1. Scope of Work
1.1 Background and Purpose
Micro Air of Texas, Inc. was contracted by the VA North Texas Health Care System (VANTHCS) to prepare a Mold Remediation Protocol based on information provided to our office by our on-site Industrial Hygienist and a VA representative. According to the information received, Service Bay 1-4 located in Building 2J, had one or more steam leak(s), which have been repaired, according to the VAMC. Tape lift samples were collected in the southwest corner of this mechanical on August 3, 2021, and the results indicated that there is a relative abundance of Stachybotrys and Penicillium/Aspergillus-like molds located on the affected walls, HVAC insulation, and pipe insulation. On August 10, 2021, mold air samples were collected throughout the Service Bay 1-4 area, and the results indicated that there are airborne Stachybotrys and Penicillium/Aspergillus-like mold spores throughout the service bay area. For purposes of this document, the southwest corner of Service Bay 1-4 will be referenced as “Area 1", and the remaining portion of Service Bay 1-4 will be referenced as “Area 2" (see drawing 1 for locations of Area 1 and Area 2). Area 1 will entail a full mold remediation, while Area 2 will be limited, at this time, to cleaning and disinfecting of all surfaces.
1.2 Scope of Remediation
Area 1: This Mold Remediation Protocol includes the removal, cleaning, and disposal of all materials listed within this section (Scope of Work), that is directly impacted by visible fungal growth. The Remediation Contractor will be responsible for removing the following:
A. Remove all wallboard located inside Area 1 located in Service Bay 1-4. There is approximately 1100 SF of wallboard located in this area.
B. Remove all pipe insulation located inside Area 1 located in Service Bay 1-
4. There is approximately 300 LF of pipe insulation located in this area.
C. Remove all HVAC duct insulation located inside Area 1 located in Service Bay 1-4. There is approximately 300 SF of HVAC duct insulation located in this area.
The above listed quantities are estimates only, and were based on site conditions at the time of the on-site pre-remediation visual inspection (08/03/21). The Contractor shall be responsible for verifying all quantities and field conditions. The Contractor

