B.4 STATEMENT OF WORK
STATEMENT OF WORK
Corindus Corpath GRX Vascular Robotic System
BACKGROUND:
The Corindus CorPath GRX Vascular Robot System consists of a radiation-shielded cockpit, a robotic arm, and a drive cassette that robotically performs the surgical procedure.
Salient Characteristics of an Equal Brand:
Bedside Unit Articulated Arm (inclusive of the Robotic Drive) The Articulated Arm is mounted and locked on the rail of the procedure table through the use of the integrated rail clamp and bed support to support the Robotic Drive. In addition to its mechanical function, the Articulated Arm serves to provide the communication and electrical connections between the Robotic Drive, Control Console, and power supplies, through its Junction Box and shielded cabling. Communications and power are brought to the Robotic Drive via a connector located at the end of the shielded cable.
The Robotic Drive is an electro-mechanical assembly which receives inputs from the Control Console. The Robotic Drive moves forward and backwards to advance and retract the guide catheter. A Bedside Touchscreen on the Robotic Drive provides the user interface for the bedside operator during system set-up, loading and device exchanges.
GRX Single Use Cassette or equal The single-use Cassette is provided sterile and is intended for single patient use only. It attaches to the Robotic Drive and can be loaded with commercially available guide catheters, guidewires, and balloon/stent catheters. This Single-use Cassette advances, retracts, and rotates the guidewire, advances and retracts the balloon/stent catheter, and rotates the guide catheter.
Remote Workspace Interventional Cockpit The Interventional Cockpit is a portable, locking-wheel cart that provides a radiation shield for the physician and houses the Control Console and video monitor mounting pole. It is constructed from lead–containing materials and wood laminate with a sturdy steel frame and hand rails on both sides. One half-inch thick lead acrylic provides additional radiation protection at the front and left side of the Interventional Cockpit above the frame without occluding physician’s view of the procedure table.
The Remote Workspace is also designed to accommodate third-party manufacturers’ angiography, hemodynamic monitors and X-ray foot pedal to afford the operator improved visualization and control of the PCI or PVI procedure. Additionally, the Workspace can accommodate third party contrast injectors and balloon inflation controls.
GRX Control Console or equal The Control Console enables the physician to remotely manipulate the guide catheter, guidewire and balloon/stent catheter. The Control Console has a touchscreen and three joysticks: one joystick for balloon/stent catheter manipulation, one for guidewire manipulation, and a third joystick for guide catheter manipulation. Each joystick allows for control of linear motion (advancement and retrieval) of its respective devices. The guidewire and guide catheter joysticks also allow for rotational movement (clockwise and counterclockwise) of the guidewire and guide catheter, respectively. The devices are controlled independently which allows operations to be performed individually, by using one joystick at a time, or simultaneously, by activating all three or any two joysticks together at once. For the guidewire, balloon/stent catheter, and guide catheter can each be manipulated in small measured increments via the touch-screen of the Control Console.
If the physician chooses to be scrubbed during the procedure, the Control Console can be covered with a clear sterile drape (provided by Corindus) that covers all surfaces but allows manual access to the joysticks and the touch-screen.
The Vascular Robotic System Console and Robotic Drive work together via a communication cable. The physician’s commands are communicated to the Robotic Drive using either the touchscreen or the joysticks. Control Console Touchscreen operation advances and retracts the balloon/stent catheter, guidewire, or guide catheter in discrete 1mm steps. The Control Console Touchscreen can also be used to rotate the guidewire in 30° increments. The joystick controls maneuver the catheters or guidewire in a continuous mode.
The Touch-screen enables incremental positioning of the guidewire and Balloon/Stent catheter, and visual feedback on the position and linear/rotational speed of the devices via a custom Graphical User Interface.
