See attached document: D.4 36C25023Q1850 Reissue 9.14.2023.

Questions and Responses.

1. The contract start date is listed as November 1, 2023. Question: Can the government please change the contract start date to January 1, 2024?

Response: The current contract ends 31 Oct 2023. If the winning bid requires longer setup time before starting service, then the Govt will choose, if advantageous, to extend the current contract until a new vendor can assume service. We ask the vendor be explicit in their offer when they can start the service.

1. Page 17 of 102, 1.3.8 Special Qualifications states: “Shall hold and maintain all required certifications and licensures required to legally provide ambulance services by the local, state and regional government or public entity where the emergency ambulance service is performed (or governed by) to include but not limited to the states of Ohio and Indiana as required by law.” Question: Does this contract include Indiana? If not, then all references to Indiana need to be removed from the solicitation.

Response: Amendment will change this sentence of PWS.

1. Page 17 of 102, 1.3.8 Special Qualifications states: “For the solicitation all offeror(s) shall submit a signed copy of their Emergency, Medical, Fire and Transportation (EMFTS) certification to perform ambulance service in the state of Ohio with their offer to be considered for award.” Question: Just to confirm, is it the government’s intent that in order for offerors to be considered for award, a signed copy of their Emergency, Medical, Fire and Transportation (EMFTS) certification to perform ambulance service in the state of Ohio MUST be submitted with their offer?

Response: the word signed will be deleted, but yes, we ask for proof of EMFTS License is to be submitted with the offer.

1. Page 31 of 102, 5.6.11 State of Ohio Approved Medical and/or Drug Protocols states: “The contractor shall provide a copy of their state of Ohio approved medical and/or drug protocol(s) with submission of their offer.” Question: I didn’t see this as a requested item in the Instructions to Offerors beginning on page 83 of 102. Please clarify if this is a requested item to be submitted in our offer.

Response: This section of PWS has been revised. It is not an evaluation criteria, but a requirement to perform the service.

1. Page 83 of 102 states: “Provide statement to confirmed ability to start performance within 30 days of contract award.” Question: Can the government change this statement to say: Provide statement to confirmed ability to start performance on January 1, 2024?

Response: The current contract ends 31 Oct 2023. If the winning bid requires longer setup time before starting service, then the Govt will choose, if advantageous, to extend the current contract until a new vendor can assume service. We ask the vendor to be explicit in their offer when they can start the service.

1. Page 83 of 102 states: “Provide three past performance evaluations or Completed E.2 52.212-2 (Addendum A) Past/Present Performance Questionnaire.” Question: I couldn’t find Completed E.2 52.212-2 (Addendum A) Past/Present Performance Questionnaire. Are Past/Present Performance Questionnaires required or will you simply review our CPARS?

Response: It will be reviewed and will be added to Amendment as attachment D.4.

1. Question: Is there space available at the Columbus VAMC that the awarded contractor can use as a hub of operations for a period of time, if not indefinite? If so, how much space is available?

Response: VAACC can accommodate two parking spots. The VAACC cannot provide space inside the VAACC. Contractor’s employees are welcome to use restrooms, public waiting areas and cafeteria.

1. Question: A.1 PRICE/COST SCHEDULE provided in Amendment 0001 is different than the A.1 PRICE/COST SCHEDULE provided in the reissued solicitation. Please clarify which A.1 PRICE/COST SCHEDULE we are to use in our offer.

Response: Please fill out the Price/Cost Schedule included with the most recent RFQ.
