3. INVOICES: Invoices shall be submitted in arrears: Monthly

4. GOVERNMENT INVOICE ADDRESS: All invoices from the contractor shall be submitted electronically in accordance with VAAR Clause 852.232-72, Electronic Submission of Payment Requests. Invoicing shall be in accordance with the requirements of the Department of Veterans Affairs (VA) Financial Services Center (VAFSC). The latest information shall be used for invoicing and payments and may change during the period of performance of this acquisition. Current Invoicing information is located at: http://www.fsc.va.gov/einvoice.asp.

All invoices shall include the following:

· Purchase Order Number

5. CHANGES/MODIFICATIONS TO THE CONTRACT: The Contracting Officer will be the only person authorized to approve changes or modify any of the requirements under this resultant contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer will be authorized to make commitments or issue changes that affect price, quantity, delivery, or quality of performance of this resultant contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered unauthorized by the local VAMC, and no adjustment will be made in the contract price to cover any increase in cost incurred as a result thereof.

9. System for Award Management: Federal Acquisition Regulation requires contractor registration in the System for Award Management (SAM) database prior to the award of a contract. Offerors and Contractors may obtain information on registration and annual confirmation requirements via the internet at https://www.sam.gov/portal/public/SAM/.

B.2 REVIEW REQUIRED REGISTRATION WITH CONTRACTOR PERFORMANCE ASSESSMENT SYSTEM (CPARS)

As prescribed in Federal Acquisition Regulation (FAR) Part 42.15, the Department of Veterans Affairs (VA) evaluates contractor past performance on all contracts that exceed $250,000 and shares those evaluations with other Federal Government contract specialists and procurement officials. The FAR requires that the contractor be provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement VA uses an online database, CPARS, which is maintained by the Naval Seal Logistics Center in Portsmouth, Virginia. CPARS has connectivity with the Past Performance Information Retrieval System (PPIRS) database, which is available to all Federal agencies. PPIRS is the system used to collect and retrieve performance assessment reports used in source selection determinations and completed CPARS report cards transferred to PPIRS. CPARS also includes access to the federal awardee performance and integrity information system (FAPIIS). FAPIIS is a web-enabled application accessed via CPARS for contractor responsibility determination information.

Each contractor whose contract award is estimated to exceed $250,000 is required to register with CPARS database at the following web address: www.cpars.csd.disa.mil. Help in registering can be obtained by contacting Customer Support Desk @ DSN: 684-1690 or COMM: 207-438-1690. Registration should occur no later than thirty days after contract award and must be kept current should there be any change to the contractor's registered representative.

For contracts with a period of one year or less, the contracting officer will perform a single evaluation when the contract is complete. For contracts exceeding one year, the contracting officer will evaluate the contractor's performance annually. Interim reports will be filed each year until the last year of the contract, when the final report will be completed. The report shall be assigned in CPARS to the contractor's designated representative for comment. The contractor representative will have sixty days to submit any comments and re-assign the report to the VA contracting officer.

Failure to have a current registration in the CPARS database, or to re-assign the report to the VA contracting officer within those sixty days, will result in the Government's evaluation being placed on file in the database with a statement that the contractor failed to respond.

B.3 SCHEDULE OF SERVICES

Contractor shall provide Outpatient Cardiac Event and Holter monitoring services that includes equipment, monitoring, equipment processing and training for facilities located within the Veterans Integrated Service Network 10 (VISN 10). VISN 10 Healthcare System serves Veterans at 12 Medical Centers, and associated Community Based Outpatient Clinics (CBOCs), and satellite facilities throughout Ohio, Indiana, and Michigan. This Indefinite Delivery, Indefinite Quantity (IDIQ) contract shall be for five (5), one-year ordering periods from May 13, 2021, through May 12, 2026.

