B.2 STATEMENT OF WORK (SOW)

Maintenance and Repair of the Physical Access Control System (PACS) and Closed Circuit Television System (CCTV) I. SCOPE: The work shall consist of maintenance, repair and modifications of the DAQ Electronics Star Watch SMS PACS and the Vicon Valerus CCTV at the Department of Veterans Affairs Medical Center, Detroit, MI 48201.

II. PERSONNEL AND EQUIPMENT:

a. The contractor shall maintain qualified personnel, supervision, and equipment to accomplish work on schedule, and to the satisfaction of the Contracting Officer, as specified under this contract.

b. Employees and supervisors shall be capable and experienced in performing maintenance and repairs of the security system as specified in the contract. All technicians sent to site must be certified as a Starwatch SMS Front End Engineer as well as Vicon Valerus certification.

c. The contractor shall provide evidence of five years of experience and prior training on the DAQ Electronics Star Watch (formerly ICIDS) Security System and Vicon CCTV equipment. Certified technician must have at least five years documented experience on DAQ Electronics security systems as well as Vicon CCTV systems and is required to oversee the work performed.

d. Evidence of all prior experience and training, as well as documentation of the qualifications of personnel, shall be provided to the Contracting Officer prior to the award of the contract. The listing of prior experience shall include the names, addresses, telephone numbers and point of contacts of previous maintenance contracts for verification by the Contracting Officer.

e. Contractor shall NOT sub-contract the maintenance work unless the sub-contractor can also prove certification as a Starwatch SMS Front End Engineer and a certified Vicon Valerus technician.

III. SPECIFIC TASKS: The Contractor shall furnish all labor, supervision, tools, material, equipment, transportation, and management necessary to provide preventive maintenance, repairs and modifications for the existing PACS at the Department of Veterans Affairs Medical Center, Detroit, MI in accordance with the requirements specified in this performance work statement.

IV. EXTENT OF WORK:

a. The major components of the Physical Access Control System at VAMC Detroit include (some approximates):

i. One (1) Main Frame System Server

ii. Four (4) Poller Cards

iii. Three (3) Operator Workstations

iv. Twenty-Two (22) RADCs/Sub-RADCs

v. Forty-Nine (49) IACs

vi. Forty-Eight (48) Motion Detectors

vii. One Hundred Thirty (130) Door Switches

viii. One Hundred Two (102) HID Card Readers

ix. Twelve (12) Duress Alarms

x. Sixteen (16) 24 Hour Fire Doors

xi. Various keypads, sensors, Request to Exit Buttons, printers, data storage and operator interfaces.

xii. All associated cabling/wiring for system.

b. The major components of the Vicon Valerus System include: (some approximates):

i. Two Hundred Eighty-Five (285) Internet Protocol 2 MP cameras -both stationary and PTZ

ii. Fourteen (14) Network Video Recorders

iii. Six (6) Viewing Workstations

iv. One (1) Valerus Server

v. Forty-five (45) Vicon Media Convertors

vi. Multiple POE Switches

vii. Three (3) KVM Switches (one with attached monitor)

viii. One 65” Viewing Monitor

ix. Two 45” Viewing monitors

x. One Quad splitter

c. The PACS system and CCTV system components are located throughout the campus in building’s #100 (Medical Center), #101 (South Parking Structure), #102 (North Parking Structures), and #103 (Energy Center). A log of all events, troubles and alarms are kept at the Police Command Center in Building #100.

V. HOURS OF WORK:

a. The contract shall perform periodic maintenance, repairs and modifications on Monday through Friday between the hours of 7:00 am and 4:00 PM, except on Federal Holidays listed below. Approval to perform periodic maintenance and repairs on other than specified times must be approved by the COTR. The Federal Holidays are as follows:

i. New Years Day

ii. Martin Luther King Jr.’s Birthday

iii. Washington’s Birthday (President’s day)

iv. Memorial Day

v. Fourth of July (Independence Day)

vi. Labor Day

vii. Columbus Day

viii. Veterans Day

ix. Thanksgiving Day

x. Christmas Day

xi. Any other day deemed a federal holiday by the President of the United States.

b. In order to adequately complete the contract requirements, contractor must be on site at the Department of Veterans Affairs Medical Center, Detroit, MI, 48201, for a total of five days (40 hours) per quarter and a total of twenty days (160 hours) per year. Contractor shall also provide an additional 24-hour telephonic support of this contract. All on site visit dates shall be coordinated and approved by the COR.

