X X X February 3, 2022 4:00 pm Central The purpose of this amendment is to correct the on-call hours on Page 27 of the solicitation in section 3.1.3. of the Performance Work Statement and to answer potential offeror questions.

The due date for offers is February 3, 2022.

B.4 Performance Work Statement Section 3.1.3. is hereby changed to state:

On-Call Coverage: Contractor must make the Contractor’s physician(s) available on-call during all hours when the James H. Quillen VAMC’s Urology clinic is closed, including evenings, weekends, and holidays, during the physician(s) assigned work week. On-call hours are from 4:00 p.m. to 7:00 a.m. on week days and from 4:00 p.m. on Friday until 7:00 a.m. on Monday for weekends not associated with a holiday. Holiday on-call will be for the on-call day only and will begin at the end of the clinic day or regular on call day.

QUESTIONS AND ANSWERS

1. Question: The solicitation lists a contract max of $752,000, if we identify a candidate who’s rate exceeds this can we submit? Will the service allow for a higher max on the contract?

Answer: All unit prices must be all inclusive firm-fixed prices including per diem, housing, and costs of all required security clearances. Potential offerors should propose rates as appropriate. The contract maximum is in correlation to Federal Acquisition Regulation clause 52.216-19 Order Limitations which sets a maximum the contract cannot exceed.

2. Question: Can the Physician do the TMS trainings during their orientation so they are paid for their time?

Answer: Yes

3. Question: Can we have multiple Urologists fill the schedule? (ex. 7 on/off rotation between 2 Providers) Answer: Yes, that is fine as long as one provider is here for a minimum of a week at a time. We would prefer not to have 12 different providers coming over 12 different weeks, but the same providers over a period of time (ie. provider A week one and Provider B week 2 with Provider A coming back week 3, etc.)

4. Question: Can you confirm you will except Board Eligible Urologists as well? (page 19, 2.1.2 states must be Board Certified) Answer: Must be board eligible or board certified

5. Question: Under section 2.7 (no employee status:… ) the Urologists are 1099 contractors and not employees, can this section be removed?

Answer: No, this section cannot be removed.

6. Question: How many other Urologists on staff?

Answer: 2.625 FTE (2 Full time; three part time)

7. Question: What is the call schedule? (from the pricing sheet it looks like they will be on call the majority of the time) Answer: When on-call, 4 p.m. to 7 a.m. during week days, and all day during Holidays and weekends. The Locums provider will be required to take on-call 2 weekends and 10 week days

8. Question: How often will they be going over to the Community living center? 4.4.2.1.2 on page 30 Answer: As needed for consults

9. Question: On average, how many calls does the Urologist receive during the weeknights? Weekends?

Answer: One to five on week nights; one to five on weekends. Most of these do not require the provider to come on site.

10. Question: On average, how often is the Urologist called back in on weeknights? Weekends?

Answer: One out of every four weekends of call; one to two nights a month for week nights

11. Question: What specialty Physician support is available?

Answer: General Surgery, Vascular Surgery, Interventional Radiology, Nephrology, Infectious Disease, Neurology, a Hospitalist service, Medical Oncology and Radiation Oncology, Dermatology, Cardiology

12. Question: What other support staff is available? (mid-levels, RN’s, etc) Answer: Four Urology mid-levels and five RNs in clinic

13. Question: What days are surgeries scheduled?

Answer: Specific Urology block dates and times will be identified once a provider has been named/credentialed; Currently Urology has OR block time on Tuesdays, Wednesdays, Thursdays and Fridays

14. Question: What procedures are required?

Answer:

Ureteroscopy and laser lithotripsy Cystoscopy and other indicated procedures (stent placement, retrograde pyelograms, TURBT, biopsy, bladder stone treatment, urethral dilation, DVIU) TURP/laser TURP Hydrocelectomy Vasectomy Circumcision/dorsal slit

ESWL

Spermatocelectomy/epdidymectomy Scrotal exploration/incision and drainage Prostate biopsy Radical or simple orchiectomy

15. Question: Are there any complex surgeries or robotic surgeries required?

Answer: Not for these providers

16. Question: Approximately how much time is spent in the clinic vs. the OR?

Answer: 30 OR/70 Clinic

17. Question: Quick question...am I reading this correctly that the facility is looking for 24/7/365 coverage?

Answer: Yes, the facility needs a total of one FTE 24/7/365. One FTE is defined by VA as a minimum of 80 hours every two weeks and does not include holidays. Regular coverage is 7:01 a.m. to 3:59 p.m. during week days. On Call coverage is 4 p.m. to 7 a.m. during week days, and all day during federal Holidays and weekends.

18. Question: Will the locum physician be the only urologist on staff?

Answer: No, there are 2 Full time and three part time urologists on staff.
