About this file
This is a bilaterally signed federal contract award document for referral-based esoteric diagnostic pathology laboratory services. The Department of Veterans Affairs (VA) Northeast Ohio Healthcare System (VANEOHS) awarded Contract Number 36C25026P0534 to Mayo Collaborative Services, Inc. on April 16, 2026, for a six-month emergency contract period extending through October 16, 2026. The total contract value is $274,712.78, with a not-to-exceed ceiling established through monthly invoicing.
The contract procures specialized referral laboratory diagnostic testing services that cannot be performed in-house due to low testing volumes, specialized equipment requirements, complex methodologies, and workforce constraints. Services are organized by diagnostic capability categories (CLINs) rather than individual CPT codes and include autoimmune neurology and paraneoplastic diagnostics, molecular genetics and genomic testing, infectious disease and molecular microbiology, antimicrobial susceptibility testing, specialized clinical chemistry, cytogenetics, tissue and cell-based testing, and anatomic pathology services. The pricing schedule includes 139 line items with unit prices ranging from $25.40 to $2,717.00 per test, plus a $65,000 pass-through testing allocation for six months. All testing must be performed within the United States in accordance with CAP and CLIA accreditation standards. The contractor is responsible for specimen handling, technical analysis, professional interpretation, reporting of results compatible with VA electronic health records systems, 24/7/365 customer service support, and utilization reporting. Invoices are submitted monthly electronically via Tungsten Network and reviewed by the Contracting Officer's Representative to verify CLIN alignment, clinical appropriateness, and compliance with contract ceilings. 2. CONTRACT TITLE: Referral-Based Esoteric Diagnostic Pathology Services- Emergency Six
(6) Month Contract
3. BACKGROUND
The VA Northeast Ohio Healthcare System (VANEOHS) requires continued access to esoteric and referral laboratory testing services necessary for the diagnosis, treatment, and clinical management of Veterans receiving direct patient care.
Pathology diagnostic testing services are inherently patient-specific, clinically driven, and continuously evolving. These services are ordered based on individual Veteran presentation and provider medical judgment and cannot be reliably forecasted in advance. As such, the Government cannot reasonably define or predict, at the time of contract award:
The exact diagnostic tests required during the period of performance
The frequency or volume of testing
The specific methodologies that will be clinically appropriate at the time of service
The cost structure associated with advancements in diagnostic science
Testing demand is influenced by multiple variables, including:
Individual Veteran clinical presentation
Disease progression and acuity
Treatment response and monitoring requirements
Co-morbid conditions
Provider medical judgment and evolving standards of care
Clinical staffing and workload changes
As diagnostic science advances, individual tests may be replaced, expanded, or modified while maintaining the same clinical purpose. Accordingly, acquisition structures based on static or exhaustively itemized test lists are not compatible with the operational realities of esoteric diagnostic testing.
The VANEOHS Pathology and Laboratory Medicine Service (PALMS) is a fully accredited, high-complexity laboratory. However, certain esoteric tests cannot be performed in-house due to low testing volume, specialized instrumentation requirements, complex methodologies, and the need for subspecialty expertise.
Esoteric testing under this requirement includes highly specialized diagnostic services such as autoimmune neurology panels, molecular genetic testing, infectious disease sequencing, cytogenetics, specialized chemistry, and tissue-based diagnostics. These services support the identification of rare, complex, or multifactorial disease processes and are critical to completing diagnostic evaluations initiated by VA providers.
This requirement is being executed as an emergency six (6) month extension to prevent interruption of these services while a follow-on acquisition is completed. Interruption would result in delays in diagnosis, disruption of treatment, and increased clinical risk to Veterans.
Many diagnostic pathways require multi-step, reflex, or sequential testing, making continuity of services operationally and clinically critical.
In-House Capability and Need for Referral Testing:
The VANEOHS PALMS is a state-of-the-art, fully accredited laboratory service providing a comprehensive range of clinical laboratory and anatomic pathology services. The mission of PALMS is to deliver timely, accurate, and appropriate diagnostic services in direct support of Veteran patient care.
While PALMS maintains robust internal capabilities, certain autonomic and esoteric laboratory tests cannot be performed in-house due to one or more of the following factors:
Low testing volumes that do not justify in-house validation, staffing, and maintenance
Specialized or proprietary equipment requirements not available at the VANEOHS
Highly complex methodologies requiring subspecialty expertise
Referral-only assays available exclusively through tertiary laboratories
Workforce constraints, including vacant pathologist positions and increasing workload within Pathology
To ensure continuity of care and access to clinically necessary diagnostics, VANEOHS must rely on external referral laboratories, including the Mayo Clinic Laboratories, to perform specialized testing that cannot be reasonably completed in-house.
The requested requirement is being placed as a sole source procurement through the Mayo Clinic Laboratories. The Mayo Clinic Laboratories is uniquely positioned to provide the referral-based esoteric, and specialized pathology diagnostic services required under this requirement in an expeditious, clinically reliable, and operationally efficient manner.
4. SCOPE
This requirement encompasses referral-based laboratory diagnostic testing utilizing blood, urine, body fluid, tissue, and fecal specimens as applicable. Services include test performance, technical analysis, professional interpretation, and diagnostic correlation necessary to deliver complete, clinically actionable diagnostic results to VA providers.
The requirement is structured as a FFP contract due to the Government’s inability to predetermine patient-specific diagnostic needs, test types, methodologies, volumes, or future advancements in diagnostic science over the period of performance.
CLINs are organized by diagnostic capability categories, consistent with customary academic and commercial referral laboratory practice, rather than by individual CPT codes or discrete test itemization. CPT codes, where applicable, are used for invoicing and utilization reporting purposes only and do not define the scope of services being procured.
This requirement is for referral-based diagnostic laboratory services, not for the procurement of discrete supplies or standalone test items. Services are rendered in association with each diagnostic capability category (CLIN) and are performed on a patient-specific, case-by-case basis in direct support of clinical care.
Under this requirement, the Contractor shall provide specialized referral testing services necessary to complete, clarify, confirm, or refine diagnostic evaluations ordered by authorized VA providers. Services include the performance of testing, technical analysis, professional interpretation, diagnostic correlation, and reporting required to deliver complete, clinically actionable diagnostic results within the scope of each CLIN.
The scope of services is intentionally structured around diagnostic outcomes, rather than individual test names or billing descriptors, to reflect industry standards for referral and esoteric laboratory testing and to accommodate evolving methodologies throughout the period of performance. Testing may include specialized, proprietary, successor, or clinically equivalent assays offered by the referral laboratory that fall within the same diagnostic purpose, specimen scope, and clinical intent of the applicable CLIN.
All services performed under this contract are intended to support direct patient care, ensure continuity of diagnostic services not available in-house, and provide VA clinicians with the information necessary to make informed medical decisions. No tangible deliverables are required beyond the diagnostic services and results associated with each patient-specific referral.
General Requirements:
The Contractor shall provide referral-based clinical laboratory and anatomic pathology diagnostic services, including pre-analytic processing, analysis, professional interpretation, and reporting of results for specimens submitted by the VANEOHS, in support of direct patient care.

Exhibits – Representative Testing and Pricing Reference:
Purpose of Exhibits:
The contract includes one or more Exhibits that identify representative referral laboratory testing and diagnostic service types by test name or organized by diagnostic capability category. These Exhibits are provided for reference and planning purposes only and are not intended to serve as an exhaustive or fixed test menu, as referral testing often experiences changes or methodology and techniques during a period of performance.
The Exhibits are designed to support the Government’s understanding of the types of diagnostic services anticipated under this requirement and to assist in establishing reasonable not-to-exceed (NTE) ceilings for the contract and associated task orders.
Exhibit Content and Structure:
Each Exhibit may include, but is not limited to:
Representative test names commonly associated with each CLIN diagnostic category
General diagnostic service descriptions
Typical specimen types (e.g., blood, urine, fecal)
Indicative or estimated pricing information based on historical utilization, commercial reference data, or referral laboratory pricing structures
The Exhibits reflect diagnostic service categories, not guaranteed availability of specific tests, and are intended to illustrate the scope and nature of referral testing anticipated during the period of performance.
