About this file
This document is an Amendment of Solicitation/Modification of Contract (Standard Form 30) for a Department of Veterans Affairs (VA) Request for Quote (RFQ) 36C25026Q0010 for preventive maintenance (PM) services on lift equipment. The amendment extends the receipt of offers deadline to September 26, 2025, at 3:00 PM EDT, with all offers to be emailed to Alexis.Alexander1@va.gov by September 29, 2025, at 3:00 PM EST.
The accompanying questions and answers provide key details about the service requirements: the VA will provide parts for 177 units, the preventive maintenance will be performed once annually in February, and the contractor will be responsible for replacing batteries on lifts with failed batteries or those meeting replacement time. The service is limited to annual preventive maintenance, with the lifts' last battery changes occurring in February/March 2025. Straps have been changed as damage was identified, and upper limit switch upgrades have only been addressed in units requiring strap replacements. Two additional documents (lift inventory age and lift inventory) were referenced but not included in this file.
