1. Per diems are not authorized under this IDIQ.
2. Fair Opportunity: Before issuing a task order, the ordering activity’s requirements will be forwarded to each contract holder through a Request for Quotation (RFQ) along with the evaluation criteria. In order to be considered responsive, contract holders shall provide all documentation within the predetermined time frames, i.e. quote due date. Evaluations will be based on CV’s (curriculum vitae)/resumes, licenses and/or certificates, timeliness/availability of proposed candidate and price. Based on a best value determination, the task order shall be placed with the contract holder that represents the best value.
3. The Government estimates, but does not guarantee, that the annual volume of professional staffing services under this IDIQ.
4. This IDIQ does not obligate any funds. The Government is obligated only to the extent of authorized purchases actually made under the IDIQ via the issuance of a task order.
5. Authorized users are NCO 15 Medical Services Warranted Contracting Officers. Authorized users shall only be allowed to issue orders against this IDIQ if funds are certified and an order number is assigned.
6. The services specified in the agreement may only be changed by written modification which will be prepared and executed by the Network Contracting Office (NCO) 15. The following actions are the sole responsibility of the Contracting Officer for this agreement; no other personnel are authorized to:
a. Make changes to IDIQ provisions. This includes changes involving increases and/or decreases in agreement price, as well as no cost changes.
b. Require extras or work outside the scope of the agreement.
c. Authorize delivery or disposition of Government property not authorized by the agreement.
d. Extend the completion time established in the contract.
e. Terminate the contract in whole or in part.
f. Render a decision under the Disputes Clause.
g. Issue instructions to the contractor to stop or start work, to exclude the scheduling of contracted services.
7. Guaranteed Minimum and Contract Maximum:
a. The Guaranteed Minimum is applicable during the base period of the fiscal year in which award was made. The Guaranteed Minimum is $1,000.00 and is only applicable during the base period of the fiscal year in which award was made.
b. The Contract Maximum is $8,010,000.00
B.3 PERFORMANCE WORK STATEMENT
1. GENERAL
1.1. Services Required: The Department of Veterans Affairs (VA) St. Louis Health Care System provides imaging services and have medical needs for such services. This acquisition will supplement VA in providing comprehensive imaging support to Veterans in need at the John Cochran Barracks, VA St.
Louis Health Care System (VASTLHCS). It is VA’s intention to achieve a pool of providers for each Radiologic Technologist category identified in paragraph 2.15. The pool of providers will be utilized in order to cover lapses in the schedule due to absences, illness, etc.
1.1.1. Services required are on-site PRN (as needed).
1.1.2. Providers are not guaranteed a 40-hour work week or 8-hour workday.
1.1.3. Services required include the following radiologic technologist modality services:

1.1.4. Estimated Full Time Equivalents (FTE)
1.1.4.1. The contractor shall provide non-personal healthcare services for up to 3 FTE Diagnostic
Radiology Technologist (DRT), 2 FTE Computer Tomography Technologists (CT), 1 FTE Ultrasound Technologists (US), 1 FTE Mammography Technologist (Mammo), 2 FTE Magnetic Resonance Imaging Technologist and .75 FTE Nuclear Medicine Technician to perform the requirements of this contract. A full-time equivalent employee is defined as a Contractor’s employee working eight hours in a day. A Contractor’s employee must be present at the VASTLHC facility and must actually be performing the required services for the period specified in the contract or the contract costs will be decreased accordingly during each billing cycle. The payment for any leave, including sick leave, vacation time and authorized Federal holidays is the responsibility of the Contractor.

1.4.34. Veterans Health Administration (VHA): The central office for administration of the VA medical centers throughout the United States. The VHA is located in Washington, D.C.
1.4.35. Veterans Integrated Services Network (VISN): The regional oversight for the VA medical centers in California.
1.4.36. Veterans Affairs St. Louis Health Care System (VASTLHCS): Unless identified with the name of a different VA medical Center, for purposes of this contract, this term shall mean the VA St. Louis Health Care System.
