Questions and Answers #1
The Network Contracting Office (NCO) 16 received questions in response to Request for Proposal (RFP) 36C25621R0001.
Please see below for the Contractor’s questions and NCO 16’s respective answers accordingly.
Question
Answer
Q1
In Section B.2 Schedule of Services on page 6 of the Solicitation states in Line Item Number 0001, the base year estimated enrollment is 2250. As the incumbent contractor, the latest billed enrollment quantity for the Greenville MS CBOC is 2091 which is 159 enrollees lower than the estimated enrollment stated. Secondly, on page 7, line item 3001 shows an 11% increase through Option Year 3 to 2,500 enrollees which historically, the enrollment has been steadily decreasing over the last 6 years. In order for contractors to provide more accurate cost estimates, would the VA consider reducing the estimated enrollment in the base and outlying years that more closely aligns with current and historical enrollment trends?
The VA will keep the numbers as to account for additional enrollment as this is just an estimated quantity.
Q2
On page 14 paragraph 2.1 of the Solicitation states, “The Clinical Pharmacy Specialist (CPS) shall be provided the same support staffing given to other providers on the team when they are working in the capacity of a mid-level provider.” Is it acceptable to utilize a PACT team member for CPS support?
Yes, the support for the CPS can be shared support for the clinic unless the volume of patients would necessitate a full-time staff member support for the CPS.
Q3
On page 18 paragraph 2.4.3 of the Solicitation states, Contractor shall provide LCSW. And paragraph 2.4.5 states PC-MHI: Contractor to provide a 0.67 FTE per PACT Mental Health LCSW. On page 20 paragraph 2.7 states the Contractor MH LCSW shall provide PCMHI and General Mental Health Services. Is the Contractor to provide 1 MH LCSW and 1 PC LCSW plus 0.67 per PACT Mental Health LCSW? Or does the Contractor provide 1 MH LCSW per PACT Team plus 0.67 PC-MHI per PACT Team? Does one LCSW provide PCMHI and General Mental Health Services? If so, per how many PACT Teams or enrolled and non-enrolled Veterans? Or is there another scenario?
LCSW will provide PCMHI and General Mental Health to all enrolled Veterans at the designated CBOC.
Contractor’s question: Is the Contractor to provide 1 MH LCSW and 1 PC LCSW plus 0.67 per PACT Mental Health LCSW?
VA’s response: Yes Contractor’s question: Or does the Contractor provide 1 MH LCSW per PACT Team plus 0.67 PC-MHI per PACT Team?
VA’ s response: 1.67 LCSW FTE per PACT Team Contractor’s question: Does one LCSW provide PCMHI and General Mental Health Services?
VA’s response: Yes Contractor’ s question: If so, per how many PACT Teams or enrolled and non-enrolled Veterans? Or is there another scenario?
VA response: 1.67 LCSW FTE per PACT Team.
Q4
On page 18 paragraph 2.4.4 of the Solicitation states, Registered Dietitian/Nutritionist: Services will be provided by the parent VAMC via telehealth (by referral), individual instructions and group classes, and/or telephone clinic.” And on page 68 paragraph 4.7.6.5 of the Solicitation states,
“Contractor shall ensure that Dietitians electronically complete an Event Capture System form for all direct patient care, whether individual visits, group visits or team meetings that discuss individual patients.” If the parent VAMC is providing the Registered Dietitian/Nutritionist, wouldn’t the VAMC ensure that the Dietitians electronically complete the Event Capture System form and not the Contractor?
Yes, VA personnel will need to track reports to ensure no gaps in request are pending at the CBOC.
Q5
On page 20 paragraph 2.8 of the Solicitation states, “Telehealth Clinical Technician (TCT) FTE Ratio performance standard 1.0 FTE Telehealth Clinical Technician (TCT) per estimated 5,000 patients at clinic, with a trained back-up (e.g., another TCT or a member of the PACT Teamlet (LIP, RNCM, or LPN), depending on size of clinic/clinic workload/clinic telehealth services).” Is it acceptable to utilize an LPN or an MA for the TCT position? With an enrollment of approximately 2250 in the first year, is it acceptable to hire a 0.5 FTE TCT or is the Contractor required to have a full time TCT available Monday through Friday, 8:00 AM to 4:30 PM? As the incumbent contractor, currently the Greenville CBOC has a full-time TCT in place.