The quantities stated in this section represent the Government’s best estimate, based on historical usage and projected need. Please note that they are only estimates and are not a guarantee of future orders. The guaranteed minimum only applies to the Federal Government Fiscal Year in which the contract is awarded. The total minimum amount or guaranteed minimum monitoring services for this contract is 650 inclusive of CPT Codes 93271, 93229, 93226 and 0297T. The minimum guaranteed amount shall be expended in the initial fiscal year in which this contract is awarded.

The maximum monitoring services for this contract inclusive of all Ordering periods is 28,832 inclusive of CPT Codes 93271, 93229, 93226 and 0297T. Individual task orders shall be funded annually. The Contracting Officer is the only authorized individual that can place annual task orders against this contract for the estimated amount of services for the specific timeframe, and the task orders shall be issued to the contractor. Offerors shall offer pricing for the entire estimated quantity for each CLIN.

B.4 PERFORMANCE WORK STATEMENT

1. SERVICES REQUIRED: Contractor shall provide Outpatient Cardiac Event and Holter monitoring services that includes equipment, monitoring, equipment processing and training for facilities located within the Veterans Integrated Service Network 10 (VISN 10). VISN 10 Healthcare System serves Veterans at 12 Medical Centers, and associated Community Based Outpatient Clinics (CBOCs), and satellite facilities throughout Ohio, Indiana, and Michigan. The period of performance shall be from May 13, 2022, through May 12, 2027, for five (5), one-year ordering periods.

Services shall be provided to the facilities listed below:

2. REQUIREMENTS:

2.1. GENERAL:

Contractor shall show that their mode and method of patient training addresses a variety of appropriate training methodologies (e.g., verbal, written, video).

Contractor shall furnish, for the life of the contract, the following equipment at no additional cost to the Government:

a. Personal cardiac event (arrhythmia) monitors

b. Personal cardiac Holter monitors

c. Personal cardiac event, with optional Holter monitor capabilities

d. Current operator’s manual(s)

The system shall have an on-board manual to include system operation and troubleshooting. Contractor shall repeat the monitoring study at no additional cost to the Government for the following reasons: device provides unsatisfactory results, device fails to transmit or download information, information is not retrievable using the web-based system, or there is an imaging defect or artifact determined by the VA to be substandard.

All issuing, and follow-up care provided under this contract shall be provided in accordance with all current Joint Commission standards.

2.2. EQUIPMENT:

a. All equipment offered shall be FDA approved.

b. All cardiac event and Holter monitors shall be provided by the contractor for VA use and shall be state-of-the-art equipment, lightweight, and simple for patients to use. Patients will use the event monitor for up to thirty-days (30), or as specified by the VISN 10 VAMC Provider. Samples shall be provided to each site during the training or orientation process, which is discussed further in this PWS.

c. The contractor and facility COR will determine an equipment inventory level at the time of contract award that shall always remain at each facility location. This equipment inventory shall be adequately restocked at all times, according to pre-determined inventory levels for each facility, to ensure that the facility can always meet the daily patient demand. The vendor and facility COR shall evaluate stock levels bi-annually and modify according to usage trends.

d. The Contractor shall provide Home Enrollment as ordered by the VA. Home Enrollment shall include mail delivery of device to the patient. Home Enrollment shall include 24/7 Customer Support which will include patient teaching via phone on self-placement. Home Enrollment of Cardiac Device shall be determined by VA on an individual patient needs basis.

e. All equipment remains the sole property of the contractor including the initial delivery of the units. The contractor is responsible for all service maintenance and repairs of the equipment and hardware.

f. The contractor shall supply all necessary supplies, cleaned, and reset monitors, and accessories in monitor package including, but not limited to, instructions, electrodes, batteries, and diaries.

g. Contractor-supplied monitors shall be issued with a postage-paid envelope for return of monitors to the contractor or the VISN 10 VAMC, where applicable.

h. Upon enrollment of an event monitor, the contractor shall then issue the same type of monitor that has been provided to all Veterans within three to five (3-5) business days. At the end of the monitoring period, up to thirty (30) days, the patient will return the monitor to the contractor or the applicable VISN 10 VAMC, who will then return it to the contractor. All patient supplies shall be included in the contract (e.g., monitor, holder, ECG electrodes, batteries, postage-paid shipping container).

