About this file
This document contains an amendment to a solicitation for surgical instrument maintenance services at the Richard L. Roudebush VA Medical Center in Indianapolis. The key details are as follows:
The amendment provides responses to requests for information regarding the solicitation requirements. It clarifies that ISO 13485:2016 certification is mandatory and would be grounds for disqualification if not held. While the medical center has its own instrument marking units, the selected contractor will be responsible for providing engraving and censitrac barcoding services. The specified frequency of four weekly on-site visits is mandatory as stated in the Statement of Work. The solicitation covers maintenance and repair of endoscopic instruments and does not include endoscopes, cameras, or power equipment. Pricing must account for repairs that cannot be done on-site due to complexity or equipment limitations.

The following questions and answers are provided as a response to an RFQ.
1. If our small business does not have ISO 13485:2016 certification, does that disqualify us?
Answer: ISO 13485:2016 is a requirement and would be a disqualification factor.
1. Does the Indy VA have its own templates and engraver for censitrac bar coding?
Answer: The Richard L. Roudebush VA Medical Center does have instrument marking units. While the medical center does have marking units, the contractor shall provide engraver and censitrac bar coding services.

Answer: It is unknown the current number of instruments barcoded (marked).
