B.4 STATEMENT OF REQUIREMENT
CLINICAL KNOWLEDGE PRODUCT
Subscription to a Patient Point of Care On-line Data Base
1.1 Introduction:
The Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Harry S. Truman Memorial Veterans’ Hospital, Columbia, MO, requires an existing commercial, off the shelf, web based, Point of Care Clinical Knowledge Product that will be made available to unlimited number of users in the Harry S. Truman Memorial Veterans’ Hospital and its associated Community Outpatient Clinics (CBOCs).
This product may also be referred to as bedside information tools because it has been designed to provide information at the patient’s bedside or point of care. They tend to provide information that will directly affect patient care. They can be a good source for background and evidence based information. What distinguishes these products from sources such as electronic textbooks or bibliographic citations databases that may link to full text is the emphasis on point-of-care, patient-oriented information. These products are distinctly different from text-based, point-of-care sources such as printed guidelines: these electronic resources are updated much more frequently than print resources and provide tools for easily searching and sharing information.
Harry S. Truman Memorial Veterans’ Hospital’s goal is to insure uniform availability of a high quality, clinical, knowledge based resource to support and improve clinical decision making at the point of care.
1.2 Objectives
1.2.1 Provide medical center wide institutional access for unlimited simultaneous users from the employee’s desktop to the Point of Care Clinical Knowledge Product to support patient care decisions and treatment, patient health information, and medical center staff education.
1.2.2 Make the Point of Care Clinical Knowledge Product available through the Computerized Patient Record System (CPRS), the VA’s Electronic Health Record, to insure consistent access at the bedside for all Harry S. Truman Memorial Veterans’ Hospital health professionals and staff treating patients.
1.2.3 Provides ACCME-accredited AMA PRA Category Continuous Medical Education (CME) points per topic reviewed for search(es) performed.
2.1 Scope of Requirement:

Authentication will be through IP address validation eliminating the need for users to enter identification, username and/or password.
2.1.2 An unrestricted number of simultaneous users on the Harry S. Truman Memorial Veterans’ Hospital network will have immediate access from their desktop Harry S. Truman Memorial Veterans’ Hospital networked computer.
2.2 Information Scope The product should include:
2.2.1 Evidence based information and topics covering;
· Internal medicine, its subspecialties,
· Primary care,
· Surgery
· Mental health
All in a format suitable for use by attending level physicians, students, nurse practitioners, house staff and other point of care health providers.
2.2.2 Topics that reflect current practice issues and support clinical decision support. The authors should be leaders in the field of medical research and information and represent the most current acceptable medical practices.
2.2.3 Drug information, readily searchable/available and directly related to the medication. Capability of printing the drug information for patient education is important.
2.2.4 Comprehensive information in a single product available for stand-alone purchase or license, independent of complementary or companion products offered by the vendor.
2.2.5 Related patient education material. Product preference will be given to those which can be printed and provided to patients, in laymen terms. Continuation of care instructions should be of the same caliber.
2.3 Information Quality
2.3.1 Information is accurate, authoritative, relevant, and current.
· Authors and sources are well recognized subject experts in their fields.
· Expert – has published journal articles or textbook/chapters on the topic
2.3.2 Topics are presented succinctly, but with adequate detail (clinical topical articles, and associated graphs, tables, images, diagrams, etc.) to be useful to clinicians. Data should be searchable by topic/disease and not require the provider to spend minutes searching or evaluating data. Preference should be given to the product is most comprehensive (graphs, tables, images (such as radiologic and pathology) among others) to be the most useful.
2.3.3 “Best practice” or evidence-based information is included in recommendations for diagnosis or therapy.
2.3.4 Policies for determining validity and standards for inclusion of material are well described.
2.3.5 Guidelines and summaries include references or sources.
2.4 Usability
2.4.1 Navigation within the system is logical and is easy to use.
2.4.2 There is fast access to topics and subtopics. Topic linking or indexing retrieves material of high relevance quickly.
2.4.3 The product has “user friendly” help and direction aids.
2.5 Perpetual updates, as the standard of care evolves.
2.5.1 Content areas are updated on a continuous, incremental cycle rather than on a single anniversary date for the entire product.
2.5.2 The date a topic was last reviewed for content is stated and easily found.
2.6 Access, content, support
2.6.1 Access to be granted through IP address validation on an enterprise basis, eliminating the need for users to enter identification, username and/or password. It is required that all authorized remote users (with VPN, dial-up, wireless, etc. access) have access to the product through any VA networked computer and/or CPRS.
2.6.2 The product is compatible with most standard web browsers, which have cookies enabled. To insure privacy, the product uses session cookies rather than persistent cookies.

If product is not 508 compliant and vendor must describe what steps are being done to make it 508 compliant.
Single point of contact for customer service issues is assigned to VA.
Questions are responded to on the same business day.