2. QUALIFICATIONS, LICENSES and CREDENTIALING
2.1. Staff/Facility: The contractor will be responsible for ensuring that each technologist providing services under this contract is fully trained and completely competent to perform the required services covered by this contract. The qualifications of Contractor’s personnel are subject to review by the Chief Radiology Administrator or his/her designee and the Chief of Radiology or his/her clinical designee. A clinical COR may be appointed, however, only the CO is authorized to consider any contract modification request during the administration of the resultant contract.
2.1.1. License: Contractor’s Radiology Technologist (s) assigned by the Contractor to perform the services covered by this contract shall be American Registry of Radiologic Technology (ARRT) certified. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. Copies of the license(s) shall be submitted with the Contractor’s proposal.
All licenses held by the contractor’s personnel working on this contract shall be full and unrestricted licenses.
2.1.1.1. Licensing, registration and certification information shall be kept current.
2.1.1.1.1. Updated licensing, registration and certification shall be provided as they are renewed.
2.1.1.1.2. Licensing, registration and certification for Contractor’s technologist(s) shall be kept current. There are no acceptable deviations.
2.1.1.1.3. The Contractor is required to report to the Contracting Officer within twenty-four hours of Contractor’s discovery any disciplinary action taken by the State Board or the institution against radiologic technologists performing under the contract.
2.1.2. Board Certification/Eligibility: Contractor’s Radiology Technologist (s) performing under this contract shall meet or exceed recognized national standards established by The Joint Commission (TJC) and have completed training from an accredited institution, and shall have received a certificate of completion of training in Radiologic Technology and have a current certification or registry in their respective sub-specialties:
2.1.2.1. The Computer Tomography (CT) Technologist must have an American Registry of Radiologic Technologists ARRT) certificate and current CT license.
2.1.2.2. The Ultrasound Technologist must have an American Registry for Diagnostic Medical Sonographer (ARDMS) certificate.
2.1.2.3. The MRI Technologist must have an American Registry of Radiologic Technologist (ARRT) certificate in MRI.
2.1.2.4. The Mammography Technologist must have an American Registry of Radiologic Technologist (ARRT) certificate and current Mammography license.
2.1.2.5. The Nuclear Medicine Technologist/Technician must be a Certified Nuclear Medicine Technologist (CNMT) through the Nuclear Medicine Technology Certification Board (NMTCB) or American Registry of Radiologic Technologist (ARRT) with a certificate in Nuclear Medicine Technology (N).
2.1.2.6. Personnel assigned by the Contractor to perform Radiologic Technologist services require a minimum two years of hospital experience or two years of ambulatory care experience.
2.1.3. Technical Proficiency: Contractor’s Radiology Technologist (s) shall be technically proficient in the skills necessary to fulfill the government’s requirements, including the ability to speak, understand, read, and write English fluently under authority of U.S.C. 7402(d) and 7407(d).
2.1.3.1. Contractor shall provide documents upon request of the CO/COR to verify current and ongoing competency, skills, certification, and/or licensure related to the provision of care, treatment, and/or services performed.
2.1.3.2. Contractor shall provide verifiable evidence of all educational and training experience, including any gaps in educational history for all Contractor’s Technologist (s).
2.1.3.3. Contractor Technologist (s) shall be responsible for abiding by the VA Facility's Medical Staff By-Laws, rules, and regulations (referenced herein) that govern medical staff behavior.
2.1.4. Basic Life Support (BLS) Certification
2.1.4.1. In accordance with VHA Directive 1177, Cardiopulmonary Resuscitation, Basic Life Support, and Advanced Cardiac Life Support Training for Staff, all staff having direct clinical contact, or who responds to a code, receive basic life support training.
