SOLICITATION 36C25220Q0420

Vendor Questions & Answers Q1: Is the VA able to provide the total number of containers by size for the facility ex. 2gl, 4gl, 8gl and 17gl?

A1: Due to the size of the campus, we are unable to provide an accurate total number of containers.

Pricing should be based upon per pounds removal, per the Schedule at B.2.

Q2: Is it the intention of the VA to put reusable sharps containers at the Community-Based Outpatient Clinics (CBOCs)?

A2: Some CBOC have disposal and some have non- disposal sharps containers and all 6 CBOCs have step on boxes in their blood labs.

Q3: Page 10 #8 it states the following:

0. The contractor is required to have, at a minimum, one (1) employee to change out reusable Sharps containers at least once a week from Hines VA Hospital campus soiled mat cage. May require up to one additional day of service (to be determined by the COR).

However, there are 2080 hours per year on the solicitation which equates to 40 hours per week. Does the VA want the sharps technician there Monday thru Friday 40 hours per week?

Also, on page 7 it says you do want a full-time employee. These two are contradicting of each other so please confirm whether or not you want the full-time employee.

A3: The contractor is required to have one full time employee for 8hrs a day Mon - Fri 6am- 2:3pm

Q4: “Contractor shall process the medical waste with microwave-grinder treatment equipment. If another method of medical waste destruction/disposal is contemplated, it must be meet all Federal EAP, state and local requirements.” Is autoclave an acceptable method of destruction?

A4: No, not at the Hines campus.

Q5: Untreated medical waste cannot be disposed of into any landfill. Untreated Potentially Infectious Medical Waste (PIMW) is banned from all landfills in Illinois. Once PIMW has been properly treated to eliminate its infectious potential, it is no longer PIMW (except in the case of sharps) and may be disposed of into any landfill permitted by the Illinois EPA to accept municipal waste. For sharps, both the infectious nature must be eliminated and the sharps must be rendered unrecognizable.

Can this be done by the method of autoclaving?
A5:Provided it meets all federal, state and local requirements, yes.

If treating and disposing outside of the state of Illinois, can this be done by autoclaving as long as it complies with the state law in our area?

A5: Provided it meets all federal, state and local requirements where it is done, yes.

Q6: “Contractor’s transport vehicle must be appropriately labeled with universal biohazard symbol, refrigerated or otherwise treated in order to prevent noxious fumes from entering the buildings, and locked when the driver is not in attendance.” What kind of alternatives would be acceptable?

A6: The trailer may be left by a loading dock for a week, as it gets filled daily by the contractor employee. Wednesday night the full trailer is picked up and Thursday morning a new trailer with clean nettle boxes is dropped off.

Q7:Who is the current provider of sharps containers?
A7:Stericycle

Q8: What is the current length of the trailer being utilized? Is it refrigerated? Is it permanently stationed at a dock?

A8: 53 feet, not refrigerated, yes, permanently stationed

Q9: What are the sizes and quantities to be installed of the Pathological and Chemotherapy Waste containers needed for removal and disposal? If any are 8, 17, or 40 gallon sizes, do they require a standard dolly or foot pedal dolly?

A9: Due to the size of the campus, we are unable to provide an accurate total number of containers.

Pricing should be based upon per pounds removal, per the Schedule at B.2. Some require a standard dolly and others require a foot pedal dolly.

Q10: What are the required total quantities to be installed of 2-gallon reusable sharps containers needed for Hines VA Campus and 6 associated CBOCs?

A10: Approximately 60 at Hines, approximately 10-20 per CBOC per month

Q11: What are the required total quantities to be installed of 3-gallon reusable sharps containers needed for Hines VA Campus and 6 associated CBOCs?

A11: We do not use 3-gallon

Q12:What are the required total quantities to be installed of 4-gallon reusable sharps containers needed for Hines VA Campus and 6 associated CBOCs?
A12:Approximately 140-150 per week at Hines and approximately 10-20 per CBOC per week

Q13: What are the required total quantities to be installed of 8-gallon reusable sharps containers and floor dollies needed for the Hines VA Campus and 6 associated CBOCs?

A13: Approximately 30-60 per week at Hines and none at the CBOC

Q14: What are the required total quantities to be installed of 17-gallon reusable sharps containers and floor dollies needed for the Hines VA Campus and 6 associated CBOCs?

A14: Approximately 12-24 per week with dollies at Hines and 10 per week at each CBOC

Q15: Is the Hines VA Hospital Campus soiled mat cage on the same floor level as the loading dock where the biohazardous trailer is parked? What is the distance to transport from soiled mat cage to biohazardous trailer?

A15: No, it is approximately three buildings away, exact distance is not known

Q16: How many 40-gallon reusable RMW totes are required to be onsite? What is the container quantities disposed of each month for the past 12 months?

A16: Due to the size of the campus, we are unable to provide an accurate total number of containers.

Pricing should be based upon per pounds removal, per the Schedule at B.2. The estimated quantities for the Schedule at B.2 are based upon historical disposal.

Q17: How many 40-gallon reusable chemo totes are required to be onsite and what is the container quantities disposed of each month for the past 12 months?

A17: Chemo containers are provided by our Logistics and Pharmacy department. Due to the size of the campus, we are unable to provide an accurate total number of containers. Pricing should be based upon per pounds removal, per the Schedule at B.2. The estimated quantities for the Schedule at B.2 are based upon historical disposal.

**Please note that the B.2 Price/Cost Schedule has been amended to include a line item for Category A waste, such as Ebola. Please review the change to B.2 Schedule, which supersedes the B.2 Price/Cost Schedule in the original Solicitation Document.
