Summary: Special Notice - UNOS Toolbox FY27

This is a Special Notice and pre-Intent to Sole Source/Request for Information (RFI) issued by the Department of Veterans Affairs, Network Contracting Office 16 in Houston, Texas. The VA intends to negotiate a sole source Firm-Fixed price contract with United Network for Organ Sharing (UNOS) for access to the UNOS Toolbox, a suite of proprietary tools and data services required for the Michael E. DeBakey VA Medical Center's organ transplant program operations. The requirement is classified under NAICS 621991 (Blood and Organ Banks) with a small business size standard of $40 million. The anticipated contract award date is November 1, 2026. The procurement justification cites 41 U.S.C. §3304(a)(1) and FAR 6.302-1, emphasizing that UNOS is the only federally contracted not-for-profit organization authorized to manage the Organ Procurement and Transplantation Network (OPTN) under the National Organ Transplant Act of 1984, making it the sole source capable of providing these services without unacceptable delay or cost duplication.

The UNOS Toolbox encompasses critical components including DonorNet (the national organ offer and acceptance platform), Offer Filters and Match Run Viewer, Waitlist Management Tools, Predictive Analytics (Kidney Donor Profile Index and Estimated Post-Transplant Survival models), and Data Dashboards and Reporting Tools. These proprietary tools are essential for maintaining OPTN policy compliance, optimizing organ utilization, reducing discard rates, and improving patient outcomes. Interested parties must submit RFI responses by October 9, 2026 at 3:00 p.m. Central Standard Time to Tiffany Burks at tiffany.burks@va.gov, with submissions limited to 8.5 x 11 inches, 12-point font, and 1-inch margins in Microsoft Word or Adobe Acrobat format. SCOPE OF WORK AND GENERAL SPECIFICATIONS

FOR UNITED NETWORK ORGAN SHARING (UNOS)

Background: The Michael E. DeBakey VA Medical Center (MEDVAMC) is seeking to procure access to the United Network for Organ Sharing (UNOS) Toolbox, a suite of proprietary tools and data services essential for the operation, compliance, and performance optimization of its transplant program. These tools are developed and maintained by UNOS, the only private, non-profit organization that manages the Organ Procurement and Transplantation Network (OPTN) under federal contract, as mandated by the National Organ Transplant Act (NOTA) of 1984.

UNOS has operated the OPTN since its inception in 1986 and remains the sole entity authorized to provide the national transplant system’s infrastructure, including the tools that support organ allocation, waitlist management, and transplant analytics. The UNOS Toolbox is a critical component of this infrastructure, offering transplant centers access to real-time data, predictive analytics, and decision-support tools that directly impact patient outcomes and regulatory compliance.

Key components of the UNOS Toolbox include:

• DonorNet®: The national platform for organ offer and acceptance.

• Offer Filters and Match Run Viewer: These tools streamline the organ offer review process and provide transparency into how allocation decisions are made. The organ offer acceptance ratio is a critical performance metric for all transplant programs, with a required minimum threshold of 0.4. Falling below this benchmark can result in program suspension. The Offer Filters tool is essential for managing this ratio effectively by allowing programs to tailor which offers they receive, reducing unnecessary declines and improving overall acceptance rates.

• Waitlist Management Tools: Support accurate, policy-compliant patient listings by allowing transplant staff to monitor which patients are active or inactive, and identify those at the top of MEDVAMC’s waitlist. These tools are essential for maintaining compliance with OPTN policies. Effective waitlist management is a required performance metric for transplant programs, and waitlist mortality is directly influenced by how well the waitlist is maintained and monitored.

• Predictive Analytics: Including the Kidney Donor Profile Index (KDPI), Estimated Post- Transplant Survival (EPTS), and other models that support clinical decision-making.

• Data Dashboards and Reporting Tools: Enable performance tracking and quality improvement initiatives.

These tools are not available through any other vendor or platform, as they are proprietary to UNOS and integrated with the national transplant system. Access to the UNOS Toolbox is essential for MEDVAMC to:

• Maintain compliance with OPTN policies and federal transplant regulations.

• Optimize organ utilization and reduce discard rates.

• Improve patient outcomes through data-driven decision-making.

• Sustain its designation and operational readiness as a transplant center.

Since UNOS is the only federally contracted not-for-profit organization that manages the OPTN and its associated tools, no other organization can provide the services required. Therefore, this requirement cannot be set aside for small business. Continued access to the UNOS Toolbox is necessary to maintain MEDVAMC’s status as a compliant and functioning transplant center under federal law and OPTN policy.