2.1.4.2. All clinically active staff employed within VA requires BLS certification. Clinically active staff normally includes all individuals that provide direct clinical care to patients. At a minimum, the clinically-active staff must include privileged, licensed independent practitioners (LIP), advanced practice registered nurses, physician assistants, licensed nurses, pharmacists, unlicensed assistive personnel (UAPs), health aid and technicians, medical instrument technicians, radiology and nuclear medicine technologists, respiratory therapists, and clinical therapists (mental health, psychology, social work), dental assistants, dental hygienists, and any staff member who provides coverage for inpatient and/or residential VA-treatment programs or VA home care programs.
2.1.4.3. VA accepts American Heart Association (AHA) and Military Training Network (MTN) certifications only.
2.1.5. Specific Requirements
2.1.5.1. Diagnostic Radiology Technologist
2.1.5.1.1. Diagnostic Radiology Technologists are responsible for the performance of diagnostic radiology procedures and to perform the full complement of exams.
2.1.5.1.2. Technologist is responsible for room preparation in anticipation of the examination, i.e., assuring that the supplies necessary for the examination are present and in a state of readiness.
2.1.5.1.3. Maintain records of patients treated, examinations performed, views taken, and settings used. Interacts with the Medical Center’s VISTA Computer system to register examinations and to complete the parts of the radiology exam tracking data system which requires technical support. The technologist shall perform other duties as assigned.
2.1.5.1.4. Converts radiology exams to digital computer images in VISTA Imaging using Digital Radiography (DR).
2.1.5.1.5. Works independently on all calendar days, evening, night and weekend tours of duty and holidays. Works without direct supervision during normal tours of duty; relies on supervisory technologist for resolution of problems with equipment/patient flow beyond the scope of the technologist.

2.1.5.2. Computer Tomography Technologist
2.1.5.2.1. The CT Technologist must be fully competent to operate Siemen’s 64 & Flash 128Multi-slice scanners or similar type of equipment, and to perform the full complement of contrast and non-contrast exams generated by this technology, as demonstrated by past work experience.
2.1.5.2.2. The CT Technologist shall be responsible for entering exam information into the medical centers Veterans Health Information Systems and Technology Architecture (VISTA) as required by service policies and procedures. The technologist shall perform other duties as assigned.
2.1.5.2.3. The CT Technologist shall work under the general guidance of the Supervisory Diagnostic Radiology Technologist and the direct guidance of the radiologist during exams.
2.1.5.2.4. Working hours shall be during the day, evening, and overnight shifts as scheduled with the Contractor. The Contractor technologist should be available holidays and weekends.
2.1.5.2.5. Work hours are listed in Central Time (CT). The current shifts are listed below but are subject to change:

2.1.5.3.1. The MRI Technologist must be fully competent to operate Siemen’s 1.5T Aera and 3.0T Skyra scanners or similar type of equipment, and to perform the full complement of contrast and non-contrast exams generated by this technology, as demonstrated by past work experience.
2.1.5.3.2. The MRI Technologist shall be responsible for entering exam information into the medical centers Veterans Health Information Systems and Technology Architecture (VISTA) as required by service policies and procedures. The technologist shall perform other duties as assigned.
2.1.5.3.3. The MRI Technologist shall work under the general guidance of the Supervisory Nuclear Medicine / MRI Technologist and the direct guidance of the radiologist during exams.
2.1.5.3.4. Working hours shall be during the day, evening, and overnight shift as scheduled with the Contractor. The Contractor technologist should be available holidays and weekends.
2.1.5.3.5. Work hours are listed in Central Time (CT). The current shifts are listed below but are subject to change:

2.1.5.4.1. The Ultrasound Technologist must be fully competent to operate GE Logiq E9 or similar type of equipment, and to perform the full complement of exams generated by this technology, as demonstrated by past work experience.
2.1.5.4.2. The Ultrasound Technologist shall be responsible for entering exam information into the medical centers Veterans Health Information Systems and Technology Architecture (VISTA) as required by service policies and procedures. The technologist shall perform other duties as assigned.
2.1.5.4.3. The Ultrasound Technologist shall work under the general guidance of the Supervisory Diagnostic Radiology Technologist and the direct guidance of the radiologist during exams.