2.3. MONITORING SERVICE:

1. Equipment that is assigned to patients shall require continuous monitoring twenty-four (24) hours a day.

1. Cardiac Event monitors shall be capable of transmitting data in real time.

1. Contractor shall immediately inform a designated VISN 10 VAMC point of contact of any significant changes as specified by the contractor’s qualified healthcare professional. This notification shall be via Health Insurance Portability and Accountability Act (HIPAA) compliant email, fax, or phone. VISN 10 VAMC point of contact may be different outside of normal business hours.

1. VAMC POC shall work with the Contractor to establish parameters for proper notification of alerts

1. Capability of event transmissions being sent via wireless/cell phone technology. If a patient does not have a wireless connection, then a cell phone will be provided by the Contractor. The cell phone will be used as a transmitter to transmit cardiac events in real time to the Contractor’s support team. Contractor shall provide multiple cell phone carriers to increase likelihood of adequate cellular coverage for all VA facilities and catchment areas within the scope of this contract.

1. Event Auto Trigger Technology.

1. Full Disclosure Recording capabilities.

1. Per hour calculation of the burden # of premature ventricular contractions (PVC’s) or premature atrial contractions (PAC’s).

1. Recordings available online through a secure website.

1. At the beginning of the contract, equipment shall require on-site training for VISN 10 VAMC personnel. Any equipment changes throughout the life of the contract shall also require training of VISN 10 VAMC personnel.

1. Two-part service: Event/arrhythmia monitoring for up to thirty (30) days with Holter capabilities and 24/48-hour holters.

1. Both patients and VA staff shall have unlimited 24-hours per day, 7 days per week (24/7) telephone access to contractor’s support staff that are knowledgeable and shall aid in resolving any monitoring problems.

1. Contractor shall have a Board-Certified Cardiologist and certified ECG technicians on staff, and available 24/7 to perform ECG interpretations. The Cardiologist shall be certified in reading and interpreting ECG data. Cardiologist shall digitally sign any final interpretations, where applicable to the specific VAMCs who will require the additional Cardiologist support.

1. Contractor shall transmit the data, analysis, and interpretation electronically to the appropriate VISN 10 VAMC for review within twenty-four (24) hours of receipt unless there is a potentially life-threatening arrhythmia and/or event.

1. In the event of a life-threatening arrhythmia and/or event, according to the American Heart Association guidelines, the contractor shall first call the affected patient to ensure actuality of the triggered event. If the event is, in fact, of life-threatening nature the contractor shall call 911 and dispatch emergency services to ensure the patient receives adequate treatment.

1. All events of an emergency or STAT nature shall immediately be phoned to the designated VISN 10 VAMC POC no later than one hour (1) after receipt and recognition of such data. Vendor will provide support based on VAMC Physician Notification Criteria as established by the facilities under the scope of this statement of work.

1. Contractor shall provide automatic shipments of devices according to pre-determined stock levels at each VAMC within the scope of this contract.

1. Additional monitors may be needed throughout the life of the contract during times of high usage. In such situations, the VISN 10 VAMC(s) will contact the contractor by phone, fax, or email to request additional monitors. The contractor will provide the requested monitors to the applicable VISN 10 VAMC(s) within five (5) business days. If the request is deemed “urgent” or “STAT” the contractor shall mail out the monitor within twenty-four (24) hours of online enrollment.