The Control Console has three joysticks: one joystick for balloon/stent catheter manipulation, one for guidewire manipulation, and a third joystick for guide catheter manipulation. When a given joystick is moved forward (away from operator) the guide catheter, guidewire, or balloon/stent catheter will advance into patient. If the joystick is moved back (toward operator) the guide catheter, guidewire or balloon/stent catheter will retract. The farther the joystick is moved forward or backward the greater the speed of advancement or retraction.
Angiographic Monitors, Hemodynamic Monitors and X-ray Foot Pedal The Interventional Cockpit is built with mounting features to accommodate a video display of angiography and hemodynamic data directly in front of the seated operator. The Interventional Cockpit also houses an X-ray foot pedal, in addition to the X-ray system foot pedal positioned at the bedside. This set up provides the operator duplicate angiographic images and informational feedback from the X-ray system monitors while seated at the Interventional Cockpit. These video signals are directly connected to the X-ray system and are not electronically interfaced to the Vascular Robotic System.
OBJECTIVES:
The robotic system is a highly specialized, technically advanced system that has been shown to reduce stent usage and radiation exposure to the cardiologist and cath lab staff. Although the procedure can be performed without a robot, the use of the robot decreases overall stent usage due to precise computerized measurement of coronary stenosis length, and substantially reduces the risk for workplace injury (the cardiologist can work in the cockpit without wearing lead) and radiation exposure (the cockpit is radiation shielded and is placed several feet away from the radiation source). Having this piece of equipment will offer the opportunity to become a center for excellence and national training center for other VA facilities.
SCOPE:
The vendor will ship and install the robotic system, providing all parts and supplies needed to install the equipment. The vendor will ship in the supply of consumables needed, and vendor will provide in servicing/training for the staff.
TASKS OR REQUIREMENTS:
Vendor will provide at minimum 2 complete printed sets as well as electronic versions of operator manuals.
Vendor will provide a clinical expert assigned to the account to provide detailed training, support, and clinical guidance to maximize the clinical benefits of the system.
Service needs are sourced through vendor expert Service Team. All costs associated with servicing and repairing the system, along with any software upgrades, are included for the first year with purchase.
DELIVERABLES OR DELIVERY SCHEDULE:
The Vascular Robotic System is delivered and installed at the customer’s site by vendor and their team of fully trained, certified personnel with an estimated 3 day install time required. Disposables required for systems use are shipped to the facility logistics department by vendor in preparation for use.

C.3 52.217-6 OPTION FOR INCREASED QUANTITY (MAR 1989)
The Government may increase the quantity of supplies called for in the Schedule at the unit price specified. The Contracting Officer may exercise the option by written notice to the Contractor within 15 Days. Delivery of the added items shall continue at the same rate as the like items called for under the contract, unless the parties otherwise agree.

(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.
(c) All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the Contractor's business records or other proprietary data reviewed or obtained in the course of assisting the Contracting Officer in assessing the Contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs.
(d) Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the Contractor to protect proprietary information as required by FAR 9.505-4, Obtaining access to proprietary information, paragraph (b). The Contractor is required to cooperate fully and make available any records as may be required to enable the Contracting Officer to assess the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

0. All email responses shall reference the RFQ number “36C24821Q1509” in the email’s subject line. Emails with attachments may not exceed 5mb. If your submission is larger than 5mb, you may send multiple emails – please add the following label to the email’s subject line, e.g. ‘1 of 3’, ‘2 of 3’, and so on.
1. Faxed offers are NOT permitted under any circumstances.
1. Contractors’ Response to Quote: Each RFQ will specify its corresponding response time for either a quote or a “No Response” statement from Contractors.
0. FORMAT
2. All commitments made in the quote may become a part of the resultant contract. The data submitted with each quote should be complete and concise, but not overly elaborate. Excessive reliance on promotional brochures is discouraged.