VI. PERIODIC INSPECTION, TEST AND MAINTENANCE: All periodic inspections, tests and maintenance shall be performed in accordance with the manufacturer (DAQ Electronics Inc.) recommendations including:

a. Quarterly:

i. Inspection of PACS and CCTV system to ensure proper functionality. Review system reports and statuses to identify system errors or faulty components.

ii. Review with COR any known issues with systems and perform needed repairs. Contractor will work off of repair list provided by COR.

iii. Install any Government Furnished Equipment purchased for PACS or CCTV systems. Relocate existing PACS or CCTV Systems equipment as requested by VAMC.

iv. Provide assistance to the VAMC in designing and specifying equipment needed to expand the capabilities of the PACS or CCTV.

v. Provide any operator, systems administrator or maintenance training requested by the VAMC.

vi. Install any manufacturer provided software and firmware updates. Specifically, to include DAQ Software Assurance package, purchased yearly from DAQ Electronics and Valerus upgrades as provided by Vicon or Government

vii. Conduct performance testing on PACS and/or CCTV, when requested.

VII. RECORDS AND REPORTS: The contractor shall maintain management and maintenance records. All records and copies of reports shall be turned over to the COR. Reports must be turned in to COR within seven (7) working days of completion of visit.

VIII. SECURITY REQUIREMENTS:

a. No later than three days prior to the start of this contract, the contractor shall furnish the Contracting Officers Representative (COR) with a list of employees who will work on-site under this contract.

b. The COR will furnish VA ID Badges for each employees working under the contract and badges shall be worn at all times while on VA property. The contractor shall report lost ID badges within one working day of the loss to the VA Police Office.

c. At the conclusion of this contract or termination of any employees work contract, the contractor shall return the ID badges to the VA Police Office.

IX. SAFETY:

a. Compliance with Fire Prevention and Safety Regulations

b. The contractor shall, in the performance of this contract, comply with all VA Safety regulations, policies and procedures; as well as with recognized commercial safety practices and procedures.

c. The contractor shall provide all necessary equipment, tools and supplies to perform the work, and at no time will the contractor be permitted to use government equipment. Contractor will be allowed to store their equipment and tools on site between visits to avoid transporting items back to their office. VAMC will provide access to a man-lift to the contractor for the inspection and repair of pole mounted CCTV cameras in the parking structures.

C.2 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days of contract expiration.

C.3 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days of contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

E.1 ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS

1.0 OFFER SUBMISSION

Offerors shall submit their offer via email to Jeremy Boston at Jeremy.Boston@va.gov by the date and time identified on the SF1449, block 8. Offers shall be assembled by the following Sections; each section shall be easily identified.

Section A - Offer Form (Standard Form 1449) Signed and dated. Data Universal Numbering System (DUNS) Number shall be entered in the block with the Offerors name and address.

Section B – Pricing Pricing shall be submitted as requested in the Schedule of Services. Total price will be evaluated by the Government. Proposed price should reflect a clear understanding of the requirements and is consistent with the various elements of the offer.

Section C– Offerors Representations and Certifications A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at www.sam.gov/portal/public/SAM/.

All Offerors shall be registered and current in the System for Award Management (SAM) database at time of award.

Contractors claiming Service Disabled Veteran Owned Small Business or Veteran Owned Small Business size status are required to be CVE verified and listed in the Vendor Information Pages (VIP) upon the solicitation closing date/time. Verification instructions can be located at https://www.vip.vetbiz.va.gov/. This statement is only required for solicitations being issued with a socioeconomic set-aside for Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. See the SF1449 and/or Contract Clauses for set-aside information.

Section D – Technical Approach Offerors shall submit a quote that addresses all aspects of the Statement of Work (SOW). All required certifications shall be submitted as part of the offer.

The offeror shall identify any subcontractors proposed under this solicitation. All technical elements applicable to the utilization of subcontractors must be addressed in the technical quote and detail provided.

The technical approach section should be as detailed as necessary to fully explain the proposed methods and rationale; it should reflect a clear understanding of the nature of the work being undertaken. The technical quote shall include information on how the project is to be organized, staffed, and managed.

