This amendment makes the following changes:

· These “Est Minimum Qty” are these quantities for every 6 months or for the year total? Under Kansas City it shows 5 Isolators. Is that 5 to be certified every six months or total for the year? Estimate 5 hoods every six months, it looks like we are going to have 6 – so six certifications every 6 months was the minimum estimate.

· The “Est Minimum Qty” under Kansas City VAMC shows 5 with a Max Qty of 7. What does that mean? Are you planning on purchasing more hoods? Yes. We have a 6th in contracting for award, we planned for worst case scenario at #6 hoods. If one breaks, or we need a recertification due to unforeseen issues then we have a spare (#7).

· Under the Kansas City VAMC, it shows 64 surface samples. Is that total for each round of testing. Viable samples states is should be taken every three months, so is it 64 surface samples each quarter or is it 64 samples over a four quarter timeframe? 64 over a four quarter time frame (estimated turnover/students/retesting per year)

· How many cleanrooms are located at each facility? It shows Particle Counts(4) and HEPA filters(3) but not cleanroom QTY per facility. The # of hoods/laminar that will need certification twice a year:

· Will we be able to organize the certification schedule?

Order of which we can do each facility as long as it is completed before certification due date. Yes, as long they are certified by the due date as pre-scheduled with facility.

· Are the Media Fill Validations to be completed at time of certification or can be completed on a different date? It shows under Kansas City it has Est Min. Qty of 60. How do you get that number? The number is based on current staff, including staffing turnover, retesting, and students. The media fill can be completed at same time as the certification – there are off tours/night shift and new employees that would require off cycle testing.

· In the Media Fill Test Kit Sets, what are you looking to be included in these kits? All consumables that are required to meet the media fill testing, see medium risk: http://www.pharma-choice.com/stage/wp-content/uploads/2016/05/Hardy-Diagnostics-Catalog-1.pdf

· How often is the HEPA Filter Leak Testing completed? Every 6 months

· What is the timeframe if retesting is needed to be completed if any failures may occur in Media Fills/Viables? If an employee fails, the retesting will need to be coordinated between the vendor & facility as soon as a failed test is identified. 2 weeks?

· How often are viable samples (air & surface) to be taken at each VAMC?

Monthly? Every 3 Months? Every 6 Months? Minimum every 6 months.

· Are any new hires tested upon an agreed date to be complete their Media Fill Validations? To be coordinated between the vendor & facility as identified. What is a realistic expectation from NCO15?

· Are we able to set the certification schedule with each VA location? Yes – to be coordinated with the Facility POC identified upon award. This would be done to save you money and us time with the order we hope to follow.

· How was it determined how many Air Samples, Surface Samples and Growth Promotion samples appeared on the chart under “Est Min Qty”? Estimates were determined based on the number of hoods, IV Rooms, ante rooms, and projected needs over the course of a potential Base + 4 year period. Estimated Min Qty is the minimum number of areas that need samples as identified per facility.
