Solicitation 36C25019Q0015 – Cleveland Perfusionist Questions (Q) and Answers (A)

Q.1 – Can you tell me if there is an incumbent currently providing Perfusionist Services at the Cleveland VAMC?

A.1 – Circulatory Services, Inc., is the incumbent currently providing perfusionist services.

Q.2 – What is the actual deadline: I have seen the following two dates. A: November 15 or November 30? Also what is the correct deadline time?

A.2 – Quotations are due 11/30/2018 at 2:00 pm local time as stated in Block 8 of the SF-1449 Q.3 - What are the exact hours for the Per Diem per year?

A.3 - There is no per diem for this contract. Pricing should be all inclusive for the perfusionists.

Q.4 - I also wanted to confirm that the hours are maximum 7280 including vacation and emergency coverage.

A.4 - Correct. See PWS 3.1.2 on page 19 of 93.

Q.5 – Are we going to submit the applicants shot record together with the proposal?

A.5 - Follow the instructions in accordance with the Addendum to FAR 52.212-1 Instructions to Offerors – Commercial Items beginning on page 69 of 93 for information required with the submission of quotations.

Q.6 – Do all perfusionists need an NPI number?

A.6 – Yes Q.7 - The cardiologists at Cleveland VAMC have a call room for duty. Will perfusionists have a call room available for call? i.e. – periods of bad weather, etc.

A.7 – The Cleveland VAMC has general call rooms at the hospital adjacent to the operating room suite that is available to any staff (including contracted staff) who may need them.

Q.8 – Do the perfusionists need ACLS Certification, or is BLS Certification acceptable?

A.8 - Both are required per PWS 2.1.6 on page 14 of 93, and 6.1.1 on page 30 of 93.

Q&A for Amendment 0002 Q.9 – RFQ Page 19, first paragraph: “Any hours worked during an on-call status for an emergency procedure shall be paid the same rate as regular tour hours.” How many hours of call coverage are perfusionists required to cover per month? On average, how many times per month are perfusionists actively called in to cover cases while on call?

A.9 – The amount of call hours is going to vary per month depending on patient needs. Last month was 45 PRN hours; however, other months may be much less.

Q.10 – RFQ Page 23. The Scope of Care lists many types of perfusion-related care. Do any other VA staff also perform some of these services, such as autotransfusion? If so, how many cases of these services are performed annually by VA staff and how many would be served by contractor staff?

A.10 – The Cleveland VAMC does not have anyone performing the main duties of a perfusionist.

Q.11 – RFQ page 70, under Part II – Technical Documents: “The name redacted copy shall not have any identifying marks indicating the company name, logo, watermark, etc.” To clarify, shall the quoter submit an entire set of technical documents without including its name in any form, including within the body of the responsive text?

A.11 – That is correct. For example, quoters can replace their company name with “NAME REDACTED” throughout the document. If also submitting a quote with a subcontractor, the quoter can state “PRIME NAME REDACTED” AND “SUB-CONTRACTOR NAME REDACTED” to differentiate between the companies.

Q.12 – What type of equipment is on site at the Cleveland VAMC?

A.12 -

· A.12 - 3 Cats cell savers

· 2 cardio help ECMO systems

· 3 sorin S 5 pumps

· Terromo custom packs to support the pumps

· GEM blood gas machine

Q&A for Amendment 0003 Q.13 - Hours are listed as FTE. Does this mean in the traditional definition of fixed salaries for perfusion services performed. If not what are the parameters for overtime A.13 - In the Schedule of Services (beginning on page 8 of the PWS), the unit cost per hour shall be all inclusive. On call services is considered part of the Perfusionist services and will not be paid separately. Any hours worked during an on-call status for an emergency procedure shall be paid the same rate as regular tour hours. See PWS 3. Hours of Operation beginning on page 19.

Q.14 - Are ECMO hours paid in addition to salary or included. It is standard in the industry for ECMO hours by a perfusionist to paid at an hourly rate additional to set salary.

A.14 – As stated in A.13, in the Schedule of Services (beginning on page 8 of the PWS), the unit cost per hour shall be all inclusive. ECMO hours are dedicated to the PRN perfusionist position. See PWS 3. Hours of Operation beginning on page 19, which states: The PRN Perfusionist will be available on an as needed basis to handle overflow and ECMO.

Q.15 - Even though there are two call rooms they are used by Surgical residents and interns who are required to stay on site.Perfusion has no access to those rooms because they do not have enough for the residents. As a result, we have a thirty to forty min call response time from home.

A.15 - The Cleveland VAMC has general call rooms at the hospital adjacent to the operating room suite that is available to any staff (including contracted staff) who may need them. Even if the rooms are unavailable, in accordance with the solicitation a 30-minute emergency response time is required.

Q.16 - The call hours as answered in question 9 for amendment 0002 is incorrect. There are three of us. One of us takes call the entire week including Sat. and Sun. You must be in the call radius of 30 to 40 min. This includes the weekend, so the call person can not leave town. The 24/7 the call hours equal around 84 hours a week on a rotational basis.

A.16 - The requirements for the resultant contract are not exactly the same as in the current contract. In the Schedule of Services (beginning on page 8 of the PWS), the unit cost per hour shall be all inclusive. On call services is considered part of the Perfusionist services and will not be paid separately. Any hours worked during an on-call status for an emergency procedure shall be paid the same rate as regular tour hours. See PWS 3. Hours of Operation beginning on page 19.

The answer to question 9 (Q.9 on Amendment 0002) is correct.

Q.17 - The PRN person is not call hours.PRN is used for vacation relief, meetings, medical emergencies to keep our staffing at three Perfusionist. They will be used for ECMO when extra hands are needed A.17 - The expectation is for 1 or 2 Perfusionists to be on duty to handle scheduled surgeries. The third will be on-call to handle emergencies, vacations and sick calls. PRN Perfusionist will be available on an as needed basis to handle over flow and ECMO. See PWS 3. Hours of Operation beginning on page 19.

Q.18 - The contract has hours of scheduled heart surgery three days a week. We have been informed by our Chief of Cardiothorasic Surgery,a third full tlme surgeon will start December 15th. He now anticipates doing two hearts a day five days a week.

A.18 - In accordance with the PWS under 3. Hours of Operation, the regular work hours for the resultant contract are Monday, Tuesday, Wednesday, and Friday.
