Questions and Answers for RFQ 36C25721Q0465 Question: Looking at the 1st item, STAT acetaminophen testing in 1 hour, this time would be extraordinary challenging for an in-house facility. A 2-hour TAT for in-house on-site testing would be more aligned with best practice standards. Also, including additional time for the couriers to pick up the specimens to deliver to a laboratory should also be included. Would you consider for STAT testing including additional time? Possibly providing an estimated time of no greater than 3 hours for transportation and testing reporting?
Answer: The reason for the STAT TAT for acetaminophen is that this particular drug (along with Salicylate) can be very toxic to the liver during an overdose and these patients are often seen in the Emergency Department. The Amarillo VA has 2 analyzers that can perform the testing but by some off chance the Amarillo VA is out of reagents, the controls do not work, or the analyzer is down, the risk cannot be taken of delaying results longer than 1 hour at this time. This is a firm 1 hour or less test.
Regarding some of the other TAT requested in the SOW as 1 hour or less, they too are firm. These times are crucial during patient interventions for the providers in ED, Surgery, PACT, ICU, and during rapid responses, code blues, etc. Many of the therapeutic drugs are toxic to the kidney and could result in permanent dialysis if results are too high, Cardiac markers (BNP, CK, CK-MB, and Troponin I) can be the reason of life or death, Coagulation testing (PT,PTT, fibrinogen, and D-dimer) can result in the patients bleeding to death due to too much warfarin, coumadin, heparin or assist in diagnosing a blood clot.
The VA providers are used to us giving results in 1 hour or less 24/7/365 for the tests listed below. The SOW is for the “STAT/URGENT 24/7/365” and is mainly in lieu of the Amarillo VA laboratory not being able to perform the required testing due to analyzer downtime for whatever reason. This SOW is not for ordinary reference lab testing.