2.1.5.5.2. The Mammography Technologist must be fully competent to operate Hologic Selenia or similar type of equipment, and to perform the full complement of exams generated by this technology, as demonstrated by past work experience.
2.1.5.5.3. The Mammography Technologist shall work under the general guidance of the Supervisory Diagnostic Radiology Technologist and the direct guidance of the radiologist during exams.
2.1.5.5.4. Work hours are listed in Central Time (CT). The current shifts are listed below but are subject to change:
• Monday through Friday
• First Shift: Staggered Shifts o 6:30am – 3:00pm o 7:00am – 3:30pm o 8:00am – 4:30pm
2.1.5.6. Nuclear Medicine Technician
2.1.5.6.1. The Nuclear Medicine Technologist must be fully competent to operate Siemens Evo, Intevo and Biograph mCT Flow scanners or similar type of equipment and perform the full complement of Nuclear Medicine SPECT, SPECT/CT, and PET/CT exams.
2.1.5.6.2. The Nuclear Medicine Technologist shall be responsible for entering exam information into the medical centers Veterans Health Information Systems and Technology Architecture (VISTA) as required by service policies and procedures. The technologist shall perform other duties as assigned.
2.1.5.6.3. The Nuclear Medicine Technologist shall work under the general guidance of the Supervisory Nuclear Medicine / MRI Technologist and the direct guidance of the radiologist during exams.
2.1.5.6.4. Working hours shall be during the day, evening, and overnight shift as scheduled with the Contractor. The Contractor technologist should be available holidays and weekend
2.1.5.6.5. Work hours are listed in Central Time (CT). The current shifts are listed below but are subject to change:
• Monday through Friday

2.1.5.7. All Radiologic Technologists must complete all other requirements (orientation, etc.) as detailed in this contract.
2.1.6. Orientation/Training Requirements
2.1.6.1. All technologists shall receive initial orientation provided by the Station on their first day of duty. The Contractor employees will be given an orientation on operation of equipment by the Supervisory Diagnostic Radiology Technologist; operation of the VISTA Radiology Package, Computer Radiology equipment, and scanners; reading departmental Standard Operating Procedures (SOP’s), Medical Center Memoranda (MCM’s), Fire and Safety Regulations; and obtaining a parking permit and ID badge. This orientation will take approximately four (4) hours to complete and shall be recorded on the contracted employee’s time and attendance record in order to receive compensation by the Government.
2.1.6.2. Orientation is an estimated period of time and should a candidate need additional training in any one area, training shall continue until the technologist has been deemed competent by the service.
2.1.6.3. Technologists shall be scheduled for orientation according to the effective date of the award or modification that identified the technologist as Key Personnel. If a technologist is unable to begin the orientation for a period of time greater than four weeks, the next technologist shall be contacted for orientation.
2.1.6.3.1. DRT Orientation: Orientation/ training shall occur the first five weeks of contracted performance. Technologists will receive one week of training in each area of Radiology (General DRT, OR, GI/Fluoro, Portables, and general computer (VistA, CPRS, PACS)) to learn VA’s equipment and protocols. Technologist will be scheduled for Monday – Friday, 8:00 am – 4:30 pm. If the technologist is unable to complete five consecutive weeks of orientation, the training will continue where it left off until all areas have been completed and the technologist is deemed competent by the service.
2.1.6.3.2. DRT Compressed Orientation: Orientation/training shall only be offered to the technologist who is unable to attend the five weeks of consecutive orientation. The compressed schedule consists of 12 days and shall be grouped in three consecutive weekdays in each area of Radiology (General DRT, OR, GI/Fluoro).
2.1.6.3.3. CT Orientation: Orientation/ training shall occur the first 4-6 weeks of contracted performance. Technologists will be assigned to work with another technologist.
Technologists will receive training in VistA, CPRS, and Vitrea (post processing in CT).
