Q1. The performance work statement states that there are 696 roof tie anchors. Are they all the same type of anchor? The performance work statement also states that there are horizontal lifeline systems. Are these separate from the 696 roof tie anchors?

Response: We believe the horizontal lifeline system is separate. Please refer to the drawings provided for further details/clarification.

Q2. Is there a specification or detail breakdown of the fall protection anchor system(s), the roof tie anchors and the horizontal lifelines, that needs to be certified and inspected?

Response: See drawings provided with this amendment.

Q3. Is there a fall protection anchor layout map of the system available?

Response: See drawings provided with this amendment.

Q4. Can some example images of each anchor type be provided?

Response: See attachment (marked pictures) and the attachment for drawings provided with this amendment.

Q5. Can any existing certification or inspection documentation be provided?

Response: The following is a summary of the last inspection that occurred in August 2018.

Q6. Can the construction/installation plans of the anchors be provided?

Response: See drawings provided with this amendment.

Q7. F.1.2 states "Contractor is responsible for any damages caused by contractor to Government property during execution of work and shall repair at no cost to the Government." If the anchor breaks or is damaged at all due to testing, is it the contractor responsibility to replace at contractors cost? Does that include reroofing as well ?

Response: It depends. For example, the contractor would not be held liable for damages during load testing because of anchor or rigging-sleeve failure as long as the contractor is performing in good-faith without fault or negligence. On the contrary, if the contractor caused damage(s) as a result of negligence (e.g. the contractor exceeded the planned load test weights due to operator error or the contractor drove their vehicle through the grass and destroyed sod – then yes, we would hold the contractor liable for loss or damages.

Q8. General Requirements on page 7 states for 5,000 lb testing. The working load is 1250. We typically test at double the working load at 2,500 lbs per industry standards. Is that sufficient for the VA requirements ?

Response: PWS has been revised to remove the testing weights. See PWS Revision 2 for changes.

Q9. Would you be able to provide the previous inspection/testing/certification report for the fall protection anchorages for our review?

Response: I’ll provide an overview of the certification findings from August 2018. The (544) bolt through roof anchors, (146) wall anchors, and (20) rigging sleeves pass testing and were certified.

The horizontal lifeline system was not certified for use. Findings state: The existing horizontal lifeline systems are equipped with a Tirsafe inline energy absorber as manufactured by Tractel and detailed in a technical sheet. Per the technical sheet, the Tirsafe energy absorber was designed to accommodate (3) fall protection users. However, the copyright issue date of the technical sheet is 2007, issued prior to the ANSI Z359.6 “Specifications and Design Requirements for Active Fall Protection Systems” issued in 2009, later updated in 2016. Neither of the Z359.6 standards are referenced on page 1 of the Tirsafe “Applicable Standards” section. Key data such as maximum user capacity, compatible end user equipment, average arrest force, and maximum free fall are not indicated on the technical sheet and the contractor was unable to qualify performance of the system with the Z359.6 standard. The contractor was unable to certify that the Tirsafe system complies with current fall protection standards. The contractor requested supplemental information from Tractel; however, to our knowledge no response was received from Tractel.

Q10. As well as confirm the roof deck construction/information?

Response: See attached drawings

Q10. Are there lifelines on the water tower & is it included in this action?

Response: No – the water tower is not included in this action. The VA verified there are no lifelines on the water tower.

Revision/changes to the Performance Work Statement (PWS), shown below:

PWS revision 1, see Section B.3 PWS, paragraph I. INTRODUCTION (RFQ page 7).

From: The Orlando VA Medical Center (OVAMC) requires a one-time service to inspect and recertify the active fall protection system installed on Buildings 1 and 2. A total of 696 roof tie anchors need to be inspected. OSHA guidelines requires fall protection building anchors recertification every 5-years by a Qualified Fall Protection Engineer to ensure their continued safety and integrity for use in active fall protection systems. Previous certification was completed August 2018.

Change to: The Orlando VA Medical Center (OVAMC) requires services to inspect and recertify the active fall protection system installed on Buildings 1 and 2. The fall protection system consists of (544) roof mount anchors, (146) wall mount anchors, (20) non-structural rigging sleeves, and a horizontal lifeline system. All services shall be conducted in accordance with current OSHA and ANSI recommendations. Previous certification was completed August 2018.

PWS revision 2, see Section B.3 PWS, paragraph II. GENERAL REQUIREMENTS (RFQ pages 7-8).

From: An anchor point consists of three main sections: opening for the tie-off, the anchor structure and the connection to the structure. The contractor shall furnish all labor, material, tools; equipment, fuel, supervision, and insurance required to acquire the necessary data, complete existing fall protection anchors will be load tested in tension to 5,000 pounds each and visually inspected at the conclusion of the testing. The existing horizontal lifeline systems will be load tested in tension to 1,800 pounds each and visually inspected at the conclusion of the provide services in accordance with industry standards. To simulate the worst case for loading, the anchors will be tested perpendicular to the connection ring. The contractor shall prepare a formal engineering report documenting observation and testing results.

Change to: An anchor point consists of three main sections: opening for the tie-off, the anchor structure and the connection to the structure. The contractor shall furnish all labor, material, tools; equipment, fuel, supervision, and insurance required to provide services in accordance with industry standards.

Section B.3 PWS, paragraph C. DELIVERABLES (RFQ pages 8-9).

From: The Contractor shall thoroughly examine and test the anchors to ensure their continued safety for use in active fall protection system. A thorough visual examination, checking for signs of wear, corrosion, damage to the anchors, proper placement and load tested to verify its structural integrity and capacity to withstand a minimum of 5,000 pounds of force.

· Completion of the report (testing results) shall be delivered no later than 15 days after the test date.

· Annual inspections shall include all necessary documentation required by OSHA so that they can be stored on-site with Hard copies (1) & Electronic copies (1) to ensure that the OVAMC is compliant.

Change to: C. DELIVERABLES. (Add paragraphs for Deliverables 1 through 5 below)
