B.2 STATEMENT OF WORK: FULL-SERVICE X-RAY GARMENT CLEANING

SERVICE

I. SCOPE OF WORK:

Background: The VA Ann Arbor Healthcare System (VAAAHS) is a tertiary care medical center that uses fluoroscopes and other x-ray producing machines to care for our nation’s veterans. Employees working with or around these machines are to wear personal protective equipment (PPE) that will protect them from harmful ionizing radiation. Ionizing radiation is known to have detrimental health effects and is therefore a physical occupational hazard from which we need to protect our employees. Lead-lined apparel, or its equivalent (nowadays this type of X-ray Wearableis made with an alternate alloy that is lighter by weight, but still has the same protective qualities as a layer of 0.25-cm lead), is used as personal protective equipment (PPE) or X-ray Wearables to protect workers from ionizing radiation. Like other forms of PPE, lead PPE is purchased by the services that use them and are the responsibility of each service to ensure they are inspected, tested, cleaned, and remain in good condition to provide the protection they are meant to provide. Lead PPE is used in operating rooms, procedures rooms, radiology, cardiac catheterization (“cath”) labs, and dental clinics, just to name a few. Many articles are assigned to staff, especially when those individuals are wearing them often; but there are times when lead PPE is shared and it is unknown or unclear whether pieces of lead PPE were cleaned between uses or when the lead PPE was last inspected or tested for cracks, holes, or other defects in the layer between the cloth that could allow for radiation to leak through to the worker. Also, whether assigned to an individual or not, some PPE cannot be found when the time comes to try to inspect it and test it under a fluoroscope for cracks, holes, or other leaks, and because there is no standardized system for keeping an inventory of lead PPE, it is unknown whether all pieces are inspected and/or tested each year.

The lead PPE must be inventoried, cleaned, inspected, and tested at semi-annually and there is no standardized process for ensuring these tasks are completed currently in place. This has recently come to be a The Joint Commission (TJC) requirement as well, with an emphasis on the inventory and tracking of the PPE and integrity testing. However, free, or low-cost, inventory software programs that were used in the past have proven to be ill-effective and are not consistently used across the services that use lead-lined PPE. This service contract must ensure that the VAAAHS has all pieces of lead PPE accounted for, thoroughly cleaned and sanitized, inventoried and documented, and fluoroscopically tested and visually inspected for defects that can be repaired if reasonably (economically) feasible, with services that must be submitted as an exhibit and approved by the Contracting Officer Representative (COR). This must take place while staff are continuing to work with loaner x-ray garments/lead PPE so that no garments are missed, and staff are still protected.

The term of this agreement is for one base year with four option years to extend the term of the contract.

II. CONFORMANCE STANDARDS:

At the commencement of the contract, the VAAAHS will submit to the contractor the most recent and complete inventory of lead PPE apparel that we have on-hand. The contractor must then verify the VA Ann Arbor Healthcare System’s inventory, making corrections when necessary.

The inventory of lead PPE that the VAAAHS believes to currently have is as follows:

Department # Aprons # Skirts # Vests # Thyroid # Full Apron

# Gonad & ½ Shields

Dental 4 - - 5 - - Endo 1 12 15 8 - - Gen Rad 5 22 22 21 2 - General - 2 3 1 - - GI Rad 2 9 8 5 1 -

IR 12 33 32 8 - 2

Nuclear Med - 2 2 - - -

OR 21 38 40 65 4 2

Urology 4 - - - - - Toledo CBOC 11 2 Canton CBOC 2 2

TOTALS 62 118 122 117 7 4

Contractor shall verify inventory of lead PPE for each department within the facility. The contractor must be able to track individual pieces with a computer software program designed for maintaining accurate inventory of lead PPE pieces and provide reports of the inventory for audit/survey needs. The inventory shall be updated by the contractor to denote the date each piece was cleaned, disinfected, sanitized, inspected, and approved for continued wear and protection of staff.

Each department within VAAAHS that uses lead PPE to protect their staff shall have a semi-annual cleaning. The requirement is to have each lead PPE article within the VAAAHS cleaned and inspected at twice a year, but more may be required depending on use and circumstances regarding the use of lead PPE. (Some articles are worn nearly every day by staff, during invasive and long procedures, whereas some are only worn periodically and will not need to be cleaned more than twice per year.) Lead PPE articles need to be cleaned and decontaminated, with ATP testing to show the level of gross decontamination at baseline. This data must be used to track and trend the use of lead PPE and to later identify areas, departments or specific pieces of lead PPE that may require more or less servicing in the future. After the cleaning and decontamination of articles, they shall be sanitized, repaired and inventoried with an RFID tag for tracking of the apron in the future. The Ann Arbor VA Medical Center does authorize the contractor to sew RFID tags onto the X-ray Wearables during cleaning by the Contractor. for tracking purposes onto Government owned X-ray Wearables.

