AMENDMENT 003 hereby incorporates the following clarifications/change(s) into the solicitation package:

1. Vendor Questions and Answers

a. 12/6/2023 - We are working on the response to this solicitation and have a few questions about the final submittal process:

1.) QUESTION: Will the government confirm that this is an in-person delivery for a sealed bid at the date/time specified on the 1442?

VA REPLY: The bids do NOT need to be hand-delivered. As long as an offer is delivered in a sealed envelope, and received by the government NLT Friday, 12/29/2023 by 1045 CST it will be accepted.

2.) QUESTION: Also, will the government confirm the address and room that the sealed bid must be delivered to at the date/time specified on the 1442?

VA REPLY: Bids must be delivered to 115 S. 84th Street, Suite 101, Milwaukee, WI 53213, NLT Friday, 12/29/2023 at 1045 CST.

b. 12/08/2023:

1.) QUESTION: Please confirm that integration to the existing Stanley patient elopement system is not part of this bid and will be evaluated at a later date?

VA REPLY: This project does not include integration with the patient elopement system.

2.) QUESTION: Please confirm Integration with the existing Midmark system is only applicable in the 1st floor CLC and 4th floor East areas?

VA REPLY: Midmark system is only on 1st floor CLC and 4-East inpatient wing.

3.) QUESTION: Will configuration, software, and troubleshooting of the Midmark system be provided by the VA or their supplied partner?

VA REPLY: The nurse call system must have the capacity to directly integrate with the existing Midmark system. Integration service would be acquired by the VA.

4.) QUESTION: Will configuration, software, and troubleshooting of the Vocera system be provided by the VA or their supplied partner?

VA REPLY: The nurse call system must have the capacity to directly integrate with the existing Vocera system. Integration service would be acquired by the VA.

5.) QUESTION: Will VA personnel or their current provider be on-site to assist with existing nurse call system removal? Devices and cabling can be removed following general industry best practices but any decommissioning through software configuration or system device removal practices will need to be provided by that system’s representative.

VA REPLY: Contractor is responsible for demolition of existing physical devices including servers. VA personnel will be available to deprogram the existing system. Contractor shall coordinate demolition efforts with VA staff.

6.) QUESTION: If replacement devices for the existing TV and lighting controls in the patient rooms are required due to failed or damaged currently installed equipment, will it be acceptable to provide pricing at a future date?

VA REPLY: Replacing all bed connect module and patient station with pillow switch are included as part of this project. Refer to plan set published with Solicitation 36C25224B0003 on www.SAM.gov.

7.) QUESTION: Will manufacture specifications of system capabilities for battery backup be acceptable in lieu of provided a 3rd party UPS that is outside of system design?

VA REPLY: See revised Spec Section 27 52 23 2.1.I - The VA specifies that servers must have dual power supplies which are connected to facility UPS and commercial power. Contractor does not need to provide 3rd party UPS. If this does not answer the question, please resubmit the question with more detail.

8.) QUESTION: Proposed system requires a test environment alongside production. Can the nurse education rooms 3246, 3249, and 3250 be removed from the production infrastructure and used for the test system? Additional devices would be installed in these rooms to mirror the rest of the facility. This will allow for a better training environment as well.

VA REPLY: Yes.

9.) QUESTION: Third floor operatory rooms 3229, 3230, 3231, 3232, 3233, and 3234 do not have corridor light icons on the drawings. Should corridor lights be installed for each of these rooms?

VA REPLY: Include wall-mounted lights for each of these rooms.

10.) QUESTION: No corridor light icon in the hallway outside isolation room 4205. Please advise if one should be added or if the light in anti-room 4207 can be moved to the hallway.

VA REPLY: Move the nurse call light from the Anti-Room 4207 outside the Anti-room door in corridor 4212C.

11.) QUESTION: Please identify a new shared headend location for the devices to be installed on 5th and 6th floors. Waiting room 2188 shown on the drawings calls for a corridor light to be installed in the room with no other devices. Please verify.

VA REPLY: 5th and 6th floor nurse call devices shall be connected to the NCCP in room 4212.

12.) QUESTION: Waiting room 2188 shown on the drawings calls for a corridor light to be installed in the room with no other devices. Please verify.

VA REPLY: Delete the device shown in 2188.

13.) QUESTION: To provide proper disaster recovery options to the VA, multiple application servers will be required for system operation. This system design will require the use of a load balancer for proper traffic distribution. Will the VA provide a network load balancer and if so, please provide system information.

VA REPLY: See revised Spec Section 27 52 23, Part 2.0 virtual platform hosting; contractor shall provide a system that meets these requirements. VA will not purchase anything additional; contractor must provide what is needed for their new system to tie into the existing system.

14.) QUESTION: During the walkthrough, the headend location in room 1214 was identified to be unusable. Please confirm room 1218A is the new headend location.

VA REPLY: Room 1214 calls out for NCCP "Nurse Call Control Panel." Due to a current construction project, this NCCP shall be in room 1216.

15.) QUESTION: For patient rooms with bed connections on the headwall, multiple heights for currently installed locations were identified during walkthrough. Please confirm height of new bed connections for rough-in.

VA REPLY: Install each at the existing height.

c. 12/11/2023: QUESTION: Will the government accept FedEx delivery of hard copied proposals and guarantee bonds before bid due date of 12/29/2023 at 10:45am CST?