The Technical Volume (Section D) shall not exceed twenty-five (25) pages in length (minimum 12-point font and no less than ½ inch margins). Page limitation includes any drawings, charts, images, etc., and excludes section dividers, table of contents, glossary of terms, cross-referencing indices, and resumes. If more than 25 pages (not counting excluded pages) are submitted, only the first 25 pages will be considered.

Section E – Past Performance and Past Experience Identify all federal, state, and local government contracts as well as any private/commercial contracts of similar scope, size, complexity that are ongoing or have been completed within the last three years. List the following information for each contract:

· Company Name and Address

· Description of Services Performed

· Name, Telephone Number and Email Address of Responsible individuals who have first-hand knowledge of performance relative to similar type of services

· Dates of Contract Performance

· Contract Type (e.g. fixed-price, cost reimbursable) and total contract value

Failure to submit a complete offer may result in an unacceptable determination.

2.0 AWARD BASIS

Note that FAR Subpart 13.5 Simplified Acquisitions Procedures for Certain Commercial Items procedures will be used. The Government intends to award a firm fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this request for quote, will be most advantageous to the Government, price and other factors considered. An award based on FAR 13.106-2 will be made based upon the following factors:

· Price;

· Technical Capability; and

· Past Performance.

If any offer has a section that is determined unacceptable, the offer shall be considered unacceptable.

For evaluation purposes the Government will evaluate offers by adding the total price for all year to the total price for the base requirement. The Government may determine that an offer is unacceptable if the prices on subsequent years are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

As this procurement is being conducted under FAR Part 13, post-award debriefings in accordance with FAR 15.506 will not be conducted.

(1) A service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or

(2) Telecommunications equipment that cannot route or redirect user data traffic or permit visibility into any user data or packets that such equipment transmits or otherwise handles.

(c) Procedures. The Offeror shall review the list of excluded parties in the System for Award Management (SAM) (https://www.sam.gov) for entities excluded from receiving federal awards for “covered telecommunications equipment or services”.

(d) Representation. The Offeror represents that it [ ] will, [ ] will not provide covered telecommunications equipment or services to the Government in the performance of any contract, subcontract or other contractual instrument resulting from this solicitation.

(e) Disclosures. If the Offeror has represented in paragraph (d) of this provision that it “will” provide covered telecommunications equipment or services, the Offeror shall provide the following information as part of the offer—

(1) A description of all covered telecommunications equipment and services offered (include brand; model number, such as original equipment manufacturer (OEM) number, manufacturer part number, or wholesaler number; and item description, as applicable);

(2) Explanation of the proposed use of covered telecommunications equipment and services and any factors relevant to determining if such use would be permissible under the prohibition in paragraph (b) of this provision;

(3) For services, the entity providing the covered telecommunications services (include entity name, unique entity identifier, and Commercial and Government Entity (CAGE) code, if known); and

(4) For equipment, the entity that produced the covered telecommunications equipment (include entity name, unique entity identifier, CAGE code, and whether the entity was the OEM or a distributor, if known).

E.3 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS—COMMERCIAL ITEMS (MAR 2020) The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (v) of this provision.

(a) Definitions. As used in this provision— Covered telecommunications equipment or services has the meaning provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.

Economically disadvantaged women-owned small business (EDWOSB) concern means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business eligible under the WOSB Program.

Forced or indentured child labor means all work or service—

(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or

(2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties.

Highest-level owner means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.

Immediate owner means an entity, other than the offeror, that has direct control of the offeror. Indicators of control include, but are not limited to, one or more of the following: Ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.

Inverted domestic corporation means a foreign incorporated entity that meets the definition of an inverted domestic corporation under 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c).

Manufactured end product means any end product in product and service codes (PSCs) 1000-9999, except—

(1) PSC 5510, Lumber and Related Basic Wood Materials;

(2) Product or Service Group (PSG) 87, Agricultural Supplies;

(3) PSG 88, Live Animals;

(4) PSG 89, Subsistence;

(5) PSC 9410, Crude Grades of Plant Materials;

(6) PSC 9430, Miscellaneous Crude Animal Products, Inedible;

(7) PSC 9440, Miscellaneous Crude Agricultural and Forestry Products;

(8) PSC 9610, Ores;

(9) PSC 9620, Minerals, Natural and Synthetic; and

(10) PSC 9630, Additive Metal Materials.

Place of manufacture means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.

Predecessor means an entity that is replaced by a successor and includes any predecessors of the predecessor.