Additionally, the technologist will learn department protocols in CT. Technologist will be scheduled for Monday – Friday, 7:00 am – 3:30 pm or 7:30 am – 4:00 pm. If the technologist is unable to complete consecutive weeks of orientation, the training will continue where it left off until all areas have been completed and the technologist is deemed competent by the service.
2.1.6.3.4. CT Compressed Orientation: Orientation/training shall only be offered to the technologist who is unable to attend the 4-6 weeks of consecutive orientation. The compressed schedule consists of 12 days and shall be grouped in three consecutive weekdays.
2.1.6.3.5. Ultrasound Tech Orientation: Orientation/ training shall occur the first 4-6 weeks of contracted performance. Technologists will be assigned to work with another technologist. Technologists will receive training in VistA, CPRS, and Vitrea (post processing in CT). Additionally, the technologist will learn department protocols inUS.
Technologist will be scheduled for Monday – Friday, 7:00 am – 3:30 pm or 7:30 am – 4:00 pm. If the technologist is unable to complete consecutive weeks of orientation, the training will continue where it left off until all areas have been completed and the technologist is deemed competent by the service.
2.1.6.3.6. Ultrasound Tech Compressed Orientation: Orientation/training shall only be offered to the technologist who is unable to attend the 4-6 weeks of consecutive orientation. The compressed schedule consists of 12 days and shall be grouped in three consecutive weekdays.
2.1.6.3.7. MRI Orientation: Orientation/ training shall occur the first four weeks of contracted performance. Technologists will be assigned to work with another technologist. Technologists will receive training in VistA, CPRS, and PACS. Additionally, the technologist will learn department protocols and procedures. Technologist will be scheduled for Monday – Friday, 10:00am – 6:30 pm. If the technologist is unable to complete consecutive weeks of orientation, the training will continue where it left off until all areas have been completed and the technologist is deemed competent by the service.
2.1.6.3.8. MRI Compressed Orientation: Orientation/training shall only be offered to the technologist who is unable to attend the four weeks of consecutive orientation. The compressed schedule consists of 12 weekdays.
2.1.6.3.9. Mammography Orientation: Orientation/ training shall occur the first two weeks of contracted performance. Technologists will receive one week of training in each area of Mammography, (General, Stereotactic Biopsies, and VA computer (VistA, CPRS, MRS, and PACS)) to learn VA’s equipment and protocols. Technologist will be scheduled for Monday – Friday, 7:00 am – 3:30 pm. If the technologist is unable to complete two consecutive weeks of orientation, the training will continue where it left off until all areas have been completed and the tech is deemed competent by the service.
2.1.6.3.10. Nuclear Medicine Tech Orientation: Nuclear Medicine Orientation: Orientation/ training shall occur the first four weeks of contracted performance. Technologists will be assigned to work with another technologist. Technologists will receive training in VistA, CPRS, and PACS. Additionally, the technologist will learn department protocols and procedures. Technologist will be scheduled for Monday – Friday, 8:00am – 4:30 pm. If the technologist is unable to complete consecutive weeks of orientation, the training will continue where it left off until all areas have been completed and the technologist is deemed competent by the service.
2.1.6.3.11. Nuclear Medicine Orientation is limited to two technologists at a time. Technologists shall be scheduled for orientation according to the effective date of the award or modification that identified the technologist as Key Personnel. If a technologist is unable to begin the orientation for a period of time greater than two weeks, the next technologist shall be contacted for orientation.
2.1.6.3.12. Nuclear Medicine Tech Compressed Orientation: Nuclear Medicine Compressed Orientation: Orientation/training shall only be offered to the technologist who is unable to attend the four weeks of consecutive orientation. The compressed schedule consists of 12 weekdays.
2.1.7. Technologist Administration
2.1.7.1. The technologist is required to use the VISTA computer to case edit all work, i.e., enter the name of the technologist performing the exam, and any modifiers to the examination. All examinations must be case edited within fifteen minutes of the exam’s completion. To be considered fully successful in this element, he/she cannot fail to complete as identified more than 15 times annually.
