About this file

This document is an Amendment to a Solicitation and Questions and Answers (Q&A) for a federal contract opportunity, along with an updated Statement of Work (SOW).

Solicitation Overview: The Department of Veterans Affairs, Great Lakes Acquisition Center, issued Amendment 0001 to Solicitation 36C25226Q0636 on August 19, 2026, for On-site Survey Readiness Services (Physical Environment/Life Safety) at eight VA medical facilities across Illinois, Michigan, and Wisconsin. The contract requires a single qualified surveyor to conduct Joint Commission mock surveys and provide readiness consultation services. The period of performance is 180 days from contract award, with surveys to be completed prior to October 1, 2027. Survey scope includes 21 total days across the eight facilities (ranging from 2-4 days per facility), document reviews, building tours, educational sessions, and written reporting. Deliverables include facility-specific reports within 15 working days of each survey and an aggregated VISN-wide summary report prior to a summation conference. All travel expenses are the contractor's responsibility under an all-inclusive pricing model with no government reimbursement.

Qualification Requirements and Q&A Clarifications: The surveyor must have a minimum of five years' experience conducting mock Joint Commission surveys covering Physical Environment, Life Safety, and Emergency Management standards, with expertise in healthcare safety demonstrated through relevant certifications, training, and subject matter expert recognition. The Q&A section clarifies that subcontractor experience does not satisfy the government's three-year consecutive experience requirement for the prime contractor, but the individual surveyor's experience must be held by the same person proposed for the contract. Mock Joint Commission survey experience performed within the VA in an official capacity counts toward experience requirements. Qualified healthcare certifications are not specifically enumerated; rather, qualification is demonstrated through expertise in healthcare safety, documented experience conducting physical environment surveys, and proficiency in Joint Commission standards encompassing safety, security, hazardous materials, fire/life safety, equipment, and utilities management.

RFQ Q&A

Solicitation 36C25226Q0636

On-site Survey Readiness Services (Physical Environment / Life Safety)

1. Please clarify whether the Government will consider the relevant experience and prior contracts of a proposed subcontractor or similarly situated certified SDVOSB/VOSB teaming partner toward the offeror’s minimum three-year experience and three-contract requirements, provided the offeror remains the prime contractor and complies with the applicable limitations on subcontracting.

A: Subcontractor experience will not satisfy the Governments’ requirement of demonstrating at a minimum, three (3) consecutive years’ experience.

2. If team-member experience may be considered, please also confirm whether the experience must be held by the same individual proposed as the surveyor, by the performing firm, or whether either may satisfy the requirement when adequately documented.

A: To satisfy the minimum five (5) years’ experience for the Surveyor under Section 6 of the Statement of Work, the experience must be held by the same individual.

3. Will the Government consider the documented Joint Commission Physical Environment and Life Safety experience of the individual who will personally perform the surveys toward the three-year experience requirement, where that experience was earned as a VHA executive rather than under contracts held by this firm? If the requirement is strictly a firm-level pass or fail gate, we would appreciate that confirmation so offerors can respond accordingly.

A: How the Offeror demonstrates the requirement of a minimum, three (3) consecutive years’ experience is ultimately up to the Offeror. Please keep in mind of the Government’s requirement that “Offeror shall provide at minimum three (3) examples of previous contracts that collectively appear to be of similar size and scope.” Government contract examples are preferred, but an Offeror will not be penalized for contracts submitted outside of the Federal Government. Subcontractor experience will not satisfy the Governments’ requirement of demonstrating at a minimum, three (3) consecutive years’ experience.

See 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (NOV 2021) (DEVIATION OCT 2025) for how Offerors will be evaluated.

4. Does mock Joint Commission survey experience performed within VHA, in an official capacity across a VISN, count toward the five-year requirement in paragraph 6? Or does the Government require that the experience have been performed under contract?

A: As long as the Governments’ requirements in Section 6 of the Statement of Work are met and the Offeror is able to demonstrate at a minimum, three (3) consecutive years’ experience, this will satisfy the Government’s experience requirements. Please keep in mind of the Government’s requirement that “Offeror shall provide at minimum three (3) examples of previous contracts that collectively appear to be of similar size and scope.”

Government contract examples are preferred, but an Offeror will not be penalized for contracts submitted outside of the Federal Government.

5. Would the Government identify which healthcare certifications it considers qualifying, so that offerors can address paragraph 6 precisely?

A: Qualified individuals shall have expertise in Healthcare Safety and Health, demonstrated experience in conducting Healthcare Physical Environment surveys, and proficiency in evaluating the Joint Commission Physical Environment (Environment of Care) standards. These standards encompass safety, security, hazardous materials and waste, fire and life safety, equipment, and utilities management. Knowledge is evident through relevant training and education, as well as recognition as a subject matter expert. This includes designing programs, implementing regulatory standards and codes within a healthcare setting, and advising on improvements in engineering controls and program advancement.

3. DEFINITIONS AND TERMS: As used throughout this Statement of Work, the following terms shall have the meaning set forth below:

a. Physical Environment (PE) The objective of this chapter is to foster a safe, functional, and supportive environment within the hospital to ensure the preservation of quality and safety. The environment of care consists of three fundamental elements:

- The building or space, including how it is arranged and special features that protect patients, visitors, and staff.

- Equipment used to support patient care or to safely operate the building or space.

- People, including those who work within the hospital, patients, and anyone else who enters the environment, all of whom have a role in minimizing risks.

b. Fire Safety Management

Activities selected and implemented by the organization to assess and control the risks of fire, smoke, and other byproducts of combustion that could occur during the organization’s provision of care, treatment, or services.

c. Fire-Rated

Material that has undergone a test and is fire protection rated, or fire resistance rated.

Two examples of the concept of fire-rated include the following:

- fire resistance rating - The time, in minutes or hours, that materials or assemblies have withstood fire exposure, as determined by tests, or methods based on tests, prescribed by the National Fire Protection Association (NFPA).

- fire protection rating - A designation indicating the duration of fire test exposure to which a fire door assembly or fire window assembly was exposed and for which it met all the acceptance criteria, as determined in accordance with NFPA 252, Standard Methods of Fire Tests of Door Assemblies, or NFPA 257, Standard on Fire Test for Window and Glass Block Assemblies.

d. Fire Watch

The assignment of a person or persons to an area for the express purpose of protecting occupants from fire or similar emergencies. Examples of this protection include:

- Notifying the fire department, the building occupants, or both of an emergency.

- Preventing a fire from occurring.

- Extinguishing small fires.

e. Hazardous Materials and Waste

Materials whose handling, use, and storage are guided or defined by local, state, or federal regulations, such as the Occupational Safety and Health Administration’s Regulations for Bloodborne Pathogens regarding the disposal of blood and blood-soaked items and the Nuclear Regulatory Commission’s regulations for the handling and disposal of radioactive waste. This also includes hazardous vapors (for example, glutaraldehyde, ethylene oxide, nitrous oxide) and hazardous energy sources (for example, ionizing or nonionizing radiation, lasers, microwave, ultrasound). Although the JC considers infectious waste to fall within this category of materials, federal regulations do not define infectious or medical waste as hazardous waste.

f. Life Safety Code®

A set of standards for the construction and operation of buildings intended to provide a reasonable degree of safety during fires. These standards are prepared, published, and periodically revised by the National Fire Protection Association (NFPA) and adopted by JC to evaluate health care organizations under its life safety management program. The VHA adopts and uses the current edition of each NFPA code.

g. Utility Systems

Building systems that provide support to the physical environment, including electrical distribution and emergency power; vertical and horizontal transport; heating, ventilating, and air conditioning (HVAC); plumbing, boiler, and steam; piped gases; vacuum systems; and communication systems, including data exchange systems.
