B.5 PERFORMANCE BASED STATEMENT OF WORK
A. PURPOSE.
The purpose of this contract is for the Chillicothe Veterans Administration Medical Center (VA) to engage a community provider (Contractor) who will provide emergency housing to Veteran families along with services consistent with Health Care for Homeless Veterans (HCHV) program objectives. The goal of this contract is to remove homeless Veteran families from the street or places unfit for human habitation and place them in community-based, residential environments. These residences must have sufficient services to meet the needs of the Veteran families served. The contract will be for one year with three option years. Awards may be based on residential capacity to serve Veteran families for up to 1,825 nights per contract year.
Special emphasis on privacy and security must be noted in mixed gender facilities. The living environments of HCHV Contract housing must include:
(1) Separate and secure sleeping arrangements (unit or wing) for each gender,
(2) Separate and secure bathroom arrangements for each gender,
(3) Screening procedures and criteria for sex offenders (mixed gender facility), and
(4) Common areas (lounges, laundry rooms, group rooms) with appropriate security arrangements.
Additionally, mixed gender facilities that share entrance and common areas (living room, kitchen, dayroom, etc.) must include onsite staffing available 24 hours a day, 7 days per week.
B. BACKGROUND.
Through its HCHV program, Chillicothe VAMC provides case management and outreach services to Veterans and facilitates their access to a broad range of medical, mental health, and rehabilitative services. HCHV’s goal is to engage homeless Veterans in treatment and place them in a safe and secure environment that supports their rehabilitation goals.
Chillicothe VAMC’s HCHV Program, an essential and critical part of VHA, is vital for providing a gateway to VA and community supportive services for eligible Veterans who are homeless. The HCHV Contract Residential Program for homeless Veterans is a vital program focused on addressing the needs of the most vulnerable and at-risk homeless Veterans. Ensuring that Veterans can be placed in community-based programs which provide quality housing and services that meet the needs of this special population is a particular focus of the HCHV program.
C. TASKS AND ASSOCIATED DELIVERABLES.
The Contractor will admit Veteran families to residential beds within 24 hours of agreed upon admission date between Contractor and Chillicothe VAMC Liaison. Specific tasks are as follows:
Task 1: Housing Available for Rapid Placement
Contractor will be responsible for identifying sufficient residential capacity to place Veteran families in safe, community-based residences. Identification of suitable residential placement must require:
a) Sufficient personnel to assure security
b) Staffing available 24 hours a day, 7 days per week. (Requirements and rates will be evaluated based on geographic location and types of services provided by the contractor)
c) Access to 3 healthy meals per day
d) Available laundry facilities and supplies
e) The ability to place a Veteran family on the same day of a referral from HCHV staff.
f) Room and Board to include beds and other furnishings such as a dresser, storage, and linen for the beds of appropriate quantity to meet the need of Veteran families.
g) The facility shall be in close proximity to public transportation.
h) Facility must be operational within 4 weeks of award notification
Contractor will be responsible for planning and coordinating all activities associated with providing safe and appropriate residential care. This includes, but is not limited to, the elements described in the preceding paragraph.
During the screening process, HCHV program staff will determine eligibility and the contracted provider will evaluate the suitability and appropriateness of the level of care. When a Veteran is determined to be ineligible or if the level of care is not appropriate, the HCHV program staff will collaborate with the Veteran and the contracted provider to make every effort to arrange for provision of care through other VA or community resources. Every effort will also be made to minimize access barriers into the program.
Veterans cannot be denied entry to HCHV Contracted Residential Services based solely upon length of current abstinence from alcohol or non-prescribed controlled substances, the number of previous treatment episodes, the time interval since the last program entry, the use of prescribed controlled substances, or legal history. The screening process must consider each of these special circumstances and determine whether the program can meet the individual Veteran's needs while maintaining the program's safety, security, and integrity.
Task 1 Deliverables:
1. The identification of an exact number of residential units specific to the needs of the HCHV program and their addresses. Contractor will be responsible for providing VA a written summary of size, location, and condition of the available residential units.
Task 2: Delivery of Support Services
Contractor must show documentation and provide supportive services to Veteran families that will provide assistance to them during their contract residential stay. These services will continue throughout their authorized stay, generally not to exceed 90 days. The Contractor will determine therapeutic, rehabilitative, and recovery services needed by the Veteran family resident in a plan developed by the Contractor with consultation by the Veteran resident and appropriate VA staff. The evaluation factors for delivery of supportive services are listed below:
a) Ability to provide structured group activities, as appropriate – examples include group therapy, social skills training, Alcoholics Anonymous, Narcotics Anonymous, vocational counseling and physical activities as appropriate;
b) Ability to collaborate with VA program staff, as appropriate.
c) Contractor must ensure individual counseling is being provided (either through direct service or referral)
d) Ability to provide assistance with health education and personal hygiene, as appropriate.

i) The ability to assess and respond to a Veteran and/or family member in crisis and make appropriate referrals for assistance;
k) The ability and willingness to provide services to Veterans with previous history of criminal offenses;
l) The Contractor must provide services to families with the understanding that only Veterans will be reimbursed for services.
m) Contractor will provide a detailed staffing and service plan.
Task 3: Status Reports
Contractor shall be responsible for billing the invoice that identifies Veterans and dates of service. Contractors will have an expectation for maintaining their own records related to program performance that may be reviewed as part of the performance monitoring, but there is no standard practice for this. The Contractor will be monitored by the performance measures in the Quality Assurance Surveillance Plan (QASP) in Section D.
Task 3 Deliverables:
1. The Contractor shall make available to the VA, documentation deemed necessary by the VA to conduct utilization review audits for the mandated national evaluation study as required by Section 2 of Public Law 100-6-32; to verify quality of patient care for Veterans, to assure confidentiality of care for Veterans, to assure confidentiality of patient record information, and to determine the completeness and accuracy of financial records. The Contractor shall comply with the VA Patient’s Bill of Rights as set forth in Section 17.34a, Title 38, Code of Federal Regulations.
2. Provider will maintain a daily census and provide any required discharge forms within 72 hours.
3. Data entered must include, at a minimum, the Universal Data Elements from the 2014 HMIS Data Standards.
4. Individualized Service Plans will be required to be completed with each Veteran family placed in residential services within two (2) weeks of admission. Individualized treatment plans are developed through a joint effort of the Veteran, the contracted provider, and the HCHV program staff. Treatment plans must be developed for each Veteran based on input from HCHV program staff assessments, other VA clinical data, and the Veteran.
5. Contractor is expected to provide HCHV staff with regular reports on changes in Veteran family status, including all changes and observations concerning needs.
D. DELIVERABLE TIMETABLE.
A contract is formed in accordance when the parties agree to be bound by the IDIQ. Upon acceptance by the contractor of beneficiaries of the Department of Veterans Affairs (VA), all terms and conditions of this contract shall apply during such time as a VA patient remains in that facility at the expense of the VA.

To remain registered in the SAM database after the initial registration, the Contractor is required to review and update on an annual basis from the date of initial registration or subsequent updates its information in the SAM database to ensure it is current, accurate and complete. Updating information in the SAM does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document.
(2)(i) If a Contractor has legally changed its business name, "doing business as" name, or division name (whichever is shown on the contract), or has transferred the assets used in performing the contract, but has not completed the necessary requirements regarding novation and change-of-name agreements in FAR subpart 42.12, the Contractor shall provide the responsible Contracting Officer a minimum of one business day's written notification of its intention to (A) change the name in the SAM database; (B) comply with the requirements of subpart 42.12; and (C) agree in writing to the timeline and procedures specified by the responsible Contracting Officer. (i) Any such clause is unenforceable against the Government.
(ii) Neither the Government nor any Government authorized end user shall be deemed to have agreed to such clause by virtue of it appearing in the EULA, TOS, or similar legal instrument or agreement. If the EULA, TOS, or similar legal instrument or agreement is invoked through an “I agree” click box or other comparable mechanism (e.g., “click-wrap” or “browse-wrap” agreements), execution does not bind the Government or any Government authorized end user to such clause.
(iii) Any such clause is deemed to be stricken from the EULA, TOS, or similar legal instrument or agreement.
