a. The purpose of this Amendment 0004 is to provide clarifications on questions and answers #36 and #37 which were posted in the Solicitation Amendment 0003 on 07/06/2020. Please see clarifications in page 2.

b. Proposal due date has been extended from July 13, 2020 to July 17, 2020 at 2:00 PM CDT.

c. All other terms and conditions remain unchanged and in full force.

Clarifications on Questions and Answers #36 and #37 which were issued in the Solicitation Amendment 0003 on 07/06/2020:

1. Does the equipment labeled as “CC” (contractor furnished contractor installed) required to be Stryker equipment? All to be provided by contractor.

A/E Answer: Yes, turnkey by contractor and yes Stryker equipment.

1. Will the VA please clarify if offerors are to include the Stryker equipment package and supporting software in their price proposal?

VA Answer: Yes

1. Was spec section 11 76 00 and the S-Series Equipment Management System Pre-Install Manual and preliminary drawings, only provided to the contractor as a reference for coordination of VA supplied equipment?

A/E Answer: All is basis of design. Stryker is quoting to all contractors as full turnkey to VA.

1. Are other manufacturers than Stryker, acceptable to provide the OR equipment?

A/E Answer: No only Stryker (unless VA directs differently).

1. If it is acceptable to use other manufacturers, please provide a list of acceptable manufacturers.

Answer: No
