1. In section 1.2 of the PWS, the Government states as follows:
“PLACE OF PERFORMANCE: The contractor shall provide a primary CBOC facility in Helena (Phillips County), AR and within 10 miles of a local hospital providing emergency room services.”
On page 168 of the Solicitation, the Government states as follows:
“Preference will be given to a single-level building structure, and preference will be given to a facility that is located within 10 miles of a hospital with emergency room facilities.”
Is it required for the CBOC facility to be within 10 miles of a local hospital providing emergency room services, or is it a preference during the evaluation?
Answer: Yes, it is required for the CBOC facility to be within 10 miles of a local hospital providing emergency room services.
Page 168, Subfactor B, is hereby revised to state the following:
This sub-factor will be used to determine that the Offeror’s facility is accessible by patients with and without mobility issues, and that access is consistent with Federal laws and regulations and provides for easy access to the clinic. The Offeror shall demonstrate easy access to the clinic for patients with mobility issues as well as those patients who do not have mobility issues. Proposed clinic site for Helena CBOC must be physically located in the city of Helena, Arkansas and located within 10 miles of a hospital with emergency room facilities. (PWS Reference: 1.2) The clinic must be devoted exclusively to the service of VA patients in one location. Preference will be given to a single-level building structure. PWS Reference (1.2)
2. In section 2.2. of the PWS, the Government states as follows:
“PHYSICIAN DIRECTOR (MANDATORY FOR ALL SITES): FTE Ratio Performance Standard: 0.05* FTE per PACT Responsible Party: Shall be provided by Contractor.”
In section 2.3.1. of the PWS, the Government states as follows:
“At least one of the PCPs is to be a 1 FTE physician.”
Can the FTE PCP Physician also serve as the .05 FTE Physician Director?
Answer: Yes; however, the 0.05 FTE time must be dedicated as Physician Director, and the panel size would have to be adjusted appropriately.
3. In section 2.12 of the PWS, the Government states as follows:
"TRAINING (ACLS/BLS/VA MANDATORY): Contractor staff shall complete VA mandatory training as requested and complete ACLS/BLS training and keep ACLS/BLS certifications current throughout the life of the contract. Certification refers to the successful completion of an American Heart Association (AHA) or Military Training Network (MTN) approved BLS or ACLS course. The AHA and MTN are the only programs acceptable by VA for obtaining required certification. Copies of current certifications shall be provided to the COR. ACLS certification is required for health care personnel that order, administer, monitor, or supervise moderate sedation, monitored anesthesia care, or general anesthesia, according to current VHA policy. The Contractor shall coordinate, and cover contract staff travel expenses in accordance with General Services Administration (GSA) rates."
Ordinarily only a BLS is required for staff, with the exception potentially of the Physician.
In particular, it would be highly unusual for an MSA, LCSW, and Dietician to perform ACLS.
Would the Government clarify if an ACLS and BLS certification is required for all administrative and clinical staff?
Answer: only BLS or ACLS ( can be one or the other) is required for all clinical staff. It is not required for administrative staff.
4. In section 4.6.3.4.3. of the PWS, the Government states as follows:
“All outpatient sites of care must have at least two WH-PCPs. (Because of small populations of women at most CBOCs, CBOC WH-PCPs will usually have mixed gender panels).”
According to the Price Schedule, the highest estimated quantity of patients requiring Primary Care Services is 966 patients in the base year.
This number is significantly below the 1200 capacity of patients for 1 FTE Physician. 1 FTE Physician is required by section 2.3.1. of the PWS.
Would the Government consider allowing one WH-PCP working as an FTE and 1 WH-PCP working as a back-up to fulfill the requirement for two WH-PCPs given the size of the Helena CBOC?
Answer: Acceptable, as long as backup is available.
5. On page 158 of the Solicitation, the Government states the following:
“Proposals must be sent by mail (USPS, FedEx, UPS, DHL, or other carrier) to the Contract Specialist and address listed below no later than August 31, 2023 at 10:00AM Central Time (CT).”
The deadline for the Solicitation listed on page 1 is listed as September 14, 2023 at 10:00 AM CT.
What is the current deadline for proposals?
Answer: The solicitation close date / proposal due date is September 14, 2023 at 10:00 AM CT.
Page 158, Instructions to Offerors, is hereby revised to state the following:
The sole point of contact for this acquisition is the Contract Specialist. Proposals must be sent by mail (USPS, FedEx, UPS, DHL, or other carrier) to the Contract Specialist and address listed below no later than September 14, 2023 at 10:00AM Central Time (CT). Proposals may be hand carried and delivered to the mailing address below. Faxed or emailed proposals will NOT be accepted. It is suggested, but not required, that the Offeror use a shipping method that allows tracking of the proposal to ensure delivery. Do not require a signature for receipt of packages to ensure proposals are not returned.
6. On page 161 of the Solicitation, the Government states the following:
“Use font Times New Roman or Arial, with a font size no smaller than 11 point.”
Will tables, graphs, and labels be permitted to be small than the font size limits?
Answer: Yes, tables, graphs, and labels may be smaller than 11 point.
7. On page 159 of the Solicitation, the Government states the following:
“The Offeror is instructed to not include its name or any Offeror specific identifying information anywhere in Volume II of its proposal.”
On page 161 of the Solicitation, the Government states the following:
“Each page should include a header that identifies the RFP number and name, and the Offeror (abbreviations are acceptable to keep the Header to a single line); and a footer that identifies the page number (using the numbering style: page X of XX pages), and a statement: “Confidential Proposal Documents.”
The two requirements shown above demonstrate contradictory instructions on whether the Offeror should include Offeror specific information in the proposal. Does the Government want the Offeror to include Offeror specific identifying information in Volume II of the proposal?
Answer: Page 161, Instructions to Offerors, paragraph (4)(c)(v) is hereby revised to state the following:
Each page should include a header that identifies the RFP number and name, and the assigned Offeror number, and a footer that identifies the page number (using the numbering style: page X of XX pages), and a statement: “Confidential Proposal Documents.”
8. On page 159 of the Solicitation, the Government states the following:
“The Offeror is instructed to not include its name or any Offeror specific identifying information anywhere in Volume II of its proposal.”
In the event that the Offeror provides resumes in support of its proposal, and those resumes are included as an attachment to Volume II, may Offeror names appear on the Resumes if the Offeror was an employer?
Answer: Yes that is acceptable.
9. On page 159 of the Solicitation, the Government states the following:
“The Offeror is instructed to not include its name or any Offeror specific identifying information anywhere in Volume II of its proposal.”
On page 167 of the Solicitation, the Government states the following:
“Offeror shall submit at least three (3) references for current/prior Outpatient Clinic and Healthcare Management contracts that are similar in size and scope to this requirement, including any Federal, State, or local government, and commercial contracts.”
Do past performance references not count as Offeror specific identifying information?
Answer: The Past Performance Questionnaires and references will be evaluated by the Contract Specialist only; therefore, there is no need to conceal the Offeror information for past performance references.
10. On page 157 of the Solicitation, the Government states the following:
“Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the Offeror specifies otherwise in the offer.”
The two items required by the schedule are Primary Care Services at the Helena CBOC and Mental Health Services at the Helena CBOC.
Could the Government accept Primary Care Services from one Offeror and Mental Health Services from a different Offeror which proposed a different location for the Helena CBOC?
Answer: The Government does not intent to award multiple contracts for this requirement.
11. In section 4.6.5.3. of the PWS, the Government states the following:
“LABORATORY SERVICES: Contractor is responsible for: 1) Entering orders for laboratory tests into the VA EHR. Information concerning the laboratory tests are provided as an attachment in Section D. 2) Sending specimens to the VA Core Laboratory twice daily, prior to the Contractor’s mid-day break period and after the close of business of the workday, except for those specified in this PWS. 3) Paying any costs of all lab work, except for lab work sent to the VA or emergency lab work sent to another site which has been authorized by the VA Communications Center and paying any costs associated with transportation of specimens to the VA and for arranging such transportation in a proper secure method and 4) ensuring that all courier service employees have completed VHA Privacy and HIPAA Focused Training 5) Ensuring the proper collection, collection supplies, and other preservation of specimens and providing appropriate specimen collection containers that are compatible with the instrumentation and methodology used by the VA laboratory.”
It is our understanding that the VA currently provides the Lab courier from the CBOC to the VA. Will the VA no longer be providing this service?
Answer: The Memphis VAMC will be providing lab courier services from the CBOC to the VAMC.
PWS Paragraph 4.6.5.3 is hereby amended as follows:
LABORATORY SERVICES: Contractor is responsible for: 1) Entering orders for laboratory tests into the VA EHR. Information concerning the laboratory tests are provided as an attachment in Section D. 2) Sending specimens to the VA Core Laboratory twice daily, prior to the Contractor’s mid-day break period and after the close of business of the workday, except for those specified in this PWS. 3) Paying any costs of all lab work, except for lab work sent to the VA or emergency lab work sent to another site which has been authorized by the VA Communications Center and paying any costs associated with transportation of specimens to the VA and for arranging such transportation in a proper secure method and 4) ensuring that all courier service employees have completed VHA Privacy and HIPAA Focused Training 5) Ensuring the proper collection, collection supplies, and other preservation of specimens and providing appropriate specimen collection containers that are compatible with the instrumentation and methodology used by the VA laboratory.
12. In section 4.9.2. of the PWS, the Government states the following:
“There will be one (1) teamwork room that combined will accommodate a total of twenty (20) personnel. Teamwork room is to be located adjacent to the exam rooms. See PACT Design Guide for additional requirements.”
Will the VA allow the Teamwork area to be adjusted to accommodate the contractors proposed staffing if it is less than 20 personnel?
Answer: Yes, the VA will allow the Teamwork area to be adjusted, as long as the Teamwork area is sufficient to accommodate all Primary Care, Mental Health, and administrative staff.
13. Does the VA require sliding doors on the patient side or both sides of the exam rooms?
Answer: The VA has no preference.
14. Will the VA accept existing hallways that are 5 feet wide and existing doors that are 36 inches wide?
15. Will the Women’s health exam room count as one of the required 3 exam rooms per Teamlet?
Answer: Yes.
16. Section 4.3.4 of the PACT Space Module Design Guide, the Government states the following:
“Use of surface mounted sliding doors maximizes floor space and is strongly encouraged.”
As shown above, the PACT Space Module Design Guide identifies the possible use of sliding doors, but does not require them. Does the VA have a preference for the use of sliding doors in the Helena CBOC? If so, would the Government please confirm if sliding doors are preferred on the patient side of each exam room or on both sides of each exam room.
Answer: The VA has no preference.
17. In section 4.6.5.1 of the PWS, the Government states the following:
“RADIOLOGY SERVICES - The contractor shall provide by way of contract through a local, accredited, and certified facility at the contractor’s expense for, general x-rays, diagnostic radiographs and mammography services (screening mammography, diagnostic mammography and breast ultrasound).”
Does the VA have specific equipment requirements or guidelines for the X-Ray equipment that will be furnished at the local facility described in the Solicitation for Radiology Services?
Answer: The VA does not have specific equipment requirements.
18. Are there any additional space requirements other than those listed in section 4.9 of the PWS?
Answer: No.
19. In order to make delivery of a new clinic site as cost effective as possible, while delivering optimal care, would the VA allow for five-foot hallways, as compared to six foot hallways as most buildings in the area utilize five foot commercial hallways?
Answer: No exceptions will be allowed for the hallway sizes listed in the solicitation.
20. In order to make delivery of a new clinic site as cost effective as possible, while delivering optimal care, would the VA allow for 42 inch doorways, as compared to 36 inch doorways as most buildings in the area utilize 36 inch commercial doorways?
Answer: No exceptions will be allowed for the doorway sizes listed in the solicitation.
21. In section 4.6.4.1. of the PWS, the Government states the following:
"Prescription Fulfillment- The Contractor will provide all medications that are to be administered to patients in the clinic (including any necessary vaccines). Routine prescriptions will be dispensed by the VA and mailed via VA Centralized Mail Out Pharmacy (CMOP) to the Veteran following appropriate Contractor’s provider order entry in the VA EHR. The VA will review all submitted non-formulary restricted, and prior authorization medication consults in a timely manner in accordance with VHA Directive 1108.08(1). VA Pharmacy Service will require Contractor clinic management or designee to conduct routine inspections of medication storage areas monthly and provide documentation per local VA Pharmacy policy to the COR. Urgent/Emergent medications needed will be filled via contracted local pharmacy for up to 10 days. To be added as an authorized provider to use this contract, providers must e-mail their DEA and NPI number, full name, and clinic location to Timmye Edwards at Timmye.Edwards@va.gov and Christy Wright at Christy.Wright3@va.gov. Prescriptions written for filling via a contracted local pharmacy, must also be documented as non-VA medication in the EHR. All other prescribed medications will be via the VA EHR, faxed, or mailed to VA pharmacy for filling. The Contractor must abide by all Joint Commission (or equivalent accreditation body) and VA policy on the storage, security, handling and disposal of all medications held in their clinic and comply with all monthly clinic inspections and the recommendations generated from those inspections.”
Based on the foregoing, would the Government please provide a list of medications that have been prescribed at the Helena CBOC and filled at a local pharmacy?
