E.8 52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)

a. The Government will award a Fixed Price single award Indefinite Delivery Indefinite Quantity (IDIQ) contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, is determined to be the best overall value to the Government, price and other factors considered. The evaluation of the offers submitted in response to the Request for Proposals (RFP) will be conducted IAW FAR 15-101-1 Best Value Tradeoff process. The following factors and sub-factors shall be used to evaluate offers. Factors are listed in descending order of importance. Sub-factors listed within the technical capability factor are equal in importance.

Factors are listed in descendance order of importance. All non-price factors (Factors 1-5), when combined, are significantly more important to the Government when compared to price (Factor 6), and each sub-factor of Factor 1 is equal in importance, for the purposes of determining the best value to the Government; therefore, the Government may award to other than the lowest priced offeror if it is in the Government’s best interest to do so.

1. Evaluation factors and criteria.

a. Offerors should address in detail their Technical Capabilities by describing their proposed approach to each factor. All factors, except Past Performance, will be considered based solely on the proposal provided, to the extent in which the proposal demonstrates a clear understanding of the requirements, and the contractor’s ability to meet those requirements.

1) Factor 1 –Technical Capability. This factor will be used to determine the Offeror’s ability to successfully manage the full range of contractual requirements, and background and relevant information regarding stability and strengths; each sub-factor is equally important.

Sub-factor (a) – Offeror’s Capabilities/Experience.

The below bullets are part of Sub-factor (a) and not separate Sub-factors.

· Offerors shall provide evidence that they can provide a full range of services to include evidence of adequate staff, financial resources to acquire or obtain necessary equipment, meet start-up requirements including payrolls and staffing capabilities, including any teaming or subcontract arrangements that may be used.

· Offerors shall describe medical and administrative equipment, other than VA-provided OIT or Tele-health equipment, and availability to be utilized under the contract. Describe contingency plan for equipment downtime. Describe familiarity with VA’s electronic medical record, specifically VHA/EHR. Greater evaluation rating will be given to Offerors with experience in VA Health Information Systems.

· Offerors shall confirm their capability to meet all staff/patient ratio requirements in the PWS.

· Offerors shall submit documentation of current workload ratio for patients per physician per day.

· Offerors shall provide current organizational chart and description of organization’s history.

· Offerors shall describe the mechanism to schedule qualified credentialed physicians and staff to cover the required clinic schedule. Describe contingency plan for covering enrolled patient’s appointments due to scheduled or unscheduled leave.

· Offerors shall describe average personnel turnover rate and how it may or may not affect contract performance. Describe the capability to recruit adequate staffing to meet contractual requirements.

· Offerors shall provide a synopsis of their overall approach to quality control and improvement. Describe performance measurement process that will be implemented to monitor quality, appropriateness of care, access to care, and patient satisfaction.

· Offerors shall indicate if the Offeror has experience providing VA CBOC services. Greater evaluation rating will be given to Offerors with VA CBOC experience.

Sub-factor (b) – Quality Staffing.

The below bullets are part of Sub-factor (b) and not separate Sub-factors.

· Offerors shall provide a list of names of the qualified physicians, nurse practitioners and other primary care provider staff intended to be utilized in the performance of the contract and/or describe the mechanism used to provide qualified physicians, nurse practitioners and primary care provider staff to perform the contractual requirements.

· Offerors shall emphasize if proposed qualified staff members possess experience providing services containing the same level of size, magnitude, and complexity of the requirements of the PWS. Greater evaluation rating will be given to Offerors proposing staff with experience providing services containing the same level of size, magnitude, and complexity of the requirements of the PWS.

· Offerors shall provide copies of the following for any physician(s) to be assigned to perform services under the contract, including any physicians to provide coverage relief:

a) Board Certification in Internal Medicine and/or Family Practice

b) Active, current, unrestricted license

c) Curriculum Vitae

· Offerors shall provide copies of the following on any nurse practitioner(s) to be assigned to perform services under the contract, including any nurse practitioner(s) to provide coverage relief:

a) Transcript(s) from an Accredited Nurse Practitioner Program

b) Graduation Certificate from an Accredited Nurse Practitioner Program

c) Active, current, unrestricted license

· Offerors shall provide a list of administrative support staff and describe the level of training and experience of each that will be utilized to meet the administrative support functions in the performance of the contract, including grievance system and quality assurance/performance improvement.

Sub-factor (c) – Implementation Plan.

The below bullets are part of Sub-factor (d) and not separate Sub-factors.

· Offerors shall describe their ability to meet performance measures related to Patient Aligned Care Team (PACT) implementation (staffing ratios, exam room access, access measurements, continuity, discharge follow-up calls as indicated in the Performance Work Statement.

· Offerors shall provide a detailed operational plan to include staffing that shows the CBOC will be ready to provide services within 90 days of contract award in accordance with the Performance Work Statement.

· Offerors shall indicate if the Offeror has experience with VA health record information database systems. Greater evaluation rating will be given to Offerors with experience in VA Health Information Systems.

2) Factor 2 – Geographic Location/Accessibility.

The below bullets are part of Factor 2 and not separate Sub-factors.