1. The contractor shall provide a detailed monthly report for all Veteran patients receiving full services to the facility COR (Contracting Officer Representative) at the end of the current billing period. Full services are defined as patients that have been identified by a VA provider as requiring monitoring services, have been assigned a monitor that has been returned to the contractor for interpretation, the interpretation has been reported back to the appropriate medical center, and the interpretation report is determined to be without technical error. The Inventory Report shall include, at minimum, the established monthly stock level of applicable devices, the number used within the reporting period, the name of the VA Medical Facility, the date range (i.e., May 1, 20xx – May 31, 20xx).

1. At the end of the patients monitoring period, the contractor shall provide a detailed summary report of the patient’s trends or ECG tracings within forty-eight (48) hours to designated VAMC POC. Types of arrhythmias can include, but are not limited to: premature ventricular contractions, atrial fibrillation, and paroxysmal supraventricular tachycardia. Report shall include arrhythmia summary, atrial fibrillation burden, bradycardia burden, tachycardia burden, symptoms, and event strips.

1. Contractor shall provide a summary report within forty-eight (48) hours of receipt of returned device.

2.4. INSTRUMENT SPECIFICATIONS:

a. Small and lightweight design.

b. Water-resistant (can be used in the shower, but should not be submerged under water – continuous wear)

c. Capability of event monitor transmissions being sent via wireless/cell phone or trans telephonic. technology. Please note that, according to the National Heart, Lung and Blood Institute, some monitors will need to be away from certain electrical devices, such as tablets, MP3 players, and cell phones. Contractor will need ensure these disclaimers, if applicable, are communicated to the patient and the VAMC POC.

d. Event Auto Trigger Technology.

e. Customizable monitors.

f. Two-part service: Event/arrhythmia monitoring for up to thirty (30) days with Holter capabilities and 24-hour or 48-hour holters depending on patient need.

g. Pacemaker pulse detection.

h. Instrument/Monitor has a date and time stamp for clinical reports.

i. Instrument/Monitor can correlate between time and symptom.

j. Holter monitors must provide at least two (2) continuous diagnostic channels, ECG for up to forty-eight (48) hours, set 2-channel pacemaker annotation, and the ability to correlate symptoms and activity with ECG changes.

k. Contractor shall supply equipment in good-working order. Equipment will arrive at the VAMC ECG lab or by delivery to patient, clean and in working condition.

l. Contractor equipment shall provide an adequate amount of batteries using standard size batteries readily available, and easily changed by the patient.

m. Up to four (4) hours of memory for devices that use cellular transmission.

n. The monitors shall be capable of monitoring heart rates between 30-250 BPM.

o. Event monitors shall be able to store at least 20 rhythm episodes, if unable to transmit, to include both pre and post symptom tracings.

p. Contractor shall distribute all necessary supplies, cleaned, and reset monitors and accessories in the monitor package to include instructions, batteries, electrodes, and patient diary.

q. Contractor-supplied monitors shall be issued with a postage-paid envelope for the return of the monitors to the Contractor or to the VAMC, if applicable.

2.5. TRAINING:

a. Contractor shall provide comprehensive training to each of the VISN 10 Medical Centers. This training shall be conducted at each facility, in person, upon the first ordering period of the contract, and during each option year (if exercised), as required by the sites. Each site may require multiple training sessions for numerous staff.

b. Training shall consist of proper use of the monitoring devices, instructions on how to upload patient information into contractor provided software and/or websites, and how to maintain or provide basic quality assurance tests.

c. Contractor shall provide training/information on contingency planning in the event the website is unavailable, reports cannot be pulled, event communications are delayed, and any other instance where issues can arise.

d. Contractor shall also provide each participant at each facility copies of manuals, equipment checklists, surveys, and specific contact information in the event that a medical center staff person needs to contact the contractor 24/7.

e. Any changes in the contractor’s personnel performing under the scope of this contract shall be communicated to the VISN 10 COR, as well as the individual VAMCs.

2.6. TECHNICAL SUPPORT:

a. Technical support is required via telephone for troubleshooting of method and/or problems. The response time for telephone technical assistance shall be within one (1) hour from the time of the initial call for technical support.
