklist forInstallationandRelo

2. Installers must have current certification to install the lifts that are being installed.

3. Contractor will provide evaluation of existing structure where ceiling lifts are needed and submission of plans to VA North Texas Healthcare System Engineering service, to assure that workable systems are installed and comply with VA design requirements and relevant standards.

4. Contractor will coordinate with Engineering service to verify access to engineering mechanical, HVAC, and fire systems components within the mounting area of the lift units.

5. Contractor will coordinate with Contracting Officer Representative (COR) to schedule installations.

6. Contractor will provide information on minimum clearances for operation of each lift, to be presented with the bid.

7. Contractor will provide a list of all construction or modifications required to install these lifts, to be presented with the bid. The goal is to install around any obstructions without requiring VA to make construction modifications.

8. Contractor will provide all qualified personnel, materials, equipment and services necessary to install ceiling lifts for the rooms described below under Location, including lifts, tracks, spreader bars, attachment hardware, and all other equipment necessary for safe operation. Detailed requirements are described in the Standards and Special Requirements sections. Lift systems may be configured in any way that covers the rooms best and meets the requirements.

9. Contractor will provide installation and testing of all equipment for ceiling lifts, according to VA checklist requirements, resulting in a turnkey system.

10. Contractor will provide training on equipment use for all new types of equipment included in this contract, one session per shift for up to 2 shifts of personnel in each affected area and for onsite trainers (up to 10 sessions scheduled after installation and before use), in coordination with COR.

11. Contractor will provide evidence of testing of all lift systems provided and schedule testing when it can be overseen by COR.

12. Contractor will mitigate any new hazards created by installation, such as ramps over interstitial support beams that create trip hazards.

13. Contractor will meet construction safety requirements, including construction orientation, safety training requirements, dust control requirements from the Infection Control Risk Assessment, TB testing, electrical safety requirements, and Above Ceiling Permit requirements including re-sealing any penetrations of fire barriers.
