DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C252-19-AP-3795 (Req. # 550-19-4-985-0825)

1. Contracting Activity: Department of Veterans Affairs, VISN 12, VA Illiana Healthcare System, Danville, IL.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Selectra Lemenius Trio Laser.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

Due to space constraints in the customer clinic, VA Illiana Health Care System Danville, IL requires a trio laser that takes up a very small footprint and is truly wheelchair accessible. This unit is compact yet offers the three different laser options. Lumenis is the only manufacturer in the world of a “triple laser” that the customer requires.

Salient Characteristics Include:

Homogeneous Laser Beam Advanced optics and laser delivery technology features uniform energy distribution across the entire laser spot area for more precise and safer photocoagulation treatment. The result is a homogenous laser spot with evenly distributed power across the full area of the spot, eliminating the potential risk for the formation of “hotspots”.

Dual Fiber Port – Instantly switch between delivery devices Changing delivery devices is fast and easy. The Novus Spectra automatically senses the delivery devices connected to it. Laser parameters are adjusted internally to compensate for the chosen delivery device to maintain selected power density on the target tissue.

ClearView™ Filters – Optimized view of treatment site The Lumenis ClearView™ filters’ photopic balanced coating eliminates color distortion of the treatment site while optimizing white light transmission or “brightness optimizing light transmission. The Selecta Duet and or Trio have a built in Physician Eye Safety Filter, that protect the user from both the YAG and Photocoagulation laser wavelengths.

AcuGuide™ Micromanipulator Accurate and safe laser beam guidance for fine-tuned laser application on target tissue.

Integrated LaserLink Cradle Safely and conveniently stow the laser adapter when not performing photocoagulation.

Storage Space The Selecta table is equipped with built-in compact storage space for lenses or other accessories.

Ergonomic Joystick with EZGrip™ Coating The ergonomically designed joystick includes a convenient integrated laser fire-switch.

Advanced Adjustable Offset The Selecta features 3 posterior defocus positions with an adjustable focal point of up to 350 posterior to the aiming beam.

Magnification Changer The Selecta full Slit Lamp function includes a 6-step magnification changer

Electronic Height Adjustment The Selecta table height can be easily adjusted using its electronic up/down switch

4. Description of market research conducted and results or statement why it was not conducted:

GSA Advantage shows that there are glaucoma treatment lasers available, but do not meet the specifications as stated in the requirement. There are no VOSB or SDVOSB’s listed on GSA e-Library under Schedule 65IIA (Medical Equipment and Supplies) SIN A-39 (Laser, Surgical). VIP market research for NAICS 339115 (Ophthalmic Goods Manufacturing) shows that there are 71 CONUS-based VOSB’s, of which 67 are SDVOSB’s.

The Danville clinic currently does not have a laser to treat Glaucoma patients (SLT Laser). Glaucoma patients make up many ophthalmology patients at the Danville facility. The current YAG and Argon lasers are not truly wheelchair accessible.

Dr. William H. Dillon at VA Illiana Healthcare System, Danville, IL, has provided a sole-source justification for this requirement. Lumenis is the only manufacturer in the world of a “Triple Laser” of the type the customer needs and therefore is the reason for the sole-source request.

In the Danville clinic, there are currently two (2) Zeiss lasers, a Yag and an Argon laser, that are supposedly “wheelchair accessible,” but not practical. They are huge and bulky with no maneuverability. Staff members literally must drag them into place across the floor, and then requires nurses or technicians to support the patient’s head and back because staff cannot get wheelchairs up to the lasers.

The Lumenis system has the Yag, Argon, and SLT Glaucoma laser on a small footprint, is easily maneuverable on a wheeled platform that can be rolled out of the way when not in use in Danville’s limited space.

Another advantage of the (Lumenis) Argon laser is that it can be upgraded when Danville gets a Retina specialist. The specialist could detach it and take it into the Operating Room and do Endoscopic or Indirect Ophthalmoscopic procedures, something the Zeiss laser will never be able to do.

There are many Veterans who require Glaucoma care at Danville that are having to be sent out to private Ophthalmologists just to get this basic care that could be provided to them at Danville with the Lumenis laser.

Lumenis has two authorized distributors for the Trio Laser – Melling LLC. dba Melling Medical and Technical Communities, Inc. dba Testmart. Testsmart’s Socio-Economic status is Small Business, Melling Medical is a SDVOSB.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. Note: COs are required to make a determination of price reasonableness IAW FAR 13.106-3. See the S19 Open Market (FAR13) Award Documentation Form with Abstract to document price reasonableness.
