GRAND TOTAL ALL CLINS / ALL YEARS (BASE YEAR AND OPTION YEARS 1, 2, 3, & 4)

For the purposes of the award of this Contract, The Government intends to evaluate the option to extend services under FAR 52.217-8 as follows:

The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but not for more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be at the same rates as those in effect under the contract at the time the option is exercised. The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.

B.2 STATEMENT OF WORK

Statement of Work (SOW) Welch Allyn Connex Software Maintenance, Service, & License Agreement (For Welch Allyn Vitals Signs Monitors) Gulf Coast Veterans Health Care System Biloxi, Mississippi 39531 07/10/2019

1. GENERAL: Vital sign monitors are an integral part of patient care in any hospital facility. Vital sign monitors are used by used by clinicians and medically qualified personnel to monitor the following (to include but not limited to): noninvasive blood pressure (NIBP), pulse rate (PR), noninvasive functional oxygen saturation of arteriolar hemoglobin (SpO2), and body temperature in normal and axillary modes. The most likely locations for patients to be monitored are general medical and surgical floors, general hospital, and alternate care environments. While not a substitute or a replacement for direct patient bedside monitoring, the utilization of vital sign monitors can help to enhance the overall quality of care that is provided to patients.

2. BACKGROUND: In 2018 the Gulf Coast Veterans Health Care System (located at 400 Veterans Avenue, Biloxi, MS 39531) purchased Welch Allyn Connex Vital Sign Monitors that were outfitted throughout the entire hospital. Since initial purchase, use of these vital sign monitors has helped to improve the overall patient care mission at the facility. Welch Allyn Connex Vital Sign Monitors are just one component of Welch Allyn’s Electronic Vitals Data Management System which is comprised of Welch Allyn Connex Vital Sign Monitors and Welch Allyn Connex Software. Welch Allyn Connex Software is not just software but is a proprietary software solution/package which is responsible for the production/transmission of data and the enabling of the vital sign monitors to operate as one system (at full functionality and capacity). When in operation, Welch Allyn’s Electronic Vitals Data Management System allows vitals to be transferred electronically thereby eliminating transcription time and errors to include providing real-time data to other clinicians. Utilization of Welch Allyn’s Electronic Vitals Data Management System is an important part of the patient care delivery structure ensuring that crucial data relating to a patient’s vital signs are sent directly into the hospital’s monitoring database (i.e. VISTA-CPRS) for analysis and monitoring by patient care providers. Transmission, analysis, and monitoring of patient vital sign data is not possible when Welch Allyn Connex Vital Sign Monitors are used alone and require the integration of Welch Allyn Connex Software for creation and effective utilization of Welch Allyn’s Electronic Vitals Data Management System.

Upon initial purchase of the equipment (i.e. Welch Allyn Connex Vitals Signs Monitors) Welch Allyn Connex Software was provided to the facility. However, the subscription/license agreement for this software was only provided for a limited time period (12 months). Unfortunately, the current subscription/license agreement is expiring, and a new agreement is needed. As Welch Allyn’s Electronic Vitals Data Management System is vital to the patient care mission, a new subscription/license agreement for Welch Allyn Connex Software is needed.

3. OBJECTIVE: The Contractor shall provide all parts, equipment, supplies, tools, materials, labor, and personnel for the following: Welch Allyn Connex Software (Maintenance, Service, & License Agreement) (for the Welch Allyn Vitals Signs Monitors currently being used in the facility).

4. PERIOD OF PERFORMANCE: The expected period of performance for this requirement is a base year and four option years (which may or may not be exercised based on the needs of and at the discretion of the U.S. government). The period of performance start date will be mutually established between the government and the contractor at the earliest possible date after contract award.

5. WORK HOURS: Except by special alternative arrangement or when required, execution of services will be performed only during the facility’s normal business hours (8:00 a.m. to 4:30 p.m. Monday through Friday excluding federal holidays). The Federal Holidays which are observed by the federal government are annotated below.

-New Year’s Day -Martin Luther King’s Birthday -President’s Day -Memorial Day -Independence Day -Labor Day -Columbus Day -Veterans Day -Thanksgiving Day -Christmas Day -Any other day specifically declared to be a national holiday (per Federal Statute, Executive Order, or by the President’s Proclamation).

Usually holidays that fall on Saturday are observed on the preceding Friday and holidays that fall on Sunday are observed on the following Monday. Any questions pertaining to scheduling will be directed to the Contracting Officer Representative (COR) (or other authorized representative/ designee).

Work to be performed under the final contract will be coordinated/scheduled between the contractor and the Contracting Officer Representative (COR) (or other authorized representative/ designee) when required or if necessary before such work begins. If requested by the government, documentation of work completed under the final contract will be provided by the contractor to the government.

INSTRUCTIONS TO OFFERORS

This solicitation is unrestricted and is open to offerors in any socioeconomic category. The Government intends to solicit this requirement and award a subsequent contract without discussions or negotiations. However, if necessary the Government will seek clarification and additional information for offers that are submitted from any eligible offeror. Therefore, offerors are encouraged to provide their best offer initially because the opportunity to enter into discussions or to provide a better offer at a later time will not be possible. The Government intends to award to the offeror whose offer represents the best value to the government after consideration of all technical and price/non- price factors. FAR Part 52.212-2, Evaluation - Commercial Items does not apply to this solicitation. Pursuant to FAR subpart 13.106-2, the Government will award a Firm-Fixed Price contract to a single successful offeror/contractor whose offer/quote in response to this solicitation will be most advantageous to the Government. Award will be made to the lowest priced offeror that meets or exceeds ALL requirements of this solicitation and the Statement of Work.

ACCEPTABLE OFFERS: One single contract will be made as a result of this solicitation. For an offer to be considered for a potential contractual award the contractor must provide all CLIN items (services) listed in this solicitation. Contract award shall be made on an ALL OR NONE BASIS by the contracting officer. Partial offers that do not address, provide, and/or quote all CLIN items listed in the Price/Cost Schedule shall be deemed not technically acceptable and shall be rejected.
