B.3 PERFORMANCE WORK STATEMENT

A.1. Medication Assisted Therapy (MAT) Outpatient Addiction Services shall be furnished to beneficiaries for whom such care is specifically authorized by the BCVAMC. MAT may include induction and the use of Methadone or Buprenorphine as a replacement medication tool used in the treatment and recovery of opioid dependence to prevent withdrawal symptoms and opioid cravings while blocking the euphoric effects of opioid drugs. Methadone or Buprenorphine is used to stabilize the individual, so the treatment and recovery experience can be maximized. Methadone or Buprenorphine treatment services provide detoxification services to remediate the effects of opiate addiction as well as time-limited daily medication administration in the case of Methadone and periodic therapy intended to eliminate illicit opiate and other drug use and to wean persons from Methadone or Buprenorphine. To accomplish this, the service shall address major lifestyle, attitudinal and behavioral issues that have the potential to undermine the goals of treatment or to inhibit the individual's ability to cope with major life tasks without the non-medical use of alcohol or other drugs. Providers should be able to maintain and meet the needs of diverse populations. Outpatient Addiction services are designed to engage individuals in recovery which may result in, but not limited to: a reduction of maladaptive behaviors, maximize behavioral self-control, or restore normalized psychological functioning, reality orientation, re-motivation, and emotional adjustment, thus enabling improved functioning and more appropriate, interpersonal and social relationships. Outpatient Addiction therapy services may be provided to individual beneficiaries or to groups of beneficiaries for whom such care is authorized by the BCVAMC, 5500 Armstrong Road, Battle Creek, MI 49037.

A.2. Non-attendance at VA approved outpatient services, in excess of two (2) missed visits shall require notification to the Contracting Officer Representative (COR) by the Contractor. Veterans shall be advised regarding absence policy by facility, which shall require re-authorization by the COR. Continuation in the outpatient program shall be determined by the COR. The COR shall be advised within two (2) business days of Veteran withdrawal or termination.

A.3. The facility agrees to accept referral of and to provide all services specified in this contract, for any person determined eligible by the Chief, Medical Director or his designee, regardless of the race, color, religion, sex, or national origin of the person for whom such services are ordered. In addition, the facility warrants that subcontracting shall not be resorted to as a means of circumventing this requirement.

A.4. It is agreed that the VA may readily have access to all medical records concerning the Veteran's care. Upon discharge or death of the Veteran, medical records on all VA beneficiaries shall be provided to VA to maintain for a period of at least three (3) years following termination of care.

A.5. It is understood that the Veteran may be provided care at the expense of the VA for a period not in excess of that stated in the length of treatment contracted, unless an extension of the authorization is provided in writing by the VA.

In all cases, the VA requires a level of safety which conforms to NFPA 101. All contract facilities shall be under State or Federal authority.

A.8. Accreditation and licensing by the State: Accreditation by Commission on Accreditation of Rehabilitation Facilities (CARF) or Joint Commission on Accreditation of Healthcare Organizations (JCAHO) is evidence of internal quality assurance mechanisms and is desirable but is not required. Each record shall contain an assessment obtained from interviews with the Veteran or from the referring VA facility. Each Veteran shall have a level of care determination based on the current version of American Society of Addiction Medicine’s (ASAM) Veteran Placement Criteria that shall be supplied to the COR. Formal contact between the agency and the Veteran shall be documented in the record.