VA REPLY: Yes.

d. 12/11/2023 QUESTION: During the pre-bid walk through it was noted that new head walls were going in all over the hospital. It appears a project is going to install all new head walls in the hospital. The new head walls included only (2) single openings for the nurse call system. The new nurse call system will require (1) triple gang opening and (2) single gang openings. Who will be responsible for cutting in these additional openings in the millwork and installing the boxes?

VA REPLY: Contract requirements are based on existing conditions. If existing conditions require the contractor to modify existing headboards, the contractor is responsible for those costs.

VA REPLY: Reference Amendment 4 revised specification 27 52 23, Part 4.3.

f. 12/18/2023 QUESTIONS 1.) BID Specs Section 4.2 , Page 276, WARRANTY- The Bid docs discuss one year warranty for workmanship and defects, but then states a 2-year guarantee period in section 1.9 . Please confirm what is expected to be provided by the vendor ( a one-year warranty or two-year guaranty) VA REPLY: Per Spec 27 52 23, Part 4.2.B.1 (Amendment 4) - The Contractor warranty requires that all provided material and equipment will be free from defects, workmanship and will remain so for a period of one year from date of final acceptance of the System by the VA.

2.) BID Specs - Section 1.9 – Page 234 Warranties/Guaranty “The Contractor shall warrant the installation to be free from defect in material and workmanship for a period of two (2) years from the date of acceptance of the project by the Owner. The Contractor shall agree to remedy covered defects within four (4) hours of notification of major failures or within twenty-four (24) hours of notification for individual station related problems.”

VA REPLY: Per Spec 27 52 23, Part 4.2.B.1 (Amendment 4) - The Contractor warranty requires that all provided material and equipment will be free from defects, workmanship and will remain so for a period of one year from date of final acceptance of the System by the VA.

3.) BID Spes Section 1.9 - For system correction times - clarification needed - Is it four hours from when the VA reaches out to the contractor that the defect issue is resolved? Also confirm that this is only during standard business hours. example. say VA reaches out at 4 pm for an issue, standard working day are until 5:00 does this allow for contractor to provide solution/remedy by next morning for 11 am?

VA REPLY: Refer to Spec Section 27 52 23, Part 4.2 B.4.a.3 (Amendment 4) for emergency calls, 4 hours from when VA contacts the contractor no matter when. The nurse call system shall be treated as any other life safety system in this regard.

4.) Require clarification on SLA for issues that occur by staff or patients? say patient physically breaks a device. do they also fall under the same one-day response time frame?

VA REPLY: This contract includes Dept of Labor (DOL) Construction Wage Rate requirements and are applied to warranty actions. DOL Service Wage Rates do not apply to this contract. Any problem caused by improper use, accidents, other vendor ,contractor, or owner tampering or negligence, for which the Contractor is not directly responsible and does not control, is not part of the warranty.

5.) BID Specs Section 4.2 – Page 278 – Required Onsite visits – Two year

a. Define equipment cleaning. Is these just head end gear, or including devices in patient/staff rooms?

b. Define exact requirements for system maintenance.

c. Is the 8 hours once every 12 weeks service inspection required during standard business hours?

VA REPLY: Per Amendment 4, the revised Spec Section 27 52 23 has deleted the original Part 4.2.B.4.b.

6.) Bid specs , page 279 - Define service contract options requirements as the contract requires a 2-year period with defined SLA. Does the vendor have to meet both the service contract requirements and the warranty provisions.

VA REPLY: The verbiage in Spec section 27 52 23 4.2 C that states "Provide Service Contract options to the VA. Provide “Fast” response time, 2 hours after notification of issue, provide call back. For “Critical” issues have someone on site by 24 hours after call back. Two business days for “Urgent” issues" has been deleted in the revised spec included in Amendment 4.

There is no service agreement under this contract, however, there is a 1-year warranty period (27 52 23 4.2B1).

7.) If contractor arrives onsite and notices that system issue is caused by improper use, accidents, other vendor ,contractor, or owner tampering or negligence, for which the Contractor is not directly responsible and does not control. - Please confirm this will be billable back to VA for time/ labor.

VA REPLY: Assuming this question refers to a warranty call, then the contractor will not be called to the site until VA has confirmed that the issue is covered under the warranty and is the responsibility of the contractor. Contractor can submit a claim to VA for consideration if they do not agree it is a valid warranty issue.

8.) Bid specs Page 248 – VOCERA Paging Equipment - Paging Integration- What is really expected from the nurse call vendor to integrate all pagers. Nurse call vendor shall demonstrate integration capability outlined in page 248 only. VA OIT team will provide required support to modify/reprogram the individual pagers. The pagers are not covered under the 2 year warranty. Correct?

VA REPLY: Contractor is responsible for ensuring that their nurse call solution can fully integrate with Vocera. Any warranty covered under this system is for one year. If this doesn't answer the question, please resubmit a more concise and detailed question.

NOTE: All references in this amendment to Specification Section 27 52 23 are related to the revised version attached to this amendment. The version included in the original solicitation, or a previous amendment, is hereby superseded.

THE DEADLINE FOR SUBMITTING QUESTIONS WAS DECEMBER 19, 2023. NO FURTHER QUESTIONS WILL BE ACCEPTED.

The bid due date has changed from Friday, 01/05/2024 to THURSDAY, 01/11/2024 at 10:45 a.m. CST. Hard copies of all bids and guarantee bonds must be submitted prior, or at the actual bid opening.