1-4 must supply all supervision, labor, materials, services, insurance, Texas licenses, and equipment necessary to carry out the work described herein. Mold Remediation work for this project will occur during the week (7:00 am to 4:00 pm), unless the VAMC changes the project work schedule, due to unforseen circumstances.
Area 2: This Mold Remediation Protocol also includes the cleaning and disinfecting of all surfaces located in Area 2 (see drawing 1 for the location of Area 2). Mold air samples collected throughout Service Bay 1-4 indicated that there are airborne Stachybotrys and Penicillium/Aspergillus-like mold spores throughout the service bay area. The following Scope of Work explains the cleaning processes and the order in which the remediation work will be conducted. The Remediation Contractor will be responsible for the following:
D. Contractor will construct a negative-pressure containment around the southwest corner of the Service Bay area (Area 1). The containment will encase this corner of the Service Bay, and totally seal this area off from the remaining portion of room. A fresh air vent located on the west wall must be sealed off with polyethylene sheeting and duct tape (if a dampener exists, it should be closed, as well). Contractor is cautioned to be extremely careful not to disturb any of the mold growth located in Area 1 during preparation of the containment. The pressure inside the enclosure must be a minimum of -0.02 inches water column (-0.02 in. WC) at all times, as verified by a continuously operating manometer, until final clearance has been achieved. All HEPA negative air machines will be exhausted to the exterior of the building.
E. After the negative pressure containment has been constructed in item D, the Contractor will install a minimum of three 2000 CFM rated HEPA filtration machines in Area 2. These machines will operate 24 hours per day to act as scrubbers (no exterior exhaust). The Contractor will then HEPA-vacuum and clean all surfaces, pipes, HVAC duct insulation, air handling units, etc. located within Area 2. Soap and water can be used to clean non-porous surfaces, and an EPA approved mold disinfectant(s) should be applied and wiped free after. Once this is completed, the HEPA filtration (scrubber) machines will be allowed to operate for a minimum of 12 hours to further filter the air in Area 2. After the filtration period is complete, ambient mold air samples will be collected in Area 2, and the sample results must have airborne concentrations of fungi below concentrations measured on August 10, 2021 (report is attached to this protocol). Stachybotrys mold spores of any concentration will not be allowed prior to the Contractor being authorized to proceed. If these criteria are not met for the ambient air samples collected, the area must be re-cleaned and re-tested until the sample results are acceptable.
F.
G. Following a successful cleaning / disinfecting of Area 2, the Consultant will inform the Contractor that remediation of Area 1 can begin. Contractor will maintain two HEPA filtration machines operating in Area 2 until clearance of Area 1 containment is achieved.
NOTE: Based on information from the VAMC Engineering Department, there is a fresh air intake located on the southwest wall in Area 1. This intake brings in fresh air from outside the building via the attached duct work to the air handler. The fresh air intake duct work is sealed to the opening in the exterior wall. It must be sealed before any work begins on this project. Additionally, there is a vent for the mechanical room located on the same wall that must be sealed with poly sheeting and duct tape. All return air located in Service Bay 1-4 is contained inside metal ducts. There should not be any air from the Service Bay entering the air handling system. The VAMC has requested that all of the doors on the air handling units located in Service Bay 1-4 be sealed with duct tape prior to any work occurring.
1.2.1 The Mold Remediation Contractor must prepare a Mold Remediation Work Plan, which will provide specific methods, materials, and procedures for the mold remediation project. The Work Plan must be specific to this project, and it must address, in detail, how the containment will be constructed, and how the removal of the wallboard, duct insulation, and pipe insulation will be done. The Work Plan must be provided to, and approved by, the Owner and the Mold Assessment Consultant prior to mobilization of the Contractor to the job site.
1.2.2 Prior to the start of remediation activities, the Contractor shall provide the Owner and Consultant with all applicable mold remediation licenses and training certificates for the company, supervisor(s), and workers that will be utilized during this project. This includes initial mold remediation training (and refresher courses). Workers who have successfully completed remediation work training and received a training certificate may perform mold remediation work for a period of not more than 30 days from training if:
A. The individual has submitted an application for registration to the Texas Department of Licensing and Regulation and provides proof of this.
B.
C. A copy of the training certificate is present at the work site at all times while the individual conducts mold remediation.
D. The individual is in possession of a valid government-issued photo identification at all times while performing mold remediation work.
1.2.3 Prior to the start of remediation activities, the Contractor shall provide the Owner and Consultant with insurance certificates indicating that the Contractor has Commercial General Liability insurance in the amount required by the contract supplied by the Owner. The Owner shall be listed as certificate holder, with a minimum of 30 days notice of cancellation of the policy (endorsements must be provided with the insurance certificate).
1.2.4 Prior to the start of remediation activities, the Contractor shall provide the Owner and Consultant with all Safety Data Sheets (SDS) for any product that will be utilized during the remediation project. All products must be approved by the VAMC prior to beginning the project.
1.2.5 Prior to the start of remediation activities, the Contractor shall provide the Owner and Consultant with a copy of the submitted Mold Remediation Notification (and amendments, if applicable) to the Texas Department of Licensing and Regulation.
1.2.6 Not later than one week after the completion of the mold remediation project, the Contractor shall provide the Consultant and Owner with the following:

Safety glasses or goggles with open vent holes are not acceptable. If biocides are used, the clothing to be used has to comply with the manufacturer’s recommendations.
2.4 Decontamination Requirements: Before any worker leaves the work area, work clothing must be removed inside the containment by the exit. Prior to exiting the containment, the workers will HEPA vacuum themselves and any clothing they have on under the protective suit. Workers will then exit the containment and remove respiratory protection, followed by dressing in a designated clean area outside the containment.
3. Post-Remediation Assessment and Clearance
At the conclusion of the remediation project, the licensed Mold Remediation Contractor will notify the licensed Mold Assessment Consultant that the remediation project is ready for clearance. The post-remediation assessment and clearance shall be conducted while the walk-in containment is in place. The Mold Assessment Consultant requires a minimum of 2 hours advance notice prior to the start of clearance inspection and sampling.
Clearance Procedures and Criteria:
3.1 Visual inspection of the work area by the Mold Assessment Consultant will be performed to confirm that all visible mold growth and mold contaminated materials have been properly removed and disposed of, and the work has been satisfactorily completed in compliance with the Mold Remediation Protocol and Mold Remediation Work Plan. The post-remediation visual assessment shall be conducted while the containment is in place. Clearance air sampling shall not be performed until the containment(s) has passed visual inspection. After a successful visual inspection, Contractor will be required to apply a suitable encapsulant material to all interior surfaces of Area 1 in order to lock down any remaining mold spores. Once this is completed, the HEPA filtration machines will filter the air without exhausting to the exterior of the building for a minimum of 12 hours prior to collection of clearance air samples in Area 1.
3.2 Following the minimum 12 hour filtration period, clearance air sampling will be conducted using high volume air sampling pumps and Allergenco-D® mold spore cassettes, manufactured by Environmental Monitoring Systems. The number and locations of air samples collected inside the containment(s), as well as outside the building, will be determined by the Consultant for each containment. Surface 3.3 sampling may also be performed by the Consultant, using tape-lift and/or cotton swab sampling media, in order to confirm the absence of high concentrations of mold spores on interior surfaces of the contained areas. Clearance sampling shall be performed while the walk-in containment is in place. Clearance samples will be submitted to a Texas licensed Mold Analysis Laboratory and will be analyzed in accordance with nationally-recognized analytical procedures. Results will be received within 24 hours of collection.
3.4 Clearance criteria for each containment:
1. Containment must pass visual inspection, as outlined in Sect. 3.1.
2. Clearance air sampling results must indicate that airborne mold spore levels are lower inside the containment area than ambient levels obtained outside the building and are qualitatively similar (types and relative abundances of mold spore types) to those measured outside the building at the time the sampling was performed. No levels of identifiable Stachybotrys spores are acceptable to meet the clearance criteria.
3. Surface sampling (if conducted) results must indicate “none detected”, “rare”, or “low” relative abundance of mold spores. No levels of identifiable Stachybotrys spores are acceptable to meet the clearance criteria.
3.5 Following successful clearance of the containment(s), the Consultant will inform the Contractor that clearance has been achieved. At that time, the Contractor will be authorized to dismantle the containment barriers, and they will be bagged as contaminated waste. The area will be thoroughly cleaned, and the work area will be returned to occupancy status before the Contractor vacates the project site.
3.6 Not later than one week after the completion of the mold remediation project, the Contractor shall provide the Consultant and property owner with copies of all information listed in Section 1.2.6 above, as outlined in TAC Title 16, Part 4, Chapter 78, Rule 78.150, of the Texas Mold Remediation Regulations.

Director These results apply to the sample(s) as received. The laboratory is not responsible for concentrations which dependend on volume collected by non-laboratory personnel. Samples are analyzed according to SOP# 7-03-2, which includes a 100% scan of the trace at 200X magnification and a minimum of 20% of the trace counted at 400X magnification. Debris rank indicates loading of particulates, both biological and non-biological, which may interfere with analysis. High debris rankings (4+) may obscure small spores and/or prevent the adherence of airborne particulates. Fungal counts on samples with high debris or
'overloaded' rankings should be regarded as minimal with actual counts being higher than reported.

These results relate only to the sample(s) submitted. This report is for the exclusive use of the addressed client and shall not be reproduced except in full without written approval by Eurofins J3 Resources, Inc. Samples were received in acceptable condition unless stated otherwise on this report. Samples are analyzed according to SOP# 7-03-3, which is solely qualitative; indications of relative abundance are based solely on a visual estimation of the sample submitted. A “Rare” designation implies a very low occurrence consistent with a random occurrence/background concentration. Penicllium/Aspergillus-like fungal spores references spherical, morphologically indistinguishable 2-6µm spores. Only the top three fungal spores in relative abundance, unless noted otherwise, are reported. There is no current data supporting a critical or threshold exposure limit to fungal aeroallergens. Blank corrections are not applied to data unless requested by the customer. To date, occupational health related organizations, such as OSHA and NIOSH have not established Permissible Exposure Limits or Recommended Action Levels, or other limit values for aeroallergens.

Lab
Director These results apply to the sample(s) as received. This report is for the exclusive use of the addressed client and shall not be reproduced except in full, without written approval by
Eurofins
J3 Resources, Inc. (EJ3).
Samples are analyzed according to the methods listed above and are subject to the inherent limitations of PLM and interference of matrix components. Reporting limit for the above method is a function of the quantity of sample analyzed, matrix interference, sample preparation, fiber size, and distribution.
Asbestos may be detected in concentrations of <1% by area if sufficient material is analyzed. EJ3 recommends TEM confirmation of soils, vermiculite and non-friable organically bound materials (NOB) reported as None Detected or < 1% Asbestos by PLM. All samples received in good condition unless otherwise noted. This report shall not be used to claim product approval, certification, or endorsement by NVLAP, NIST, or any agency of the federal government.
