VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17 Revision 01 Eff Date: 04/23/2018
DEPARTMENT OF VETERANS AFFAIRS
Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)
Acquisition Plan Action ID: 36C249-19-AP-2474
1. Contracting Activity: Department of Veterans Affairs, VISN 09, 603 Robley Rex-Louisville
VAMC PR# 603-19-3-984-0275
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
Requirement is for one Spyglass Spyscope Capital for use in the Endoscopy Department. The estimated value of this procurement is $100,000.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Market research was conducted to determine potential vendors to support this requirement. VIP search returned 0 results. This product is the only one available that allows physicians to directly visualize the target site, obtain a diagnostic tissue sample with the disposable SpyBite Biopsy Forceps, and deliver therapies such as laser and EHL. Additionally, without the SpyGlass System, direct visualization of the pancreatico-biliary system cannot be performed, due to the significant limitations of traditional cholangioscopy systems. This system allows physicians to make diagnoses of patients without the need for exploratory surgery.
4. Description of market research conducted and results or statement why it was not conducted:
Market research was conducted through searches of GSA Advantage, FPDS, and NAC.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