Note: requirement is for semi-annual cleaning of each lead PPE. Once PPE is identified and needing higher frequency of cleaning, a report must be submitted to the COR identifying the pieces needing higher frequency of cleaning. Report must include breakdown of cost for increase of cleaning. The contracting officer will issue a bilateral modification to document the changes to include effective date of commencement of change to the contract.

In addition to visual inspections to be performed by the contractor at each service round, each lead PPE article shall be X-ray scanned once per year to determine whether there are cracks or holes in the protective inner layer that would render the PPE ineffective. X-ray images must be provided to the COR to review to determine if X-ray Wearables should be replaced or taken out of circulation.

Loaner lead PPE pieces and wearables shall be made available for VAAAHS staff to use while government owned lead PPE articles are being serviced. Contractor shall confirm with the COR for the contract how many and what types of lead PPE loaners will be needed while the government owned PPE is being serviced. Each article of loaner pieces shall be received by the VAAAHS in clean, sanitized and inspected condition to ensure that the article is safe for wear of staff while performing procedures and must present no risk to staff or patients. Loaner Pieces will be returned to the contractor when the government owned lead PPE articles are received and put back into use by staff at the VAAAHS; loaner pieces are not to be worn outside of the department in which they are intended to be worn and must not leave the VAAAHS premises as they are not government property.

Prior to each Cleaning Period, VAAAHS will identify any repairs to be made to the lead PPE;

however, the contractor will visually examine garments for any needed repair work and must notify the COR of any additional repairs needed that were not reported initially. If the contractor visually examines garments and notes needed repair work, contractor must discuss with COR to determine whether the repair is economically feasible or whether the lead PPE wearable is beyond economical repair. Contractor shall accommodate a majority of all repair needs, for example, Velcro repairs to wearables.

Contractor must provide delivery of loaners to VAAAHS, at which point, VAAAHS will be prepared to provide the Government owned X-ray Wearables for servicing. Quote must provide method of delivery. If shipping is the proposed method of delivery, shipping and handling charges must be incorporated into the cost of the contract.

Once services have been completed and the VAAAHS-owned garments are returned to the VAAAHS, loaner garments will be returned to the contractor and the process will start over in another area of the hospital, i.e., the next department to be serviced until all departments have had an initial, baseline, cleaning, disinfecting, sanitizing, and inspection. This contract shall allow for two rounds of cleaning, disinfecting, sanitizing, inspection, repairs, and inventory tracking and documenting for each article of X-ray Wearables within each contract year.

III. HOURS OF WORK:

A. Hours of Work are defined as weekdays Monday through Friday, except for United States Federal Holidays, from 8 a.m. to 4:30 p.m. Eastern Standard Time. For shipping and handling purposes, those hours are 8 a.m. to 3 p.m. when the onsite warehouse closes.

Service:

Lead PPE is required to be inventoried, cleaned, inspected, and tested at least annually by The Joint Commission (TJC) standards to which we are evaluated during every survey period, triennially. This (TJC) requirement has recently been known to put an emphasis on the inventory and cleaning of the PPE as well as integrity testing. This service contract will ensure that the VAAAHS has all pieces of lead PPE accounted for, thoroughly cleaned and sanitized, inventoried and documented, X-ray tested and visually inspected for defects, and repaired if reasonably (economically) feasible, all while staff are continuing to work with loaner x-ray garments/lead PPE so that no garments are missed, and staff are still protected, as specifically set forth herein.

IV. PERFORMANCE MONITORING, DOCUMENTATION & REPORTS:

The contractor shall document and provide detailed service reports for services provided that meet TJC standards. Any additional charges not included within this service contract shall have the prior approval by receipt of a separate purchase order before those costs are incurred. Any costs incurred without obtaining a separate purchase order may be determined by the VA to be unauthorized work and the contractor may bear the burden of such costs.

The contractor’s performance shall be monitored by the COR throughout the installation and verification process. All coordination of shipping and receiving of VA-owned and contractor-owned lead PPE articles will go through the COR.

V. COMPETENCY OF PERSONNEL SERVICING EQUIPMENT:
