1. Question: In which year are the five-year full load tests due for the traction elevators?

Answer: These tests will need to be conducted in the base year of the contract.

2. Question: In which year are the rupture/overspeed valve tests due for the hydraulic elevators? These tests will require full load/capacity weights to properly perform this inspection.

Answer: These tests will need to be conducted in the base year of the contract.

3. Question: Will this contract only require 1 visit per year?

Answer: A semi-annual and annual testing will be required, 2 visits a year.

4. Question: Has your agency ever acquired this service before and is there an incumbent? If so, what is the contract number and value?

Answer: We have had a previous contract, but I am not at liberty to disclose any further information.

5. Question: What scope changes have been made from the old contract to the new contract?

Answer: Some elevators were taken off of the original SOW, some elevators were upgraded.

6. Question: How come there is only a week to respond to this and no site visit?

Answer: This is an emergent need of services.

7. Question: Can you provide the CATS and Rupture Value due dates so that we can price it correctly in the price schedule?

Answer: CATS & Rupture Valve tests need to be completed within the first year

A.2 STARTMENT OF WORK

VA NICHS, Fort Wayne and Marion Campuses Elevator & Dumbwaiter Annual and Semi-Annual Inspections and Testing Part I: General Information A. Introduction The Department of Veteran Affairs VA Northern Indiana Health Care System (VANIHCS) requires semi-annual maintenance inspections and annual witnessing of safety tests on the elevators located at both Marion and Fort Wayne campuses.

B. Background VA NIHCS is a medical facility providing both inpatient and outpatient services to Veterans. This service is to ensure the elevators and associated equipment is maintained in accordance with all codes and regulations.

C. Scope The contractor shall provide all materials, labor, and expert supervision to perform: Semi-Annual/ Annual safety inspections and testing, witnessing annual safety test, witnessing annual rupture valve test, and witnessing five-year full load test as required to meet any and all codes required per industry standard in strict accordance with all applicable Federal, State, and local laws, regulations, codes and directives. This shall include (18) elevators located at Marion, Indiana Campus and (4) located at the Fort Wayne Campus, in accordance with the manufacturer instructions and specifications herein and subject to the terms and conditions of this contract.

PART II: WORK REQUIREMENTS

A. Technical Requirements

1. The Contractor shall provide all supervision, labor, parts, equipment, supplies, tools, and transportation required.

2. The contractor shall witness, oversee, and monitor rupture valve test and safety tests of the elevators and dumbwaiters. Attached herein is the list of elevators and dumbwaiters of both campuses. The “American Standard Practice for the Inspection of Elevators, Inspectors Manuel, ASME A17.2 (latest edition), and the American National Standard Safety Code for elevators, dumbwaiters, escalators and moving walks, ASME, A17.1 (latest edition), shall be used by the inspector as a guide in performing inspections and witnessing tests.

3. The contractor shall accomplish semiannual maintenance inspections, witness the annual no-load tractions, annual hydraulic pressure tests, annual hydraulic rupture valve tests and five-year traction load (with the assistance of the VA elevator maintenance contractor). The VA elevator maintenance contractor shall perform all the test and provide and set up the weights for the full load tests.

4. The contractor shall notify the COR at applicable campus not less than thirty (30) days in advance of the dates proposed to visit the campus. In the event dates cannot be met, the contractor shall provide the COR 24-hour advance notice of cancellation.

5. Inspections shall be thorough and cover all items as specified. A minimum time of one and one half (1 ½) hours shall be spent on each elevator and one (1) hour on each dumbwaiter and/or cart lift per inspection. Parts shall be removed where necessary to determine the exact condition of parts which could not otherwise be determined, and the equipment shall be cleaned where necessary to make the proper inspection.

B. Deliverables

1. As a part of VA NIHCS quality control program, a competency verification forms shall be completed and returned. Also, provide copies of any technician training that may be available. Provide calibration reports of equipment used for testing/inspection.

2. VA forms 10-6166, Report of Inspections of Elevators and 10-6167, Report of Test of Elevator Governor and Safety Devices shall be thoroughly completed for each test and inspection. Completed forms shall be signed and received in duplicate by the COR at the designated VA campus prior to processing the invoice. Payment will not be processed until completed forms are received by the COR.

Part III. Supporting Information A. The C&A requirements do not apply, and Security Accreditation Package is not required.

E. Qualifications of Key personnel Inspectors shall be certified by an organization accredited by NAESA in accordance with the requirements of ASME QE1-1. All employees assigned by the Contractor to perform inspections and tests shall have a current certification to inspect elevators by the licensing authority of a political subdivision or recognized authority, be physically able to perform the assigned work, and be free from any communicable diseases. A copy of the current certification for each employee shall be submitted to the Contracting Officer’s Representative (COR) prior to the start of work.

F. Hours of Work Normal hours of work are 7:00 a.m. to 3:30 p.m. Monday through Friday, excluding federal holidays. Federal holidays are New Year's Day, Birthday of Martin Luther King, Jr. , Washington’s Birthday, Memorial Day, Juneteenth National Independence Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and any other day specifically declared by the President of the United States to be a national holiday (see OPM.gov).
