About this file
This document provides details for a federal contract opportunity to renovate an existing space into a simulation lab at the VA Northern Indiana Health Care System campus in Fort Wayne, Indiana. The project involves gutting an entire room in the basement and remodeling it to include a classroom, control room, mock patient room, and mock exam room. All hazardous materials such as asbestos and lead-based paint must be properly removed and handled according to OSHA guidelines, with testing conducted to identify locations. The contractor must follow construction drawings and specifications from the architect/engineer firm. Additional requirements include testing and balancing, using specified equipment brands where listed, and completing the project within 210 calendar days from the notice to proceed. The justification provided is that the existing space is not suitable for use as a simulation lab and does not adequately simulate patient exam and room environments.

QUESTION AND ANSWER (RFI)
CORRECTION AND CLARIFICATION
Question 2: Is FAR 52.229-3 Federal, State, and Local Taxes applicable to this project?
Answer 2: Yes, FAR 52.229-3 is a clause added to federal contracts and deals with the applicable taxes related to the project. It governs whether federal, state, and local taxes must be paid by the contractor or the government agency. Please review this clause.
Question 3:
There are several spec sections listed (see below) on the table of contents with no corresponding section within specs. Are these missing or were these spec sections incorrectly listed on the table of contents?
- 25 10 10 ADVANCED UTILITY METERING SYSTEM
- ALL SPEC SECTIONS LISTED UNDER DIVISION 33 - UTILITIES SECTION OF
SPECS
- ALL SPEC SECTIONS LISTED UNDER DIVISION 34 - TRANSPORTATION SECTION
OF SPECS
- ALL SPEC SECTIONS LISTED UNDER DIVISION 48 - ELECTRICAL POWER
GENERATION SECTION OF SPECS
Answer 3:
Specifications sections are not required for this project.
Question 4:
Are we to include patient lift in this project? There is no spec for patient lift. However, the drawings show a patient lift system. Please clarify what is to be included as part of this project.
Answer 4:
All offerors need to provide a quote for the product and installation of a Handicare model # C625 patient lift and rail system. If documentation is required, then the VA will furnish it to the awardee.
Question 5:
Please clarify the scope of work pertaining to casework/cabinetry. Refer to Drawing QH101, Casework Schedule. It does not clarify if these (F0005 - F0020) are to be provided and installed by contractor or if this is by the owner?
Answer 5:
The offerors must provide a quote for product and installation of the Herman Miller Compass casework. A certified Herman Miller installer must be part of the bid.
Question 6:
There is a small "curb" wall around the perimeter of the room that currently has rubber base glued to it. The drawings call to "Remove resilient tile flooring & Wall base". Please clarify what is expected regarding this concrete base? Are we to simply remove the rubber base from the concrete, or is the concrete to be cut & chipped away/removed? See below Attachment.
Answer 6:
The concrete curb should be removed and infilled as needed to allow for a smooth finished wall.
Question 7:
Drawing HA101 is the hazardous abatement plan & calls out "Area for removal of asbestos-containing flooring and associated mastic". The statement of work located on page 5 of 72 of the solicitation document states "ACM and lead based paint are expected to be present in the construction area. All hazardous material must be removed and handled per OSHA guidelines.
The contractor shall conduct their own CSM testing to confirm the location and type". For the purposes of this bid, should the contractor only include abatement of floor /mastic as shown on HA101? confirm that any asbestos removal above & beyond what is shown on HA101 would be treated as a change order?
Answer 7:
The offeror will include a cost for performing their asbestos-containing materials (ACM) investigation in their base bid. The results of this report shall be shared with the VA. If additional ACMs that differ from the design drawings are discovered, it will be handled accordingly.
Question 8:
Confirm working hours are off hours? can any work be done during normal working hours?
Phasing Plan, Phasing General Note B states "Project shall be completed during night shifts, 5:00 pm to 5:00 am. Coordinate with COR. All work to be completed and open for public use at the end of each night's work." Is this the intent during the entire project that the space must be made "usable" at the end of each shift? Or will we be given the entire space & we have the entire period of performance to complete the work at which point it will be turned back over to the VA/public use.? Please clarify the intent regarding what is expected.
Answer 8
The basic work shall be conducted between 7 am and 4:30 pm. The space will not be available to the rest of the hospital during this project and locked outside of construction hours. If Patient Care in the surrounding areas require the demo to be temporarily halted at times, then the COR corridor will handle it. Coordinating a laydown yard with the COR and other projects is necessary due to the limited space available outside.