2.1.7.2. To be considered fully successful, a technologist must be in compliance with all departmental and medical center safety policies. Copies of the medical center radiation safety policies are maintained in Radiology Department and may be viewed, by appointment, by contacting the Supervisory Technologist/RSO. If a technologist violates these policies more than twice annually, he/she is unsuccessful.
2.1.8. Setting up Radiographic Rooms and Exams
2.1.8.1. If any piece of Radiology Equipment stops functioning properly, it is the technologist's responsibility to inform the radiology supervisor and/or the COR.
2.1.8.2. It is the responsibility of the technologist assigned a room to set it up for procedures and to keep it clean. Rooms are subject to random inspections and are expected to be maintained to a level of cleanliness that complies with medical center infection control policies. This includes ensuring the equipment is wiped down with Cavi wipes between each patient and at the end of the day. All supplies are to be stocked or restocked, in an orderly manner, daily or when supplies are low. There will be an adequate supply of linens available in the individual exam rooms, and all expended supplies/linens from the previous exams should be properly disposed of in the room as appropriate for either the trash can or the dirty linen bag. When the dirty linen bag is full, the technologist shall take it to the EMS linen chute and replace with a new empty bag.
Environmental Management Service will be responsible for maintaining the floors and emptying the trash. The cleanliness of the equipment is the sole responsibility of the technologist. If the technologist fails to comply more than three times annually, he/she is unsuccessful in this element.
2.1.8.3. Exposure Control Plan: The technologist is responsible for observing the departmental and hospital policies for the Exposure Control Plan for infection control or safety policies. Copies of the medical center exposure control plan are maintained in the Radiology Department and may be viewed, by appointment, by contacting the Supervisory Technologist or Administrative
Officer. To be considered fully successful, the technologist cannot violate these policies more than three times annually.
2.1.9. Probationary Period Requirement: The first 3 months of employment are probationary. During this period, a contracted technologist can be removed from the Key Personnel and contract at any time through a modification. Grounds for removal can include, but is not limited to: (1) inability to meet the required technologist skill set as stated by the Performance Work Statement and the service’s established quality standards, (2) unsatisfactory performance by the technologist, and (3) inability to adequately function within the capacity needed as a technologist.
2.1.10. Continuing Education Unit (CEU) Requirements: Contractor will provide the COR copies of current CEUs as required or requested by the VAPAHCS. Contractors registered or certified by the American Registry of Radiologic Technologist (ARRT), Nuclear Medicine Technologist Certification Board NMTCB), and the American Registry of Diagnostic Medical Sonographer (ARDMS) shall continue to meet the minimum standards for CEU to remain current.
2.1.11. Training (BLS, CPRS, and VA MANDATORY): Contractor shall meet all VA educational requirements and mandatory course requirements defined herein; all training must be completed by the Contractor’s Technologist (s) as required by the VA before being granted access to VA information and its systems.
2.1.11.1.1. Remedial Training: When notified, contract employees must complete remedial training and education to address any detected compliance exceptions.
2.1.11.1.2. Proof of Training: Contract employees are responsible for submitting proof of awareness and remedial training completed to the Contracting Officer’s representative (COR) for this contract. The COR will retain proof of training in accordance with applicable Records Control Schedule.
2.2. Credentialing and Privileging: Credentialing and privileging will be done in accordance with the provisions of VHA Handbook 1100.19. This VHA Handbook provides updated VHA procedures regarding credentialing and privileging, to include incorporating: VHA policy concerning VetPro; credentialing during activation of the facility Disaster Plan; clarifications for the Summary Suspension of Privileges process in order to ensure both patient safety and practitioner rights, the credentialing requirements for other providers.
2.2.1. Credentialing shall be completed prior to the initial appointment and contract performance.
2.2.2. All contracted personnel who require access to the Department of Veterans Affairs and its computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all sub-contractor personnel requiring the same access.
2.2.3. All contracted personnel shall be required to provide personal information, undergo a Federal Bureau of Investigation (FBI) National Criminal History Fingerprint Check, and obtain a USAccess Badge prior to being permitted access to any such facility in performance of this contract. No contracted personnel will be permitted routine, unescorted access to a VA facility without a proper USAccess Badge.
