a. All blood must be collected by a closed system under aseptic conditions, processed in appropriate solutions and the container appropriately labeled. The label must also bear the expiration date of the contents, which must not exceed the Food and Drug Administration (FDA) allowable limit for the type of anticoagulant in which the blood component is drawn.

b. All blood supplied must be free of hemolysis, clots, and excessive lipemia.

3. Autologous blood/blood components must be typed for ABO and Rho(D) in accordance with licensed methodologies and must be tested for all transfusion transmitted disease markers currently required by the FDA.

4. Supplier must provide platelets that are processed in such a way that they adhere to the FDA Guidance “Bacterial Risk Control Strategies for Blood Collection Establishments and Transfusion Services to Enhance the Safety and Availability of Platelets for Transfusion,” December 2020.

a. The transfusion services will not implement secondary rapid testing in order to comply with this FDA Guidance.

5. The Contractor shall supply 100% “volunteer donor” blood as defined in 21CFR606.121(c)(5)(ii). All blood and blood components must be collected from donors in accordance with the requirements of the AABB and the FDA.

6. In the event of a recall, Contractor shall promptly send copies of the recall request to the Blood Bank Medical Director and Blood Bank Supervisor at the VA medical center where the blood product was issued.

7. Contractor shall make Immunohematology Reference Lab (IRL) testing available to all VA medical centers. The offeror must be capable of providing blood components and the specialized IRL testing detailed in Attachment A on a 24-hour per day, seven days per week basis.

B. Changes in Testing Requirements for Transfusion Transmitted Diseases

1. If additional testing for transfusion transmitted diseases is nationally mandated by the FDA during the contract period, specific price adjustments may be requested by the supplier to address the cost for the additional testing. If this should occur, a letter detailing the rationale for the price increase must be submitted to the Contracting Officer at least 30 days before the intended implementation date.

C. Donor Requirements

1. The contractor shall maintain readily available blood donor lists. Such lists must indicate whether, and on what date, blood of a specific donor was furnished to the VA under this contract.

2. Offeror’s donor identification numbers must be unique to the donor unit and must have an FDA-approved numbering schematic.

3. Donor selection must be in accordance with criteria established by the FDA and/or the AABB.

D. Orders/Deliveries

1. Orders will be categorized into three types: Routine, Timed, and STAT. These categories are defined as follows:

a. Routine – Shipments of blood products to maintain minimum inventory levels or blood products ordered to fulfill a standing request.

b. Timed/ASAP – Shipments of blood products requested on a date at a specific time. Timed hours must be requested 4 hours before the required delivery and must be delivered within 1 hour of the time requested.

c. STAT – Shipments shall be received no greater than 90 minutes from the time of request but ideally within 60 minutes.

1 deliveries/day; seven days/ week

b. It is estimated that Timed delivery services will occur at the following rate:

VISN 12 VA Facility
Timed/ASAP Delivery Rate
537 - Jesse Brown VA Medical Center (JB)
50 deliveries/year
556 - Lovell FHCC
20 deliveries/year
578 - Hines (HI)
75 deliveries/year

c. It is estimated that STAT delivery services will occur at the following rate:

VISN 12 VA Facility

75 deliveries/year
556 - Lovell FHCC

3. The specific volumes distributed in a routine order will be established by each VA medical center. However, the contractor shall submit a proposed delivery and route schedule for approval within ten days after contract award. Schedule may be adjusted periodically.

4. Once units are received by a VA medical center, those units become the property of the VA medical center, and payment will be made appropriately, based on contract terms and proper invoicing. They are not to be treated as though they are on consignment. However, for inventory control purposes, efforts will be made by each VA medical center to assist the contractor and other facilities in meeting patient transfusion needs.

5. The VA will be subject to and comply with all requirements and regulations associated with the recall and withdrawal of blood or blood components from its inventory due to health and safety risks.

6. Efforts will be made to assist the supplier in hosting blood donor drives at the VA medical center(s) in those cases where it can be reasonably accommodated by the hosting VA site, and where there are no previous commitments to other blood centers, including the Department of Defense.

E. Shelf Life

1. All blood and blood components must be labeled, stored, and shipped in accordance with the current regulations of both the FDA and the AABB. If these regulations change during the term of the contract, the contractor shall implement the necessary changes as directed/recommended by the regulatory and accrediting agencies.

2. For routine orders, allogeneic blood and blood components must be supplied with appropriate remaining shelf life, regardless of anticoagulant, as detailed below.

Blood and blood components
Appropriate Remaining Shelf Life
Leukoreduced Red Blood Cells
More than 21 days
Fresh Frozen Plasma
More than 6 months
Plateletpheresis
More than 48 hours, if not collected for a specific patient
Cryoprecipitate
More than 6 months

3. For Timed/ASAP and STAT orders, the contractor shall provide units with the appropriate remaining shelf life whenever possible; however, the VA understands that the urgency of the situation and the availability of the blood product may preclude this possibility. If the appropriate remaining shelf life cannot be applied, every effort to provide credit should be made by the contractor when applicable.

4. When the VA returns units prior to expiration in accordance with the contractors return policy, the contractor shall credit the VA for units which are returned. The contractor will provide a copy of their return policy to be included in the contract for reference.

5. When the VA returns units prior to expiration in accordance with the contractors return policy, it is expected that there will be no restocking fee applied. An exception will be made for the return of apheresis platelets which will not have a restocking fee applied unless more than 40% of the number of apheresis platelets ordered in a month are returned.

F. Immunohematology Reference Lab

1. The contractor will offer a wide array of advanced Immunohematology reference lab testing to the facilities. See Attachment A for a list of tests that should be offered.

2. Tests should be able to be ordered 24 hours a day, 7 days per week with varying levels of urgency (i.e. routine, STAT, etc.)

3. The contractor will provide notice to the facility before proceeding with any additional testing on specimens sent to the IRL to confirm the need for the additional tests which were not originally ordered. The facility reserves the right to request that the additional testing not be performed/charged.

4. The contractor will clearly designate what days/times are considered “after hours” for IRL testing orders and if an additional charge for orders placed during this time will be applied.

5. The contractor will clearly designate if there will an additional charge for IRL testing ordered that are considered STAT in nature.

G. Value Added Services

1. Contractor shall provide an action plan for instances when blood products may be required within less than the STAT turnaround time i.e. during times of emergent patient care, Massive Transfusion Protocols, Mass Casualty, etc.

2. When HGB S negative units are requested the contractor will perform testing for HGB S and clearly mark the unit as HGB S negative.

3. Contractor shall provide physician consultative services that are consistent with the services offered to other non-VA contracted customers. A description of the services must be included with the offeror’s proposal.

4. Contractor shall provide educational programs that are consistent with the programs offered to other non-VA contracted customers. A description of the educational programs must be included with the offeror’s proposal.

5. If available, the Contractor will provide biospecimen material to the Government for teaching, competency assessment, and validation at no additional cost without the need for an additional contract or agreement.

The Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. The Government may require repair or replacement of nonconforming supplies or reperformance of nonconforming services at no increase in contract price. (B) The date of the first written demand for payment, including any demand for payment resulting from a default termination.

(vi) The interest charge shall be computed for the actual number of calendar days involved beginning on the due date and ending on—