· Offerors shall specify the location of the proposed facility(which must be located within Lowdnes county) to be utilized in the performance of the contract, including full street address (the government will evaluate proposals to determine whether offered facilities/locations are within Lowndes County and/or near the city limits of Columbus, Mississippi). Greater evaluation rating will be given to Offerors proposing CBOCs within the Columbus city limits.

· Offerors shall describe access to public transportation to the facility.

· Offerors shall describe the location of the facility in relation to major highways.

· Offerors shall describe the availability of adequate parking at the facility.

· Offerors shall describe handicap accessibility to the facility.

· Greater evaluation credit will be given to Offerors that show greater/easier access to public transportation, major highways, and parking.

· Provide copies of floor plans for proposed space and schematics or maps showing locations relative to other services, etc. Describe compliance with ADA requirements and how the proposed space meets the design standards in the PACT Space Module Design Guide.

· The Government may require a site visit.

3) Factor 3 – Past Performance.

(a) The Government shall review available past performance data in the Federal Awardee Performance and Integrity Information System found in the Past Performance Information Retrieval System found at https://fapiis.ppirs.gov/

(b) Offerors shall identify a minimum of three (3) recent/relevant contracts that are similar in type, scope, size, complexity, and dollar value to the contract being offered herein, to include contract name, dates of performance, point of contact, telephone number and e-mail address. Contracts listed may include those entered into with the Federal Government, State or local agencies, or commercial customers. More recent and relevant past performance will be given more weight than less recent and less relevant past performance. Refer to ATTACHMENT D6 in Section D of the RFP. Offerors are encouraged to include a narrative in the Past Performance References attachment that demonstrates satisfactory performance under the referenced contract.

(c) Each offeror shall provide their references with a copy of the Past Performance Questionnaire/Survey (ATTACHMENT D5) in Section D of the RFP, and have it returned to the CO by the response date listed in Block 8 of SF1449. Questionnaires can be scanned and emailed to corey.labbe@va.gov.

(d) The Government reserves the right to obtain past performance information from any available source and may contact customers other than those identified by the Offeror when evaluating past performance.

(e) In accordance with FAR 15.305(a)(2)(iv), if an offeror has no record of relevant past performance or for whom information is not available, they will not be rated favorably or unfavorably.

4) Factor 4 – Veteran Participation In accordance with VAAR 852.215-70 included herein, this factor will take into consideration an offeror’s status as an SDVOSB, VOSB and/or its proposed use of eligible SDVOSB and/or VOSB as subcontractors. This factor will give credit in the following order of descending importance:

· Service-Disabled Veteran-Owned Small Business (SDVOSB).

Provide SDVOSB status if applicable to the offer. The greater the equality of offers within the non-price factors and price, the more important this status becomes in selecting the best value to the Government. Preference will be given to SDVOSB whose offer is equal to or exceeds all other offers in non-price and price factors and provides the best value to the Government. Furthermore, preference will be given to the SDVOSB whose offer is equal to or exceeds the proposals of any Veteran Owned Small Business and provides the best value to the Government.

· Veteran-Owned Small Business (VOSB).

The greater the equality of offers within the non- price factors and price, the more important this status becomes in selecting the best value to the Government. Preference will be given to the VOSB whose offer is equal to or exceeds all other offers in non-price and price factors and provides the best value to the Government.

In order for SDVOSB or VOSB to receive credit under this factor, the offeror must be registered and verified in the Vendor Information Pages database at https://www.VetBiz.gov per VAAR 804.1102.

5) Factor 5 – Sub-Contracting Non-SDVOSB or Non-VOSB committing to subcontracting with either SDVOSB or VOSB in the performance of the contract. In an effort to achieve socioeconomic small business goals, the VA will evaluate offerors based on their veteran-owned or service-disabled veteran-owned status, their proposed use of veteran-owned or service-disabled veteran-owned small businesses as subcontractors, and their past performance on adherence to subcontracting plan requirements. See below Agency Sub-Contracting Goals.

To receive preferential consideration for the offeror’s proposed use of verified VOSB or SDVOSB concerns in subcontracting effort, offerors must state in their proposals the names of the veteran-owned and service-disabled veteran-owned small businesses with whom they intend to subcontract and the approximate dollar values of the proposed subcontracts. To qualify, the subcontractor(s) must be registered and verified by The Center for Veterans Enterprise at https://www.vetbiz.gov.

6) Factor 6 - Price

Prices shall be evaluated for fairness and reasonableness and will be compared against competing proposals and the independent Government cost estimate.

b. Ordering Periods. The Government will evaluate offers for award purposes by adding the total price for all ordering periods to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the ordering period prices are significantly unbalanced. Evaluation of the ordering period shall not obligate the Government to issue task orders for an ordering period(s)

c. All proposals will be evaluated in accordance with FAR 15.305. The Government intends to award without discussions with offerors. However, the Government reserves the right to conduct discussions if it is in its best interest to do so. If discussions are conducted, the Government may establish a competitive range of the most highly rated proposals, unless the range is further reduced for purposes of efficiency pursuant to FAR 15.306(c)(2).

d. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a purchase order without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are communications after its receipt, unless a written notice of withdrawal is received before award.