2.2.3.1. Initial applications for clinical privileges and applications for renewal of privileges must be submitted to the CO, upon notice from the Contracting Officer. Prior to providing services at VASTLHCS, all contractor personnel must be verified by the CO as having been credentialed. Additionally, if requested, the contractor shall make all proposed personnel available for interview by telephone prior to selection before the credential verification process begins.
2.2.3.2. The Contractor is responsible to provide complete and timely documentation to support the Credentialing Process which will initially commence on award of the contract and then upon contract renewal (or extension). Contractor employees must be able to pass the VASTLHCS’s credentialing process in accordance with the Department of Veterans Affairs’ guidelines and regulations.
2.2.3.2.1. The Contractor shall submit or have Contracted personnel submit the following required forms to the VA Office of Security and Law Enforcement within 30 days of receipt.

2.2.3.2.1.5. Self-Certification of Continuous Service
2.2.3.3. Should the personnel proposed by the Contractor to provide services under this contract be denied privileges, or should the privileges of Contractor personnel be suspended, terminated, or revoked, the contractor, as well as the employee (s) in question, shall be notified of the basis for such actions.
2.2.3.4. VetPro is VHA’s electronic credentialing system and shall be used for credentialing all
Radiologic Technologists who are granted privileges or are credentialed for other reasons.
2.2.4. VASTLHCS reserves the right to refuse or dismiss contract personnel whose personal or professional conduct jeopardizes patient care or the regular and ordinary operation of the facility. Reasons for refusal or dismissal include, but are not limited to unsatisfactory performance prior to and/or during the term of the contract, failure to receive favorable adjudication during a VA background investigation, failure to satisfy the requirements of the contract, physical or verbal abuse to patients, staff, or visitors, intoxication or debilitation resulting from drug use, theft, patient abuse, dereliction or negligence in performing directed tasks, ethical misconduct, conduct resulting in formal complaints by patients or other staff members, and any other valid reason considered objectionable.
2.2.5. USAccess Badge will identify individuals as Contractor employees. Contractor employees shall display their USAccess Badge on their person at all times while working in a VA Facility and shall present their cards for inspection upon request by VA officials or VA security personnel.
2.2.6. The Contractor Shall be responsible for all USAccess Badge issued to the contractor’s employees and shall immediately notify the contracting officer if any USAccess Badge cannot be accounted for. The contractor shall notify the contracting officer immediately whenever any contractor employee no longer has a need for the VA issued USAccess Badge(s) (e.g., employee terminates employment with the Contractor, Contractor employee is removed from the Key Personnel Roster). In such cases, the contracting officer will instruct the contractor on how to return the PIV card. Upon expiration of this contract, the contract ING officer will instruct the contractor on how to return all VA issued USAccess Badge not previously returned. The contractor shall not return USAccess Badge to any person other than the individual named by the contracting officer.
2.2.7. Failure to comply with the Contractor personnel security requirements may result in termination of the contract for cause.
2.3. Key Personnel: All Contractor Radiologic Technologists provided under this contract shall comply with the education and training requirements in order to work with VA patients and shall be identified as Key Personnel. All Key Personnel working on this contract must have Contracting Officer (CO) approval once a clearance submittal by a contract employee has been successfully accepted by and access granted by the (CO).
2.3.1.1. Key Personnel assigned shall be identified by a Key Personnel Roster on the issued Task Orders.
2.3.1.2. The Contractor shall provide complete resumes, education and training for the proposed Radiologic Technologists, and any additional information requested by the CO. The CO will notify the Contractor of the decision on the proposed Radiologic Technologists. The Task Order will be issued to reflect approved key personnel.
2.3.1.3. Any changes, whether additions or deletions, to Key Personnel shall be supported by a bilateral modification to the Task Order. The Contractor is responsible for notifying the CO and COR of any changes.
