STATEMENT OF WORK (SOW)
For Purchase of Additional Stands for Cardio Hub and Spoke Program Contract
GENERAL INFORMATION
1.0 Scope of Work: The Harry S. Truman Veterans' Administration Medical Center (HSTVA), located at 800 Hospital Drive, Columbia, MO 65203, requires the purchase of four (4) additional stands. These stands are essential to support the Cardio Hub and Spoke program by ensuring each Community-Based Outpatient Clinic (CBOC) affiliated with HSTVA is equipped with the necessary equipment to provide comprehensive cardio services. The objective of this project is to enhance the capacity and efficiency of the Cardio Hub and Spoke program by stationing these stands at each remote CBOC facility.
2.0 Background:
The Cardio Hub and Spoke program at HSTVA aims to provide integrated cardiovascular care across multiple facilities, ensuring veterans receive consistent and high-quality treatment. Due to the current distribution of resources, additional stands are required to equip each CBOC with the necessary infrastructure to support cardio equipment and facilitate effective patient care.
Increasing accessibility and improving the service delivery model are critical components of our future vision for this program.
3.0 Product Specifications/Technical Requirements
• Ensure all stands are durable, stable, and suitable for cardio equipment.
• Each stand should have adjustable height and a sturdy support structure.
• Carts must be compatible with HSTVA current Mobile Philips 5000 ultrasound system.
4.0 Government Furnished: Setup of these stands will be done by the facility.
5.0 Deliverables / Schedule:
• Delivery will be to HSTVA in Columbia. No installation required
• Product Manuals and Warranty Information:
Provide with each delivered stand.
To be provided on the day of delivery.
