Amendment 1, QUESTIONS IN RESPONSE TO 36C25522Q0187

1. How many days of orientation and training does this client require to onboard new providers? - This can vary depending on if the provider has previously worked at a VA and is familiar with our Medical Record System. Also, all providers need to be VA Out of OR Airway Management currently certified (OOORAM) which can be provided during orientation if needed.

Generally, orientation can be completed within 2-3 days.

2. Can providers work 24 hour shifts in-house? - Currently we are allowing 24 hours shifts

3. How many hospitalists are on duty per day shift and per night shift? - Currently 1 hospitalist on duty per day shift M-F, MOD provider is hospitalist nights, weekends (days and nights) and holidays (days and nights).

4. How many hospitalists are on duty per day shift and per night shift? - See answer to question #3.

5. Does the MOD have to perform invasive procedures? - No

6. Does the MOD manage ventilators? - Generally, no, as soon as a patient is intubated the process to transfer them to another facility is initiated.

7. Does the MOD cover ICU? - This facility does not have an ICU

8. Number of beds on the inpatient service? - We have a total of 10 beds with normal census of 2-4

9. Number of beds in the ICU? Open or closed ICU? - This facility does not have an ICU

10. Does the MOD manage / run the code blue response? Is there a response team? - Yes, when the MOD is on duty, they will run the code blue response. We have other staff on the response team.

11. Number of patients per day shift? - Day shifts are normally Saturday, or Sunday and the average is about 15. Occasional holiday day shift will be higher depending on holiday, average about 30.

12. Number of patients per night shift? - Night shifts would average 4 to 5 patient per shift.

13. How many MOD physicians on staff? Do they have a respiratory team? What subspecialty support staff are available? - This is a small facility with only urgent care not emergency room. We have 1 MOD physician on staff at a time. Usually, we have 2-3 total. We have respiratory therapist that are available on call nights and weekends. We have pharmacy, radiology and lab available on call after 6:00 pm weekdays. We have excellent cooperation with local hospital 5 minutes away with specialty providers available through their ER. Patient with emergent needs that we cannot provide are transfer there.

14. Do they have MRI or CT onsite? Do they have x-ray and ultrasound? - We have the services during regular business hours. Afterhours we have x-ray and head CT by on call staff.

Depending on which tech is on call, other services may be available. We can also send to local ER for emergent needs.

Amendment 1, QUESTIONS IN RESPONSE TO 36C25522Q0187

15. Are labs and pharmacy available 24 hours a day? If not, what are the available hours?

- Both are available in house until 6pm weekday and then on call.

16. Is the urgent care open 24/7? If not, what are the hours for urgent care? - Urgent Care is open 24/7

17. Does the MOD round on patients? If so, in which units? - MOD is not required to round on patients. They are expected to respond to questions or emergencies for inpatients either on acute ward or on CLC.

18. How often is the MOD called back to physically report to the CLC for a patient issue?

- Census for CLC has been greatly reduced during COVID, only now starting back admitting additional patients. Calls to physically report to CLC are limited, perhaps once a week. This may vary as census increases.

19. Are RNs able to call 911?, Do Nurse Practitioners have full scope practice authority to diagnose, treat, and write prescriptions? - The RN can call a code, but the physician will indicate that 911 needs to be called. The MAA is usually instructed by the physician to make that call. The RN can help facilitate that message to the MAA. Some of our NP do have full practice authority and some require a collaborative provider. The one that usually works in UC and the ones that cover CLC currently all have full scope practice authority.

20. Who is the incumbent contract holder or is this a new requirement? - Loyal Source Government Services LLC, VA255-16-A-0036.

21. What is the patient load for inpatient/urgent Care? - Inpatient census averages 2-4 patients but we are licenses for 10 beds. CLC is currently at a census of 12. Urgent Care night shift is generally 4 to 5 patients per shift, weekend 24 hour shifts average about 15, holiday shifts vary depending on the holiday but probably average about 30.

22. How many shifts per month? Currently we need coverage 3 weeks out of every 4 weeks.

Monday through Friday that is from 4pm until 8am the next morning and on Saturday/Sunday that is for 24 hours. Holidays are also 24 hours. The number of shifts per month can vary slightly depending on how the weeks fall and if there are holidays. Currently, one provider is covering one week and the other is covering the other two weeks.
