The services being requested are for the reference laboratory testing of patient specimen. Services are to be performed at the South Texas Veterans Healthcare System, Laboratory, 7400 Merton Minter Blvd., San Antonio, TX 78240.

The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition.

The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The evaluation factors are technical acceptability, past performance and price. Under technical includes the ability to provide all of the requested items in accordance with the Performance Work Statement, and have, at minimum, a “Satisfactory” past performance rating.

Offerors are to provide a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.

The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. There are no addenda to this clause.
