A revised Statement of Work Medical Oxygen Cylinders 3.4.2025 is provided below questions.
Answers to missed questions asked:
1. Are there any VA owned Cylinders utilized by either facility given there is a CLIN for Valve Replacement and Hydrostatic Testing? If so, how many GFE’s are owned on the VA’s property books, by line item?
All of our oxygen E tanks (Approx 300) and oxygen D tanks (approx 3) are owned by the VA. We will be renting the tanks for: Oxygen H tanks (12), Yellow Air Compressed (approx.
15), Grey Caron Dioxide (approx 20).
2. Request to remove the following requirements in YELLOW regarding the delivery ticket as these are nonstandard for an SDVOSB contract and would not be visible on a subcontractor’s delivery ticket:
PG 5 of 75, para 1.4, “The Contractor shall provide a legible signed and dated written or online document that identifies the number of cylinders at the ordering facility prior to delivery, the number of cylinders being delivered, the number of cylinders being picked-up, and the number of cylinders at the ordering facility after the delivery has been made.” No supplier provides a Proof of Delivery ticket upon delivery that notates that QTY balances of all cylinders on contract. For QTYs, delivery tickets only notate the # of full cylinders being delivered and the # of empty cylinders being returned.
PG 8 of 75, para 4.5: The contractor is required to provide a legible signed and/or electronic delivery slip that has been signed by the deliverer as well as the countersigned by the facility representative supervising the delivery. The delivery slip must include:
4.5.1.1. The number of cylinders at the ordering facility prior to delivery
4.5.1.2. The number of cylinders being delivered
4.5.1.3. The number of cylinders being picked-up
PG 9 of 75, para 6.5: The contractor is required to provide a legible signed and/or electronic delivery slip that has been signed by the deliverer as well as the countersigned by the facility representative supervising the delivery. The delivery slip must include: 6.5.1.1. The number of cylinders at the ordering facility prior to delivery
6.5.1.2. The number of cylinders being delivered
6.5.1.3. The number of cylinders being picked-up
6.5.1.4. Liquid Nitrogen fill (if applicable)
PG 10 of 75, para 10.1: The Contractor shall provide a legible signed and dated written document that identifies the number of cylinders at the ordering facility prior to delivery, the number of cylinders being delivered, the number of cylinders being picked-up, and the number of cylinders at the ordering facility after the delivery has been made.
PG 12 of 75, Para 12: Each delivery must contain a legible delivery ticket covering the following:
12.1.1. Contract Number,
12.1.2. Facility Purchase Order Number Need this line.
12.1.3. Delivery Order Number
12.1.4. Date of Order
12.1.5. Date of Delivery
12.1.6. Item number(s) and/or product identification number(s)
12.1.7. The number of cylinders being delivered
12.1.8. The number of cylinders being picked up
The Statement of Work has been modified and is attached. Edits are highlighted.
3. What are the technical submission requirements of the offeror as this is not notated in detail under Instructions to Offerors?
52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES
(NOV 2021) was added with 36C25525Q0175 0002.
4. What is the rationale in allowing the facility up to (1) year to pay Loss of Use (identified as the result of the annual joint audit)?
The Statement of Work has been modified and is attached. Edits are highlighted.