2. Electronic Copy: Provide one (1) electronic copy of the complete quote. Electronic copies must be in Microsoft Word or Adobe Acrobat format, and virus checked prior to submission.
i. Volume 1: Pricing Information. Quoter is to complete Section B.2 Price/Cost Schedule or submit a separate attachment.
ii. Volume 2: Technical Information: Conforming to solicitation statement of work, delivery, cost and price schedule.
c. Quoters, at the discretion of the Government, may be asked to provide information for clarification purposes regarding their quote. Requests for clarification information do not constitute discussions.
d. The Quoters shall be held responsible for the validity of all information supplied in his/her quote, including information provided by potential subcontractors. Should subsequent investigation disclose that the fact and conditions were not as stated, the quote may be rejected and receive no further consideration.
e. If revised quotes are requested, the number of revised copies to be submitted shall be the same as those of initial quotes.
f. Submissions can be sent in multiple emails to avoid computer system email size limitation, to CO. Quotes submitted by any other method will not be considered. All proprietary information must be clearly marked. The use of hyperlinks in quotes is prohibited. Late quotes will not be accepted for evaluation. The contractor is responsible for confirming the Government’s receipt of the contractor’s quote.
4. VOLUME REQUIREMENTS
a. Volume 1 – PRICING: Quoters shall submit a signed SF 1449 (blocks 30 a, b, and c), a completed Section B.1 Contract Administration Data (fill-ins), signed amendment(s) or acknowledgement of amendment *completed Price/Cost Schedule in Section B.2.
b. Volume 2 – TECHNICAL – General Instructions: The Technical volume should include necessary information regarding the offeror’s ability to perform all requirements outlined in the solicitation. Failure to provide a Technical quote in accordance with the solicitation instructions may render a quote incomplete and ineligible for award.
Technical quotes shall not include price or pricing information. In no case, shall statements such as “we comply with the requirements of the contract” or its equivalent be acceptable to meet the requirements of this request for quot. The offeror should not simply rephrase or restate the Government's requirements but shall provide a rationale or explanation to convince the Government that the offeror’s approach will meet the Government requirements well. The level of detail in a technical quote is significant. A technical quote of the work described in the SOW shall be provided in adequate and precise detail to establish with a high level of confidence that the Contractor understands the technical merits, scope, and complexity of the work to be performed. Additionally, the information should be presented in a way that best highlights the quoter’s competitive advantage for the services being requested and that the Contractor has the capability to accomplish the work well within the time frames established. To that end, the quote shall also include enough detail of supporting information that clearly and concisely substantiates the validity of stated claims/capabilities. The Government may consider the quoter’s assertions or merely restating or paraphrasing the Governments requirements without further explanation, clarification, or context to be weaknesses, deficiencies, or, if such content is substantial, it may warrant a quote’s evaluation of unacceptable.
1. COMPANY EXPERIENCE - Quoters shall submit detailed information on company experience.
1. AUTHORIZED LETTER – If quoter is a direct authorized distributor, quoter shall submit an authorized letter from the OEM authorizing them to provide the items/services in compliance with SOW and Limitations of Subcontracting.
NOTE: Quoters who fail to submit required documentation may be considered ineligible for award.

E.2 52.209-7 INFORMATION REGARDING RESPONSIBILITY MATTERS (OCT 2018)
(a) Definitions. As used in this provision— "Administrative proceeding" means a non-judicial process that is adjudicatory in nature in order to make a determination of fault or liability (e.g., Securities and Exchange Commission Administrative Proceedings, Civilian Board of Contract Appeals Proceedings, and Armed Services Board of Contract Appeals Proceedings). This includes administrative proceedings at the Federal and State level but only in connection with performance of a Federal contract or grant. It does not include agency actions such as contract audits, site visits, corrective plans, or inspection of deliverables.
"Federal contracts and grants with total value greater than $10,000,000" means—
(1) The total value of all current, active contracts and grants, including all priced options; and
(2) The total value of all current, active orders including all priced options under indefinite-delivery, indefinite-quantity, 8(a), or requirements contracts (including task and delivery and multiple-award Schedules).