Exclusion of CPT-Level Itemization:
Specific CPT codes are intentionally omitted from the Exhibits and from the base contract CLIN structure. This omission is necessary due to the clinical, patient-specific nature of referral laboratory testing and the Government’s inability to reasonably predetermine:
Which specific tests will be clinically appropriate for individual patients
The frequency or volume of particular tests
Changes in test methodologies, platforms, or nomenclature over time
Annual or interim CPT code updates, reclassifications, or retirements
CPT codes, where applicable, may be utilized at the invoicing and reporting level only and do not define the scope of services authorized under the contract.
Role of Exhibits in Contract Administration:
The Exhibits are intended to function as reference tools, not contractual limitations. Specifically, the Exhibits shall be used to:
More fully identify the types of referral diagnostic services anticipated under each CLIN
Support Government planning and budgetary analysis
Establish and justify contract and task-order NTE ceilings
Facilitate invoice review and CLIN-level verification
Provide transparency into expected laboratory activity
The absence or presence of a specific test name within an Exhibit does not determine whether a service is authorized under the contract. Authorization is determined by whether the service:
Falls within the overall scope of the contract
Aligns with an authorized diagnostic capability category (CLIN)
Serves the same diagnostic purpose and clinical intent
Utilizes authorized specimen types
Pricing Schedule:
The Contractor shall provide a comprehensive Pricing Schedule at the time of contract award.
The Pricing Schedule shall:
Align with the overarching diagnostic CLIN categories and associated exhibits.
Identify unit pricing or pricing methodology for representative autonomic referral and esoteric laboratory tests.
Reflect the Contractor’s commercial pricing structure in effect at the time of award.
Serve as the baseline pricing reference for all orders issued during the Period of Performance.
The Pricing Schedule shall be incorporated into the contract as an attachment or exhibit under each associated CLIN and shall constitute the approved pricing framework for services performed under this agreement.
To ensure pricing transparency and contract administration integrity, the Contractor agrees that:
Unit pricing established at contract award shall remain valid for the duration of the contract period unless otherwise modified by bilateral contract modification.
Prices for testing services shall not be increased, restructured, or replaced
The Contractor shall not introduce alternative pricing methodologies, bundled charges, or revised fee structures that deviate from the approved Pricing Schedule without Government authorization.
6. Performance Monitoring
Invoice Review and Validation Process:
Invoices submitted under this requirement shall be reviewed using a multi-step verification process, which includes:

Clinical Alignment Review:
Ensure testing supports the documented clinical indication
Confirm that testing contributes to completion of the diagnostic evaluation
Pricing and Ceiling Review:
Validate pricing against approved contract rates or pricing methodology
Ensure cumulative charges remain within the requirement not-to-exceed (NTE) ceiling and applicable task order limits
Duplicate and Redundancy Check:
Confirm tests were not duplicative unless clinically justified
Validate add-on or reflex testing aligns with the original diagnostic intent
Contracting Officer’s Representative (COR) Oversight:
The Contracting Officer’s Representative (COR) shall provide ongoing oversight of services performed under this requirement to ensure compliance with contract scope, authorized diagnostic categories (CLINs), fiscal controls, and applicable VA policies.
COR oversight is focused on verification and administration of services, not clinical decision-making, and is intended to ensure that all referral laboratory services are rendered under this contract:
Fall within the authorized diagnostic capability categories (CLINs)
Support direct patient care based on valid provider orders
Are consistent with the scope and purpose of the requirement
Are properly invoiced and applied against the correct CLIN and contract ceiling
COR Responsibilities:
The COR shall be responsible for the following oversight functions:
Reviewing invoices to confirm that billed services align with authorized CLIN categories and diagnostic purposes
Verifying that services invoiced were rendered and that diagnostic results or interpretations were provided
Ensuring that testing performed under CLIN 0010 (Miscellaneous, Successor & Emerging Esoteric Diagnostics) represents authorized successor or equivalent testing and does not constitute scope expansion
In the event of scope expansion, the COR will notify the Contracting Officer that a modification is required to include a new testing category and reject the invoicing of that service.
Monitoring cumulative expenditures at the CLIN and contract level to ensure compliance with not-to-exceed (NTE) ceilings
Requesting clarification or supporting documentation from the Contractor when test categorization, scope alignment, or pricing requires validation
Identifying potential discrepancies, anomalies, or trends that may indicate inappropriate billing, duplication of services, or scope misalignment