Contractor’s question: Is it acceptable to utilize an LPN or an MA for the TCT position?
VA’s response: Yes Contractor’s question: With an enrollment of approximately 2250 in the first year, is it acceptable to hire a 0.5 FTE TCT or is the Contractor required to have a full time TCT available Monday through Friday, 8:00 AM to 4:30 PM?
VA’s response: No, 1.0 Full time FTE is required and shall be available Monday through Friday.
Q6
On page 33 paragraph 4.6.2.1.2 of the Solicitation states, “Mammography services will be provided by via an approved community care consult placed by the contractor provider in the VA EHR.” Is mammography provided by the VA at a cost to the VA?
VA Care in the Community will cover Mammography Cost.
Q7
On page 52 paragraph 4.6.5.4 of the Solicitation states, “The Contractor shall provide the following clerical support services for mental health providers (including VA providers if VA provides MH services onsite) at the CBOC.” Is it acceptable to utilize a PACT team member for clerical support services of the mental health providers or will a dedicated MSA be required?
Yes, duties can be share depending on our size of patient volume and case mix.
Q8
On page 58 paragraph 4.6.8.4 states, “The Contractor shall provide space for clinical pharmacy telehealth services at the Contractor’s location as appropriate.” On page 58 paragraph 4.6.8.4.1 states, “Space should provide privacy for patients to meet confidentially in an individual or group setting with providers at the VA via electronic transmissions.” On page 73 it indicates the Contractor is to provide a private group room at least 300 sq. ft. in size for mental health. Also on page 73 it indicates the contractor is to provide individual or group setting with providers at the VA via electronic transmission for telepsychiatry. Is the Contractor required to provide a clinical pharmacy telehealth services group room, a telehealth group room and a telepsychiatry services group room for a total of 3 group rooms? If not, how many and what type of group rooms would the VA like the Contractor to provide?
Three rooms total are required. One of the three required room needs to be a group room.
Q9
On pages 56-57 paragraph 4.6.7 of the Solicitation states, “Contractor shall implement VHA Telehealth Services…” and states, “It is the responsibility of the contractor to ensure that all telehealth services provided by contractor’s staff meet all such requirements. Staff, Space and Equipment requirements shall be as required by this document.” On page 57 paragraph 4.6.8 states, “The veterans may visit with primary care VA providers, specialty care providers and rehabilitative service providers over a proprietary VA Telehealth Network that initially includes Tele-Retinal, Tele-Dermatology, Tele-Pharmacy and Mental Health Services Primary care providers may refer for other telehealth consultative services from specialty providers and rehabilitative service providers.” On page 57 paragraph 4.6.8.1 of the Solicitation states, “The Contractor shall provide teleretinal imaging services…” On page 58 paragraph 4.6.8.3 states, The VA shall provide qualified professional tele-mental health services.” And on page 58 paragraph 4.6.8.4 states, “The Contractor shall provide space for clinical pharmacy telehealth services at the Contractor’s location as appropriate.” In addition to the group rooms asked about in question 10 above, how many individual telehealth rooms are required by the VA?
Three multipurpose telehealth rooms are needed.
Q10
In addition to the group rooms, telehealth rooms in questions 10 and 11 above, how many VA staffed personnel will be providing services in the Greenville CBOC and how many offices will be needed for those VA provided staff?
None
Q11
On page 72 paragraph 4.9.1 of the Solicitation includes a table with minimum space requirements which includes 5 exam rooms for two PACT Teamlets. And on page 73 it states, “Contractor shall provide space and administrative support for VA mental health practitioners who will provide services to enrolled CBOC patients via teleconferencing. At least 1 exam room(s) should be available for mental health services.” Do the 5 exam rooms listed on page 72 include the exam room for mental health services or is that an additional exam room?
