Statement of Work (SOW) Prevention Personal Protective Equipment (PPE) Isolation Rolling Carts
12 July 2024
1. PURPOSE: The Central Arkansas Veterans Health Care System (CAVHS) has a requirement for Infection Prevention PPE Isolation Carts.
2. OBJECTIVE: The objective is to have PPE gear readily available and accessible to all staff caring for CAVHS patients.
3. SCOPE: The Contractor shall furnish all equipment and services required for a quantity of 12 each PPE Isolation Carts.
3.1. All equipment and configuration shall meet the manufacturers and VA specifications.
3.2. The Contractor shall be responsible for any missing parts and components not included.
3.3. SALIENT CHARACTERISTICS.
• Locking Casters
• Folding Work Surface
• All doors shall have latches
• Drawer dividers included.
• All drawers shall have storage space between the dispensing fronts.
• Drawers shall hold 15 pounds
• Only one (1) drawer at a time can open
• Working surface must be able to handle up to 35 pounds.
• Storage bin and clipboard holder must be able to hold 5 pounds
• Drawer windows can be see-through or closed
• Must come fully assembled
• The carts shall be a minimum of four (4) drawers
• Non-Stainless Steel
• Empty Cart must not weigh more than 80 pounds
• Cart Dimension shall be a minimum of 23.75x15.375x54.375
• 5 inch caster wheels with locks
• Made of Aluminum Composite Material (for its dimensional stability and low weight)
3.4. The PPE Isolation Carts shall be configured to the manufacturer’s specifications maintaining Federal, state, and VA safety standards.
3.5. The Contractor shall provide all applicable software and manufacturer’s manual.
4. SHIPPING/DELIVERY. The Contractor shall ship/delivery all equipment to the Central Arkansas Veterans Healthcare Systems (CAVHS), 2200 Fort Roots Dr. Bldg. 182, North Little Rock, AR, 72205.
4.1. The carts shall be delivered in the manufacturer's original sealed containers.
4.2. The equipment shall be packaged to prevent damage or deterioration during shipment, handling, storage, and configuration.
5. WARRANTY: The Contractor shall provide a standard manufacturer’s warranty.
5.1. The warranty shall include all shipping costs associated with any warranty repair.
6. HOURS OF OPERATION: The CAVHS normal business hours are 8:00 AM – 4:30 PM Monday through Friday, Central Standard Time (CST), except Federal holidays.
7. PERIOD OF PERFORMANCE: The Contractor shall notify the Contracting Officer’s Representative (COR) when carts are being shipped.
8. Type of Contract: The Government anticipates award of a Firm-Fixed Price contract.
9. FEDERAL HOLIDAYS: The Contractor is not required to perform services on the following Federal holidays:
New Year’s Day January 1st Birthday of Martin Luther King Jr. 3rd Monday in January Washington’s Birthday 3rd Monday in February Memorial Day Last Monday in May Juneteenth National Independence Day June 19th Independence Day July 4th Labor Day 1st Monday in September Columbus Day 2nd Monday in October Veterans Day November 11th Thanksgiving Day 4th Thursday in November Christmas Day December 25th
In addition to the days designated as holidays, the Government observes the following days: Any other day designated by Federal Statute. Any other day designated by Executive Order. Any other day designated by the President’s Proclamation.
10. NARA Records Management Language Clause:
1. Contractor shall comply with all applicable records management laws and regulations, as well as National Archives and Records Administration (NARA) records policies, including but not limited to the Federal Records Act (44 U.S.C. chs. 21, 29, 31, 33), NARA regulations at 36 CFR Chapter XII Subchapter B, and those policies associated with the safeguarding of records covered by the Privacy Act of 1974 (5 U.S.C. 552a). These policies include the preservation of all records, regardless of form or characteristics, mode of transmission, or state of completion.
2. In accordance with 36 CFR 1222.32, all data created for Government use and delivered to, or falling under the legal control of, the Government are Federal records subject to the provisions of 44 U.S.C. chapters 21, 29, 31, and 33, the Freedom of Information Act (FOIA) (5 U.S.C. 552), as amended, and the Privacy Act of 1974 (5 U.S.C. 552a), as amended and must be managed and scheduled for disposition only as permitted by statute or regulation.
3. In accordance with 36 CFR 1222.32, Contractor shall maintain all records created for Government use or created in the course of performing the contract and/or delivered to, or under the legal control of the Government and must be managed in accordance with Federal law.
Electronic records and associated metadata must be accompanied by sufficient technical documentation to permit understanding and use of the records data.
4. Central Arkansas Veterans Healthcare and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Records may not be removed from the legal custody of CAVHS or destroyed except for in accordance with the provisions of the agency records schedules and with the written concurrence of the Head of the Contracting Activity.
