Background: Building 1A experienced a failure within the preheat system piping. An inspection revealed missing metal at the top of the flange on one of the preheat system/heat exchangers resulting in a leak of approximately five (5) gallons of a glycol-water mixture down the floor drain. Assessment confirmed that the heat exchanger flange had failed.

The units in Bldg.1A effected are AHU-1(SPD), AHU-4 (Nuclear Med), AHU-5(Audiology), AHU-7 SE Quadrant 2nd Floor This would be Zone D / G units. This also impacts AHU-18 and AHU-19 which serves the main bed tower entrance.

Other failed heat exchangers are in 6018, and we have also located failing valves in BE208. The facility is in need of repairs to its heating system. The assessment of our heating system includes replacing heat exchangers, check valves, steam traps, vacuum breakers, gate valves, actuator valve assemblies, and the re-insulation of repaired piping system.
