Attachment D.3: FAR 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERICAL ITEMS

ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The version of FAR 52.212-1 in the addendum is tailored for Simplified Acquisition Procedures and supersedes the current version of FAR 52.212-1 contained in the FAR.

The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:

FAR 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS

System for Award Management: IAW FAR 52.212-4 (t) Offerors are encouraged to go to the SAM database at https://www.sam.gov/portal/public/SAM and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. After initial registration, the contractor is required to update registration data as changes occur and must re-register annually to ensure all data remains current. Noncompliance with this requirement will preclude the exercising of any option periods that may be included herein and will be cause for termination of the contract at such time noncompliance is discovered. Refer to cited Clause for more details.

Amendments to this Solicitation: Offerors are encouraged to monitor the Contract Opportunities website at www.beta.sam.gov with respect to this solicitation because any amendments to this Solicitation will be posted on that website.

Format of Proposal: Offerors should review this entire solicitation before submitting a proposal to ensure that their proposals meet all requirements. Specific guidance on proposal preparation is provided in Section 52.212-1 of this solicitation.

Questions: All questions regarding this solicitation shall be submitted to the contract specialist via e-mail to duryea.williams@va.gov Questions will not be accepted via fax or by US mail. Due to the urgent need for these services, there is no assurance that questions received after [ten calendar days before the closing date of solicitation] will be answered. All responses to questions will be incorporated into a written amendment posted to the Contracting Opportunities website (www.beta.sam.gov).

INSTRUCTIONS FOR PROPOSAL SUBMISSION

CONTRACTORS SHALL INCLUDE THE FOLLOWING INFORMATION IN THEIR PROPOSALS TO RECEIVE FULL CONSIDERATION FOR AWARD:

1. Submission of Proposal – Submit proposal to the office specified in this solicitation at or before the exact time specified in this solicitation. Proposals may be submitted on letterhead stationery, or as otherwise specified in the solicitation. As a minimum, proposal must show:

· The solicitation number;

· The time specified in the solicitation for receipt of proposal;

· The name, address, and telephone number of the offeror;

· A Technical Approach of Contractor proposal in sufficient detail to evaluate compliance with the requirements in the solicitation. (See Factor 1)

· Past Performance Information, which will include recent and relevant contracts for the same or similar services, (including contract numbers, points of contact with telephone numbers and other relevant information.)

2. Pricing – Proposed pricing shall be submitted on the Price/Cost Schedule page. Additional sheets may be attached if required. The unit price provided shall be the proposed fixed hourly rate for After Hour Pharmacy Services. The estimated number of hours shown in the schedule to be work during the period of performance, calculated by multiplying the unit price per hour rate by the estimated hour provided, this will give you an estimated total. If the amount provided for any contract year is different than the amount that results from the formula provided in the previous sentence, the amount stated shall presumed to be in error and the amounts stated in columns shall be assumed to be correct.

3. Representations and Certifications - Completed copy of representations and certifications included in FAR 52.212-3 and the representations in 52.209-7.