No, the 5 exam rooms do not include the mental health exam room. A mental health exam room is required but can be a shared room.
Q12
On page 73 of the Solicitation Mental Health states, “Contractor shall provide space and administrative support for VA mental health practitioners who will provide services to enrolled CBOC patients via teleconferencing.” Is it acceptable to utilize a PACT team member for administrative support for VA mental health practitioner, or will a dedicated mental health clerk be required?
Yes, the support can be shared support for the clinic.
Q13
On pages 79-80 paragraph 4.8.21 of the Solicitation states, “Closed Circuit Television (CCTV): Shall be provided by the Contractor to monitor building entrances, restricted areas, mission critical asset areas, and alarm conditions. CCTV system shall be used for surveillance and observations of defined exterior areas, such as site and roadway access points, parking lots, and building perimeter, and interior areas such as hallways, common areas and waiting areas, CCTV system will be viewed from a VA location determined by the VA Physical Security Officer.” The requirement for the VA to be able to monitor the CCTV activities remotely appears to be intended for a VA leased and operated facility as opposed to a contractor operated and leased facility. And requires cellular based services and special equipment to accomplish. Our current contracted CBOC facilities across the U.S. utilize high resolution CCTV security cameras for both interior and exterior monitoring and routinely have recording capability of up to 30 days. Was it the VA’s intent to utilize this level of monitoring by the VA, as opposed to the contractor responsibility, to monitor the camera activities at the privately leased contracted facility? Or was this requirement placed in the specification in error as it is not normally utilized? Will you please clarify?
There is no current requirement of VA monitoring by the VA Security Officer.
Q14
Paragraph 2.6.1 requires both a certified Medical Laboratory Technician and a Phlebotomist. Would the VA consider removing the requirement for a certified Medical Laboratory Technician? This is a scarce specialty that generally performs more advanced services than those needed in the CBOC setting. For example, it is anticipated that the CBOC will only perform limited amounts of on-site test processing. We have found that most clinics of the size of the solicited CBOC require full-time phlebotomist staffing only.
A Medical Laboratory Technician is required but can also perform Phlebotomy if workload allow and great customer service is maintained.
Q15
Paragraph 2.4.5 requires the contract to provide LCSW staffing at the rate of 0.67 per PACT for PCMHI and General Mental Health services, while paragraph 2.7 indicates that “Specialty Mental Health services will be provided by the parent facility…” We further note that under the Schedule of Services, there is a separate line item for “Mental Health Services (General and PCMHI).”
a. Please clarify how patients will be added to the Mental Health billable roster since the VA is providing specialty mental health services and not the contractor.
b. Please confirm that 0.67 FTE per PACT the appropriate staffing level to account for both the PCMHI and general mental health services to be provided by the Contractor’s LCSW that is required by paragraph 2.4.5.
The Contract LCSW will provide both General Mental Health and PCMHI Services. Certain Mental Health services are not provided by the CBOC contractor may be provided by the Medical Center by VVC when appropriate.
Contractor’s question: Please clarify how patients will be added to the Mental Health billable roster since the VA is providing specialty mental health services and not the contractor.
VA’s response: We project a 10% increase in additional Mental Health roster billing due to specialty Mental Health from the VA.
Contractor’s question: Please confirm that 0.67 FTE per PACT the appropriate staffing level to account for both the PCMHI and general mental health services to be provided by the Contractor’s LCSW that is required by paragraph 2.4.5.

