ATTACHMENT 4

52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014), The Government will award a firm fixed price contract, resulting from this solicitation based on the lowest evaluated price of the offers meeting or exceeding the acceptability standards for nonprice factors, to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) technical capability to meet the Government requirement; (ii) price; and (iii) past performance. Nonprice factors will be rated on an “Acceptable/Unacceptable” basis. Failure to meet any of the requirements will result in a “Unacceptable” rating and eliminate the offeror from further consideration for contract award. To be eligible for award, an offer must be rated no less than acceptable in the non-price factors. For price evaluation purposes, the lowest price technically acceptable offer will be selected for award. Offeror quotes shall be evaluated under FAR Part 13.106-2(b) -- Evaluation of Quotations or Offers. The offeror’s quote shall be evaluated to determine if offer has the capabilities to provide Tuberculosis (TB) Isolation Inspection & Certification Services as detailed in the Statement of Work. To be considered technically acceptable for award each offeror must provide with their submitted quote/offer:

1. Technical Factor - Provide information on your company’s ability to perform the services. Describe how your company will perform the services described in the Statement of Work: Limit to 15 pages.

Offeror shall address as part of their quote, the following information. This information shall constitute the technical:

A . Statement of work.

Meets all description of work requirements functionality and performance specification for inspection and testing of HEPA exhaust units, laminar flow hood and biological safety cabinet certification.

SOW Paragraph: C Required Documentation: Provide documentation describing services that shall be performed under this contract.

B. Deliverables – This factor evaluates the delivery terms to determine the offer response timeframe. The Government may determine an offer unacceptable if the proposed material, parts, labor and travel cost are not included in certification and inspection.

a. The offer has the ability to schedule certification and inspections for all services required under this contract.

SOW Paragraph: C Required Documentation: Provide documentation describing proposed cost of inspections and certifications in accordance with attached Price Schedule.

C. Experience. Offeror exhibits at least three years’ experience and/or operation in the field of tuberculosis, isolation control, biological safety cabinet certification , microbiological and USP 797 testing.

SOW Paragraph: G Required Documentation: Provide copy of documents evidencing business incorporation date, or documentation evidencing experience (Can include key management involved in daily operations).

Provide copies of certifications and/or other credentials for service technicians.

D. Emergency Repair Deliverables – This factor evaluates the emergency repair delivery terms to determine the service response timeframe for emergency service.

SOW Paragraph: C: Routine and Emergency Services beyond preventative maintenance Required Documentation: Provide documentation describing proposed after hours and 24 hours response travel and labor cost and emergency phone numbers.

Note: Parts, labor and material will be determined fair and reasonable prior to repair approval. Offer shall not list price of parts, labor and material in Price and Cost Schedule.

2. Experience- The quoter shall provide a minimum of three past references for work similar in price and technical scope. References shall identify the agency name, point of contact, phone number, contract or delivery order number, a brief scope of contract, and contract value. The references shall have occurred within the last 5 years of the date of submittal. The government will assess contractor’s experience and consider past performance for award consideration.

3. Price- An Acceptable Price/Schedule shall illustrate the basis for pricing including labor, equipment, materials, and all other items and services necessary to perform the work. Price shall be submitted on the attached Price Schedule.
