This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested. The Government reserves the right to make no award from this solicitation.

Solicitation number 36C25021Q0448 is being issued as a Request for Quote (RFQ). The government anticipates awarding a firm-fixed price contract resulting from this solicitation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-05, effective March 10, 2021.

The associated North American Industry Classification System (NAICS) code for this procurement is 561720 (size standard $19.5 million).

This requirement is 100% set aside for Small Business.

The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO) 10 requires a contractor to provide annual generator testing and preventive maintenance for the VA Ann Arbor Medical Center, located in Ann Arbor, Michigan, for a period of performance of one year with four possible option years. Please reference the attached Statement of Work for specific project information and requirements.

The anticipated contract period is June 22, 2021 through June 21, 2026, if all options are exercised.

Quotes from all businesses shall be received on or before the date and time specified in Block 8 of the SF1449, Solicitation. Solicitation posting and closing dates are subject to change as necessary and amendments will be issued accordingly.

This solicitation requires registration with the System for Award Management (SAM) prior to award pursuant to applicable regulations and guidelines. Registration information is at www.sam.gov.

This is an open-market combined synopsis/solicitation for services as defined herein. The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:

A site visit will be held Tuesday, May 18, 2021 at 9:00 AM EST, at 2215 Fuller Road, Ann Arbor, MI 48105. Parking isv located in the visitor's garage off Fuller Road. Face masks covering nose and mouth shall be worn at all times, on site.

This solicitation includes FAR 52.222-41, which describes contract requirements under the Service Contract Labor Standards Act. Offerors are responsible for complying with the attached wage determination and determining appropriate classification to apply to its workforce.

B.2 STATEMENT OF WORK

The Ann Arbor VA Medical Center, located at 2215 Fuller Road, Ann Arbor, Michigan 48105, requires annual interior and exterior window cleaning. The contractor shall include all necessary labor, supervision, management, transportation, supplies, equipment and travel necessary to complete the requirement.

There will be two window cleaning jobs per year:

Interior & Exterior Window Cleaning (Annually): All windows in all buildings, trailers, and parking structures shall be thoroughly cleaned on interior and exterior window surfaces. Window surfaces shall include window frames, glazing, sills, and screens. Once cleaned, all surfaces shall be clean, free of film, smudges, foreign matter, streaks, stains, and shall be wiped dry. Daily rounds will be made by the Contracting Officer’s Representative (COR) to ensure standards for the requirement have been met. There are approximately 5,850 windows with a total of 51,750 square feet of surface area on each side on the entire campus.

Exterior Window Cleaning Only (Annually): Thorough exterior window cleaning to exterior side only of the windows of all medical center buildings, trailers and parking structures. Exterior window cleaning shall include window frames, glazing, sills, and screens. Once cleaned, all surfaces shall be clean, free of film, smudges, foreign matter, streaks, stains, and shall be wiped dry. Daily rounds will be made by the Contracting Officer’s Representative (COR) to ensure standards for the requirement have been met.

When cleaning exterior windows, all run-off shall be removed to prevent staining on lower windows and walls. Cleaning means removal of all dirt, dust, bugs, bird droppings, spider webs, nests, and staining. Care should be taken not to disturb patients and employees. Customers shall be informed in advance by the COR that cleaning will occur so they may remove personal items before vendor’s arrival. COR shall provide access to secure areas and will provide necessary keys for windows and roof access. Keys and/or passes will be signed out at beginning of shift and shall be returned at end of duties each day.

All work is to be performed according to all local, state, and federal guidelines and ordinances.

Work is to be done using scaffolding, or Bosun’s type chairs and harness, or ladders may be used for windows located 2 stories and below. An industrial man lift may be used with permission granted by COR before commencing work. Ladders must be Type I or Type II industrial ladders and comply with all safety requirements. All staff must have received all federal, state, and locally required training/certifications regarding the use of all window cleaning equipment, machinery, man-lift equipment, supplies and procedures before performing work at the Ann Arbor VA Medical Center, and contractor shall provide the COR with employee-signed documentation that they have received said training(s).

Contractor shall meet all OSHA requirements, including requirements for fall protection covered in CFR 29, Part 1910.140 and Part 1926. Before commencing work, contractor shall meet with VA Safety and Engineering staff to ensure proper anchoring points are used and understood. At no time shall contractor use building equipment as an anchoring point. A copy of the current year’s anchor point inspection shall be given to contractor before work begins. Copies of all Safety Data Sheets will be provided to the COR for all cleaning chemicals being used by vendor.

Hours of Work:

Hours of work will be Monday through Friday, from 7:00 a.m. to 4:30 p.m., excluding federal holidays. Holidays observed by the Federal Government are New Year’s Day, Birthday of Martin Luther, Jr., Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. At the discretion of the President of the United States of America, any additional federal holidays may be added.

