This is Amendment 0001 to RFQ 36C25225Q0099 for hazardous waste collection and removal services at the William S. Middleton VA Hospital in Madison, WI and its affiliated CBOCs. The amendment provides answers to vendor questions and removes line item 28 (55-Gallon Drums) from the Price Schedule and Performance Work Statement, resulting in 28 line items per year instead of 29.

Key details from the Q&A include: the incumbent is Earth Smart Environmental Solutions, LLC (contract 36C25220D0025); pharmaceutical waste will be true waste only; partial bids are not accepted; all fees must be factored into per-pound rates; cylindrical drums are contractor-provided and swapped 1-for-1; rectangular containers are VA-provided; large lab pack jobs occur 1-2 times annually taking 60-90 minutes; battery sorting is done by VA personnel; and CBOC service calls are billed as a flat rate plus applicable line item costs. The solicitation due date remains unchanged. The facility's EPA Registry ID is 110001921467. Bidders must acknowledge and return the amendment with their quotes.

RFQ 36C25225Q0099 – Vendor Questions with Answers

1. We would like to know if this is a brand-new contract OR if there is (was) an incumbent performing these services. If not brand new, could you please provide the current / previous contract number?

The previous/current contract number is: 36C25220D0025. The incumbent is EARTH SMART ENVIRONMENTAL SOLUTIONS, LLC.

2. Will the included pharmaceutical waste be true waste, or non-returnable medications per the current reverse distribution self-service program?

The included pharmaceutical waste will be true waste.

3. Would reverse distribution be considered with this waste contract as an addendum?

No.

4. Will a partial bid on select service be accepted?

No.

5. Can we include a line item for an environmental impact fee on invoices or should this be factored into the per lb rate that we give them?

No. All fees should be factored in to the per lb. rate.

6. Can they give us an idea on the quantity and size of containers they ship per month/per year for each waste type?

See “Estimated quantities and frequency chart” on pages 50-52 of the solicitation.

7. What containers do they need us to supply? It was unclear whether we’re providing black RCRA bins, 5gal pails, etc. for things like lab waste and universal waste.

Cylindrical drums are swapped on a 1-for-1 basis and provided by the contractor. Rectangular containers are purchased from a separate program and provided by the VA. Pharmaceutical waste is emptied into contractor owned containers. Some government owned containers are sealed and transported as is and pricing is determined by the line item it applies to. All other containers, in accordance with the Performance Work Statement (PWS), and Price Schedule – listed in the Solicitation - shall be provided by the contractor.

8. What is the expected amount of labor needed to complete lengthy onsite tasks such as large lab pack jobs and sorting/taping batteries. I didn’t see a line item for labor anywhere in the RFQ.

Labor costs should be built into each line item. For context, large lab pack jobs are required, on average, 1 - 2 times per year. Some years may require 0 – 1 large lab pack jobs per year. When the large lab pack jobs are required, the estimated time for completion is ~ 1 hour – 90 minutes; these estimates are based on previous/recent services. Additionally, the sorting/taping of batteries is completed by VA personnel.

9. What is the poundage and types of waste generated at the other 6 satellite clinics? Is this included in the RFQ? There is a line item for a service call at a satellite clinic. What needs to be included in the service call charge? (i.e. trans, disposal rate, supplies, etc.)

A flat rate for the service calls only. Page 52 of the solicitation: Service calls to CBOCs may be as few as zero a year. Facility reserves the right to transport waste from CBOCs to Facility if desired. Any/all service calls shall invoice in accordance with the price schedule pricing (Line items 1-28). For example, if a CBOC requires a pickup for tires – the invoice shall include 1 service call flat rate fee and the poundage cost for the tires – in accordance with the Price Schedule pricing (Line items 1-28).

10. Can you please provide your EPA ID number?

The EPA Registry ID for the William S. Middleton Memorial VA Hospital in Madison, Wisconsin is 110001921467.
