DESCRIPTION
On behalf of the Memphis VA Medical Center, Network Contracting Office (NCO) 9 is issuing this Sources Sought Notice (SSN) as a means of conducting market research to identify parties having an interest in and the resources to support this requirement for a firm-fixed-priced Replacement Flooring Contract for the Memphis VA Medical Center facility specified offices listed below. The result of this market research will contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 238330, Flooring Contractors.
1. THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for bids, proposals, or quotes; submission of any information in response to this market survey is purely voluntary; and the government assumes no financial responsibility for any costs incurred.
2. If your organization has the potential capacity to perform these services, please provide the following information:
a. Organization name, address, point of contact, email address, web site address, and telephone number.
b. Business size: small, small disadvantaged, woman-owned, Hub Zone, serviced-disabled veteran-owned small business (SDVOSB), veteran owned small business (VOSB), large business, etc.
c. Business type under NAICS 238330, which contains a size standard of 19.0 million dollars, Commercial and Government Entity (CAGE) Code and System for Award Management (SAM) Unique Entity ID (UEI) number.
d. Tailored capability statements addressing the particulars of this effort, with appropriate documentation supporting claims of organizational, and staff capability.
e. Responses to this SSN shall not exceed 4 pages and should include all the foremost mentioned information. In addition, all submissions should be provided electronically in a Microsoft Word or Adobe Portable Document Format (PDF).
f. The government will evaluate market information to ascertain potential market capacity to:
i. Provide services consistent in scope and scale with those described in this notice and otherwise anticipated.
ii. Implement a successful plan that includes contractor compliance with program standards, quality control, and performance.
iii. Provide services under a firm-fixed-price contract.
3. NOTE: Respondent claiming SDVOSB and VOSB status shall be registered, and Center for Veterans Enterprise (CVE) verified in VetBiz Registry https://veterans.certify.sba.gov.
4. Based on the responses to this SSN/market research, this requirement may be set-aside for SDVOSBs, VOSBs, small businesses, procured through full and open competition, or sole sourced.
5. Submission Instructions: Interested parties who consider themselves qualified to perform the services are invited to submit a response to this SSN by 10:00 AM CST, May 25, 2023. All responses under this SSN must be emailed to Alexis.Bowen@va.gov. Telephone inquiries will not be accepted or acknowledged, and no feedback or evaluations will be provided to companies regarding their submissions.
6. SAM: Interested parties shall be register in the System for Award Management (SAM) as prescribed in Federal Acquisition Regulation Clause 52.232-33. SAM information can be obtained by accessing the internet at www.sam.gov or by calling 1-866-606-8220.
7. Disclaimer and Important Notes. This notice does not obligate the government to award a contract or otherwise pay for the information provided in response to this SSN. The government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the government to determine the organization’s qualifications to perform the work. Respondents are advised that the government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation or combined solicitation may be published in Contract Opportunities (formerly Federal Business Opportunities or FBO). However, responses to this notice shall not be considered adequate responses to a solicitation.
8. Confidentiality. No proprietary, classified, confidential, or sensitive information should be included in your response. The government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).

PERFORMANCE WORK STATEMENT
Maintenance and Repair Service:
To Replace damaged flooring in various Locations
Lt. Col Luke Weathers Jr. MEMPHIS VA MEDICAL CENTER, MEMPHIS, TN 38104
I. INTRODUCTION – This maintenance and repair (M&R) service includes the replacement of damaged flooring, such carpet and vinyl tile, in various locations as identified in Table 1 contain here in.
1. Title of Project: Replace damaged Flooring in various locations
2. Scope of Work: The contractor shall provide all labor, supplies, tools and/or equipment required to remove damaged flooring, carpet, vinyl tile, and cove base; install new government-furnished vinyl flooring and cove base. The contractor shall provide floor patch as required to ensure smooth/level subfloor before installing new flooring.
The service required includes:
1. Removal of existing flooring and cove base in the rooms/spaces identified in Table 1.
2. Prep/patch subfloor to ensure smoothness and leveling, prior to installation of new flooring.
3. Install new government-furnished vinyl flooring, cove base and adhesive(s).
4. Repair damage to any walls, floorings and/or surrounding surfaces caused by the contractor’s personnel and/or sub-contractor(s).
5. This work shall be performed after hours to prevent delay in patient care when possible

PERFORMACE:
The period of performance is twelve weeks with a completion date of no later than August 31,2023 Note: Contractor price does not include taxes. The Contractor base price on performing this scope of work during our normal business hours of 7:30am till 4:30pm Monday through Friday except for holiday when available and After hours 4:30pm – 07am Monday through Friday
The Carpenter Shop Supervisor and the Maintenance & Repair Supervisor will issue necessary keys/cards to the contractor. The keys/cards issued are to remain government property. The contractor or his representative will be required to sign in receipt of any keys/cards issued and is responsible for returning them to the Facility Management Engineer.
a. At the end of the flooring repair event, the contractor shall notify that the room is complete to the COR or government rep onsite for signature. This will document what cover items to include but not limited to the time, date, and room clean after the work is complete. This will correspond to invoice received for payment though the IPPS System.
b. Contractor and their staff are not permitted to take any pictures, videos, or drawings of any type. Taking of photographs/videos are prohibited while on VA premises.
c. The contractor’s staff will check in with the VA police to receive a VA work authorization badge. The badge must be worn facing forward and above the waistline. The contractor’s staff are required to wear the VA always issued badge while on the VA premises. The contractor will deliver the services outline in this Statement of Work.
Page 1 of Page 1 of Work Hours and Period of Performance - VAMC Memphis normal working hours are Monday through Friday 0700-1600 (with one half hour for lunch), excluding federal holidays. All flooring service shall be performed after normal working hours unless the Contracting Officer’s Representative (COR) provides written consent to the Vendor. The facility observes the following federal holidays:

Period of Performance:
The period of performance shall be from time of award plus 12 week estimated completion of services not to exceed August 31, 2023.
The contractor is required to follow all regulations and safety requirements of the VA such as OSHA, NFPA, EPA, etc. Appropriate PPE must be worn always as well.
Contractor and their staff are not permitted to take any pictures, videos, or drawings of any type. Taking of photographs/videos are prohibited while on VA premises.