Scheduling:

Contractor shall coordinate all window cleaning jobs with the COR, who will approve when window cleaning can commence. Contractor shall give daily work schedules to COR when beginning the cleaning project. Contractor shall complete all work within 31 consecutive days.

Training:

No person may be assigned to work under the provisions of this contract until the COR has been given certification that training, and orientation has been provided to the individual. It must be shown that individuals are competent to use the equipment required in the performance of this contract.

Uniforms and Identification:

Contractor’s employees shall wear a uniform that is distinct and different than uniforms worn by VA personnel. A contractor-provided badge identifying the employees name and company shall be worn at all times and shall be clearly visible. Any incident or situation outside the scope of normal cleaning duties shall be reported to the COR and/or VA Police Department immediately. Changes outside the scope of the contract shall not be performed unless authorized, in writing, by the Contracting Officer, or upon receipt of a separate PO.

At certain times, contractor personnel may be required to wear special protective clothing, which shall be provided by the VA Medical Center. All non-disposable items shall be returned to the VA in good condition or shall be replaced at contractor’s expense.

Conformity to Regulations:

The contractor shall conform to all federal, state, and local regulations governing the licensing and certification of glazing maintenance operators, performing of glazing maintenance, and use of chemicals and equipment.

Service Completion Ticket:

Upon completion of services, contractor shall provide the COR with a service ticket including dates of service and a narrative of completed work.

C.2 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days of contract expiration.

C.3 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days of contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed shall not exceed five (5) years.

C.7 SUPPLEMENTAL INSURANCE REQUIREMENTS

In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:

(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.

(b) General Liability: $500,000.00 per occurrences.

(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.

(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

SECTION E - SOLICITATION PROVISIONS

E.1 ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.

The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:

1.0 OFFER SUBMISSION

Offers shall be submitted via email, with the RFQ number included in the email subject line, to Jeanne Ortiz at jeanne.ortiz@va.gov by the date and time identified on the SF1449, block 8. Any and all questions shall be submitted via email to Jeanne Ortiz at jeanne.ortiz@va.gov no later than Friday, May 21, 2021, by 10:00 AM Eastern Standard Time (EST).

Offers shall be assembled by the following Sections; each section shall be easily identified.

Section A - Offer Form (Standard Form 1449) Signed and dated. Data Universal Numbering System (DUNS) Number shall be entered in the block with the Offerors name and address.

Section B – Pricing Pricing shall be submitted as requested, in strict accordance with the Price Schedule, with no alteration to the price schedule whatsoever. Total price will be evaluated by the Government.

Section C – Offerors Representations and Certifications A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at www.sam.gov/portal/public/SAM/.

All Offerors shall be registered and current in the System for Award Management (SAM) database at time of offer.

Section D – Technical Approach Offerors shall submit a quote that addresses all aspects of the Statement of Work (SOW). All required certifications shall be submitted as part of the offer.

The offeror shall identify any subcontractors proposed under this solicitation. All technical elements applicable to the utilization of subcontractors must be addressed in the technical quote and detail provided.

The technical approach section should be as detailed as necessary to fully explain the proposed methods and rationale; it should reflect a clear understanding of the nature of the work being undertaken. The technical quote shall include information on how the project is to be organized, staffed, and managed.

The Technical Volume (Section D) shall not exceed twenty (20) pages in length (minimum 12-point font and no less than ½ inch margins). Page limitation includes any drawings, charts, images, etc., and excludes section dividers, table of contents, glossary of terms, cross-referencing indices, and resumes. If more than 25 pages are submitted (not counting excluded pages), only the first 25 pages will be considered.

Section E – Past Performance and Past Experience Identify all federal, state, and local government contracts as well as any private/commercial contracts of similar scope, size, complexity that are ongoing or have been completed within the last three years. List the following information for each contract:

· Company name and address;

· Description of services performed;

· Name, telephone number and email address of responsible individuals who have first-hand knowledge of performance relative to similar type of services;

· Dates of contract performance; and

· Contract type (e.g. fixed-price, cost reimbursable) and total contract value.

Failure to submit a complete offer may result in an unacceptable determination.

2.0 AWARD BASIS

Note that FAR Subpart 13.5 Simplified Acquisitions Procedures for Certain Commercial Items procedures will be used. The Government intends to award a firm fixed-priced contract to the responsible offeror whose offer, conforming to the requirements in this request for quote, will be most advantageous to the Government, price and other factors considered. An award will be made after comparative evaluation of offers in accordance with FAR 13.106-2, based upon the following factors:
