STATEMENT OF WORK (SOW)

Title: Procurement of GYN Surgical Instruments and Sterilization Accessories Facility: VA Saint Louis Health Care System Requesting Service: Women's Health / Surgical Services

1. Objective

The purpose of this procurement is to acquire a full set of gynecologic surgical instruments, laparoscopic tools, and associated sterilization containers to support the expansion of GYN services for Women Veterans. These instruments are essential for procedures including but not limited to hysterectomy, laparoscopy, dilation and curettage, biopsy, and cervical diagnostics.

2. Scope

This acquisition covers reusable surgical instruments and sterilization support systems for open and minimally invasive GYN procedures. Instruments must be surgical-grade stainless steel, FDA-registered, autoclavable, and compatible with existing sterilization workflows.

3. Salient Characteristics

A. Clamps, Forceps, and Graspers

• Backhaus, Kelly, Pean, Roch Ochs Forceps: Towel clamps and hemostats with curved or straight jaws for securing drapes or controlling vessels. Standard sizes range 135mm–160mm. Serrated jaws for non-slip grip; box-lock mechanisms for secure closure.

• Wertheim & Heaney Clamps: Heavy-duty uterine and parametrial clamps (220– 250mm), curved and straight, with longitudinal serrations for firm tissue grasping during hysterectomy or myomectomy.

• Allis, Babcock, Gemini, and Sponge Forceps: Tissue-holding forceps with delicate fenestrated or toothed jaws. Allis forceps (4x5/5x6 teeth) and Babcock forceps ensure atraumatic holding of intestinal, uterine, or fallopian tube structures.

• Russian and Ferris-Smith Tissue Forceps: For firm grip of dense tissues or uterine walls. Toothed jaw designs, ideal for deep pelvic or abdominal work.

B. Dissecting & Surgical Scissors

• Mayo, Metzenbaum, Potts, Jorgensen Scissors: Used for dissection and cutting soft tissue. Metzenbaum for finer dissection; Mayo for heavier cutting. Available curved or straight; some with tungsten carbide (TC) inserts for prolonged edge retention.

• TC-Mayo, Hegar, and Heaney Needle Holders: Designed for suturing during GYN procedures; serrated jaws and optional TC inserts for secure needle grasping. Heaney types have curved profiles for deeper pelvic access.

C. Tenaculum & Uterine Manipulation Instruments

• Braun & Hulka Tenaculum: Designed for atraumatic cervical traction. Hulka tenaculum includes probe tip; Braun offers fine-point grasping. Ergonomic handles enhance precision.

• Uterine Polyp Sponge Forceps: Long, curved design ideal for retrieving polyps or securing intrauterine sponges.

D. Vaginal & Abdominal Retractors

• Deaver, Ribbon, Army-Navy, Dixon, and Kelly Retractors: Stainless steel retractors ranging from 210mm–330mm in length. Deaver and Dixon types provide deep cavity access; Ribbon retractors are malleable for customizable retraction.

• Balfour Retractor System: Includes center blade and wire side blades; self-retaining for pelvic exposure. Multiple sizes included to accommodate variation in patient anatomy.

• Universal Ring Retractor Kit: Modular self-retaining system for abdominal GYN surgery; adaptable to various surgical sites.

E. Vaginal Specula

• Jackson, Graves, Auvard, Steiner-Auvard Specula: Rigid stainless specula with various widths and lengths. Weighted options (e.g., Steiner-Auvard) for hands-free vaginal canal exposure. Concave design enhances visualization and light penetration.

F. Curettes & Dilators

• Kevorkian-Younge and Recamier Uterine Curettes: Blunt or sharp, rigid instruments used for sampling or removing endometrial tissue. Multiple diameters for tailored application (8.5mm to 15mm).

• Hegar Uterine Dilators: Graduated dilators in paired sizes (5/6 to 15/16mm), 195mm length. Smooth, polished finish allows gradual cervical dilation with minimal trauma.

G. Probes and Cannulae

• Sims Uterine Probe: Graduated, curved probe to assess uterine depth and orientation.

• Uterine Cannula with Cones: Dual-cone system allows sealed introduction of dye or fluids for hysterosalpingography or tubal testing.

H. Laparoscopic Instruments

• Scissors, Dissectors, Graspers (Maryland, Bullet Nose): 5mm diameter, 310–360mm length. Insulated for electrosurgery, ratcheted or non-ratcheted grips. Designed for precise dissection, coagulation, and tissue manipulation.

• Tenaculum, Durogrip Needle Holder: Laparoscopic instruments for uterine traction and suturing in minimally invasive procedures.

• Electrosurgical Accessories: Monopolar electrode tips, handles, and cables;

compatible with standard OR generators. Suction-irrigation tubes with Luer-locks for safe fluid management.

• Nathanson Retractor Set: 5mm and 6.5mm sizes; curved blades for liver and organ retraction during pelvic laparoscopy.

I. Sterilization & Storage Systems

• Rigid Containers (Full and ¾ Size): Primeline Pro aluminum containers with silicone seals and steam permeable lids. Available in 135mm and 90mm heights.

• Perforated Baskets: Stainless steel, various depths (56mm–106mm), sized to fit rigid containers. Enhance steam penetration and drying.

• Silicone Mats & Cushion Pads: Used to secure delicate instruments and protect edges during reprocessing.

• Stringers: Lockable metal bars (100mm–300mm) for holding ring-handled instruments in open position.

• Universal Holders & Arm Mechanisms: Mechanical arms with 3-joint locking systems, adapter bases for OR table integration. Used for laparoscopic or diagnostic instrument stabilization.

4. Performance Requirements

• All items must be new, unused, and delivered in sterile or ready-to-sterilize condition.

• Manufactured to meet ASTM or ISO standards for surgical instruments.

• Must be validated for repeated autoclave cycles (min 250°F/121°C).

• Must be interoperable with VA's existing sterilization and instrument tracking systems.

5. Warranty:

6. Delivery & Inspection

• The VA reserves the right to inspect and test all items upon delivery.

• Acceptance based on item completeness, absence of damage, and compliance with specification.

• Vendor will ensure that instruments are sent to the Attention of Biomedical Engineering and will include the VA Saint Louis PO Number on the shipping label.

• Delivery of oversized, palletized orders or orders that contain a large number of components will be coordinated with VA Saint Louis Biomedical Engineering POC who will communicate internally with Warehouse Staff.

7. Hours of Operation

• Normal hours of coverage are Monday through Friday, 7:00 AM to 3:30 PM Central

Standard Time excluding national holidays.

Note: Hardware/software update/upgrade installations, where applicable, may be scheduled and performed outside normal hours of coverage at no additional charge to the Government (unless it would be detrimental to equipment up-time; to be determined by the VA Saint Louis Biomedical Engineering POC).