Medications will vary with the level of Primary Care complexity, case mix and staff privileges for MD or NP Primary Care providers but comparable to what is seen in the private sector for Primary Care.
The historical Vaccine rates for Veterans are approximately 40% for this catchment area.

The build out and transition timeframe indicated in paragraph 6.1.2 of 90 days is extremely restrictive and associated with high overtime costs for construction. We note that there are ongoing supply chain disruptions that are beyond the Contractor’s control due to the COVID-19 pandemic and other related factors. Would the VA consider allowing a longer build out time frame of at least 120 days?
The government will adjust the performance start period to begin 180 days after contract award, therefore there will be no negative impact on start-up costs.
Q55
Reflective of this issue, we note that recent VA CBOC solicitations by other VA hospital systems have called for 180-day build out time frame, such as 36C24920R0114 (Tupelo, MS) and 36C24920R0007 (Hopkinsville, KY).
Same responses above.

On 141-142 it states the Offeror shall complete and submit the following in two separate volumes: Volume I – Price and Volume II – Technical Capability, Past Performance, and SDVOSB/VOSB Evaluation and Subcontracting. However, on page 142, Volume I is Price, Volume II is Completion of SF 1449, Signed, Clauses/Provisionary Fill-Ins and Attachments, and Volume III is Technical Capability Geographic Location/Accessibility, Past Performance, Veteran Participation and Subcontracting. Then on page 143 it states Volume III includes Past Performance, SDVOSB or VOSB, and Subcontracting. Will the VA please clarify what goes in Volumes I, II and III because several items, i.e. Subcontracting, SDVOSB/VOSB and Past Performance are listed in more than one Volume?
In regard to Addendum to FAR 52.212-1 Instructions to offerors the following clarification to the submission Volumes is outlined below.
Volume 1
· Price
Volume II
· Completion of SF1449, Signed Amendments, Clauses/Provision fill-in and attachments
Volume III
· Volume III - Technical Capability, Geographic Location/Accessibility, Past Performance, Veteran Participation and Subcontracting
Q57
On page 142 indicates the completion and submission of the following items will constitute Volume
III and includes Sections: 1 Technical Capability, 2 Geographic Location/Accessibility, 3 Past Performance and 4 Veteran Participation. However, on pages 143-144 it states the following will be included and legible in each of the respective sections of Volume III and includes: Section 1 Technical – Offeror Capability which include Offeror’s Capability/Experience; Quality Staffing; Coordination and Continuity of Care; Key Personnel, including salary range worksheet and other supporting documents; and Implementation Plan. It also includes Section 2 Geographic Location/Accessibility, Section 3 Past Performance, Section 4 Veteran Participation, and Section 5 Subcontracting. And on page 149 in the Evaluation Factors the Technical Capabilities lists: a) Offeror’s Capability/Experience, b) Quality Staffing and c) Implementation Plan, but does not mention anything about Coordination and Continuity of Care or Key Personnel including salary range worksheets, etc. Is the Coordination and Continuity of Care and Key Personnel part of the evaluation factors, should it be added to the proposal, and if so, what should be addressed in these two sections?
And secondly, over the past 20+ years submitting CBOC proposals, we have never been required to provide salaries to the VA. Given that the Solicitation Schedule of Services is Per Member Per Month (PMPM) and the bidders are required to meet the service contract labor standards, why would a salary range worksheet and other supporting documents be required?
In regard to FAR 52.212-2 Evaluation Commercial Item the following clarification to the technical capability criteria is outlined below.
· (a) Capability/Experience
· (b) Quality Staffing/Key Personal (includes salary range worksheet)
· (c) Implementation Plan
In response to the inclusion of “Salary Worksheets” this will assist the Contracting Officer in determining the PMPM rates are fair and reasonable as well as assist the Contracting Officer in determining fair and reasonable pricing to any future Contract Modification requesting additional staff.

PWS, 2.1 MINIMUM PATIENT ALIGNED CARE TEAM (PACT) STAFFING REQUIREMENTS
The CLINS reflects base year of 1850 primary care patients; the final sentence of section 2.1 states the Total Estimated patients enrolled are 1700. Will the government please clarify, including any adjustment to the year-over-year anticipated growth?
The VA will keep the numbers as to account for additional enrollment as this is just an estimated quantity.

Please confirm that the cost of radiology procedures performed by the parent facility or by a community facility will be borne by the Government.
The government will be responsible for all cost related to the VA provided radiology services.

In Section B.2 Schedule of Services on page 6 of the Solicitation states, “* All unit prices shall be a Fixed-Price (FP) and all inclusive. Any build out cost incurred shall be the responsibility of the contractor. The government will only pay for services identified in PMPM rate. PMPM includes cost of unassigned patient visits, contractor shall not bill separately or propose an additional Line Item or bill separately for these costs." Does this mean that the contractor cannot include the costs of its buildout as line items in the calculation of the PMPM rate? And, if this is the case, and no VA funds can be used to offset the costs of buildout, then why is the contractor required to pay Davis-Bacon wage rates for construction, as specified in Section 4.9, on page 71 of the Solicitation?
We are required in solicitations where build-out may result, to identify the applicable Davis-Bacon wages. However, build-out costs and associated functions in this solicitation/requirement will be the responsibility of the contractor. It is at the discretion of the contractor on what they would like to submit for their PMPM rate as one line item in accordance with the solicitation requirements.

In Section B.3 (PWS), paragraph 6.2.1.1.4 PC CLIN Billing Roster and 6.2.1.1.5 MH CLIN Billable Roster on page 95 states that the roster will include all patients assigned to the Contractor’s site of care in PCMM with at least one qualifying encounter by the Contractor’s Primary Care provider within the previous 12 months. Will the current PCMM assigned patient’s previous 12 month qualifying encounter rollover to the new Contractor for both MH and PC patients, and be billable on day one?
Yes

16. On page 93 paragraph 6.1.2 of the Solicitation states, “The Contractor shall have 90 days from contract award to commencement of the provision of medical care to local Veterans.” Based on the size, scope and logistics of drawings, permits, and realistic construction timeframes, would the VA consider a minimum of 150-day start-up period for the cost effectiveness and efficiency for all systems start-up components?
The government will adjust the performance start period to begin 180 days after contract award.
