SCOPE OF WORK:
Brand Name Philips Skyplate Detector Replacement with Installation Contractor shall furnish all labor, materials, parts, equipment, tools to exchange and install an IR Large Skyplate Detector belonging to the Philips Digital Diagnost (S/N: 15000136) at the Michael E. Debakey VA Medical center located at 2002 Holcombe Boulevard, Houston, TX 77030. The skyplate detector is defective and must be exchanged. The detector installation must be performed by trained technicians. The contract price shall be firm-fixed, all-inclusive, and not limited to specified products/equipment, installation, and setup/configuration.
All companies must provide documentation proving that their technicians are certified on the equipment specified above. Additionally, the contractor shall provide documentation of at a minimum 2 occurrences of similar work. The contractor shall provide only exchange / replacement parts that are authorized by the Original Equipment Manufacturer (OEM) to be sold in North America. All parts used in the exchange must be certified new from the OEM and must not be gray market parts.
1) Contractor shall exchange, install all parts to complete repair, perform inspection and shall furnish all labor, materials, parts, equipment, tools to exchange and repair an IR Large Skyplate Detector belonging to the Philips Digital Diagnost (S/N: 15000136) at the Michael E. DeBakey VA Medical Center located at 2002 Holcombe Blvd, Houston, TX 77030 in a maximum of 2 days from the start of the repair.
2) Contractor shall be an OEM-authorized distributor and installer of the IR Large Skyplate.
3) Contractor shall provide only exchange/replacement parts that are authorized by the OEM to be sold in North America.
4) Contractor will have up to 5 days to begin the repair of the equipment after receiving confirmation from the COR or VA POC.
5) The Contractor will not act outside of the scope of this project without guidance from the COR or VA POC.
6) The IR Large Skyplate Detector shall be installed to OEM operating and fully-functional standards.
PRIVACY/SECURITY
The contractor will not have electronic connectivity to the devices and will not have access to patient databases or receive screenshots with PHI present.
Any electronic or magnetic data storage devices such as hard drives, etc. shall be turned into the ISO. Electronic or magnetic data storage devices are not to be removed from VA property.
